Employee was in the process of starting the last row of bolts in the #3 face on the #4 unit when a piece of slate approximately 1'L x 1' W, 1/4" T(Estimated weight of 6/12 lbs) fell from 6"-12" above EE's head contacting the back of EE's head and back resulting in muscle soreness.

Employee was in the process of reclaiming HV cable. While putting a loop of the cable in a tub EE felt a sharp pain in EE's lower back. EE is being treated for sciatica.

Employee had connected the miner tail with a chain to the front bumper of a shuttle car to attempt to free the car from being stuck. Once the chain became tight the chain came loose contacting the employee in the lower back resulting in a fractured vertebra.

Employee was backing the bolter out of the #7 entry when the cable came off the side and ran under the cab. Employee reached from the deck behind the bolter to adjust the cable while backing up and contacted a de-energized car with the back of the bolter catching their left arm causing a laceration/ avulsion that was surgical repaired.

Employee was loosening a J-bolt on a bottom belt hanger bracket with right hand and left hand contacting the top chair for leverage. While loosening the bolt left small finger accidentally contacted the top roller causing a friction burn from contacting the roller. They received three stitches to the finger.

Employee had finished bolting the #6 entry on the #4 unit and was standing beside the roof bolter between the dust box and the boom when a hose fitting broke causing the employee to get oil in their eyes. Employee was treated for a scratch to their eye.

While testing the drum brake on the Hanson hoist it was discovered to allow the drum to move while under a load from the electric motor. The drum was found to have a residue that was removed and the system then passed the performed test.

Employee was drilling the roof when a piece of slate broke loose and came down the drill steel revealing a small nodule that also came loose from the roof and contacted the top of the employees left boot. No fractures found and the employee was treated for a bruise to EE's left foot.

Employee was attempting to remove a steering jack pin with a sledge hammer. During the process right index finger was caught between the handle of the hammer and the frame of the scoop causing a laceration. They continued to perform their normal duties until 8/13/25 when they were treated for a fracture and given 5 sutures for the laceration.

While picking up the plate with EE's gloved right hand, EE received a small scratch to EE's ring finger that later became infected. EE is being treated for the infection and was taken off work.
