Employee was in the process of breaking slate that had fell on EE's drill steel when a small piece contacted EE's left knee causing EE to twist EE's left knee. An MRI revealed a tear to the left knee.
Employee was greasing the feeder when EE slid down a pile of material beside of the feeder causing EE to strain EE's back. EE continues to perform EE's normal duties but started physical therapy on 8/18/25.
Employee was walking to bolter and slipped on a wet rock that caused employee's knee to buckle under employee. Employee was treated for a strained knee. The employee became incarcerated on 2/13/25 for an undetermined amount of time.

Employee was taking bolts out of a cover, when the bolt came loose employee slipped and twisted falling backwards left foot did not move. Resulted in twisting the knee. MRI reveals Near complete to complete ACL tear.

Employee was getting out of a shuttle car when foot slip causing EE to fall against the coal rib cutting left hand. The cut required stitches.
Employee was hooking a chain up to wire mesh and stepped back tripping over a piece of wire mesh. The employee started missing work on 3/15/16
Employee was walking over the miner cable and tripped falling into the coal rib hitting left shoulder and arm. After investigations of all avenues it was determined that injury was not preexisting.
Employee was walking in front of the sub at the slope project and step into a hole twisting his lower back. After a thorough investigation; we deemed it a reportable accident as of 6/3/2014.
Employee was carrying a bundle of pin plates in each hand to the roof bolter and tripped over a cable bolt and landed on his right lower back. On 8/7/13 employee reported an injury that the operator deemed questionable. On 10/17/13 the employee was referred to a specialist that reported the employee did in fact have an injury and that may have been treated incorrectly.
Employee was helping hang the miner cable when he twisted his ankle causing him to fall and facture his left ankle.

While attempting to stop a leak on the refuse box, two workers were going to apply a patch by using a piece of belting and metal. Co-worker climbed up a permanent ladder and injured employee grabbed the closest ladder, the upper part of an extension ladder. As he climbed up the ladder, it slipped causing him to fall.
Employee was walking around a pillar to get a tool and trip over a rock and fracture his elbow.
EE tripped & fell walking from parking area to bottom. He hung his foot in something didn't know what it was. He hit his left shoulder and ribs.

Pressure washer had quit. Worker climbed into back of truck, fixed the problem by securing a wire, turned around & fell off the truck. Initially, this was not a reportable accident because worker did not seek med. treatment & no time was lost. Worker did not seek med attn until 9/14/10 & was placed on restricted duty. Effective 9/20/10, he was off work completely.
While examining the maintenance shop for day shift, ee stepped off a bank which caused his knee to twist. A MRI was performed and found he has a torn meniscus.
He stepped off his ride , in a hole and fell. Later Doctor report showed that he actually had a baker cyst on this knee.
