Showing all 7The employee was getting out of triaxle truck at the bed wash. The employee's foot slipped when employee stepped onto the ground. The employee fell backwards onto employee's shoulder. The employee did not seek medical attention until 12/27/23. Employee received an x-ray on 12/27/23 & it showed employee fractured left clavicle.
Employee was walking from the scale house to personal vehicle and slipped on some snow covered ice and fell to the ground onto right side.
Employee was flushing out the power washer water tank with a 2 inch water hose. EE was using bleach to clean the tank. When EE pulled the hose out of the tank, the pressure of the hose pushed EE back and caused EE to become unbalanced. EE fell approx. 3 feet from the top of the toolbox to the ground. EE fractured 2 ribs and pulled a groin muscle.
Mechanic was standing on the rear bumper area of a Euclid R40C water truck and was connecting air lines to the spray valves. He reached into his pocket to get a wrench and inadvertently stepped back and his foot slipped off the bumper area. He fell to the ground and landed on his right side, causing fractures to his ribs.
Employee lost footing while climbing up onto excavator to start the machine at the beginning of the shift. Site was wet and muddy. Employee twisted back when he slipped. This caused pain in lower back. Employee continued to work and did not have medical attention until 12/14/12. Doctor reported strained muscle. Employee did not miss any scheduled work.
Employee slipped on ice covered with 3 inches of snow and twisted left knee.
