Showing all 18EE and foreman were moving miner cable. EE fell backwards while holding the cable. He stated he felt pain in his back. This EE had a prior injury to his back in the same area on 11/17/11. This is possibly a continuation of this same injury.
The EE was using a piece of diesel equipment (low trac) The EE had stopped the Eq. and while getting out of Eq. he slipped and fell hitting his left hand against the machine, which resulted on a break to the 3rd finger on his left hand. On 12/6/11 the Dr. determined that minor surgery would be required to repair the break to the EE's finger.
EE was hanging a curtain when he lost his footing and fell on his left shoulder. He fell against the rib and was complaining of shoulder pain. EE went to the doctor and he continued to work at his regular job until 10/5/11. He had surgery on 10/5/11.
Employee was loading supplies for the roof bolter. As he was turning around, he lost his balance and fell twisting his back as he fell. He continued to work until 06-06-11 when he went to Pineville Hospital. He then went to see another doctor on 6-7-11. This doctor had him off work until 6-9-2011.
EE was helping move miner cable out of roadway in the #5 entry, when they started moving the miner cable caught his feet knocking them out from under him. This caused him to fall on his left hip, hurting his lower back. The EE did not go and see a doctor until 12/17/10, the doctor took him off of work.
Employee was on off side of beltdrive rockdusting. Accident site was muddy. He slipped and fell backwards. Trying to catch himself with left hand he fell over own SCSR on right side of lower back.
Employee was getting out of shuttle car when he stepped into a hole twisting his ankle and breaking it.
EE was turning around in shuttle car deck, when he lost his footing, falling into the instrument panel and struck his left rib cage.

Employee was holding slate bar while unrolling cable off of spool. The spool caught the bar and threw employee down. When he fell the slate bar cut his right hand palm. He received 2 stitches.
