Employee walked through wing curtain and lost his balance falling on self-rescuer and bruising his hip.

EE was starting to walk beneath a belt underpass guard and hit his left shoulder on the guard causing him to loose his balance, and fall. The area was muddy and he had his coat and lunch bucket in his hands. When he fell he broke the lower end of his fibula on his right leg.
Employee was attempting to cross over a belt and stepped on a square timber lying on the mine floor. The timber rolled pitching the employee forward against the belt and framing. The employee struck his left knee against tubular belt framing.
Employee was stepping down off of Gob truck and missed bottom step with his foot causing right knee to hit step. Employee did not start missing work until July 17, 2005.

INJURED PARTY HAD JUST GOT THROUGH LOADING SUPPLIES WITH THE FRONT ENDLOADER ONTO A TRAILER WHEN HE WAS CLIMBING DOWN TO GET OFF HIS FOOT SLIPPED ON A STEP CAUSING HIM TO STRAIN HIS BACK.
THE INJURED PARTY WAS GETTING OFF THE D-8 DOZER WHEN HE SLIPPED ON THE DOZER ARM OF THE BLADE. INJURED PARTY STRADDLED THE BLADE ARM AND CAUGHT HIMSLEF. THE JOLT CAUSED PAIN IN HIS LOWER BACK AND RIGHT LEG.
INJURED PARTY WAS BOLTING IN THE #3 ENTRY. WHEN HE GOT FINISHED HE STEPPED AWAY FROM THE ROOF BOLTER AND STEPPED ON A ROCK CAUSING HIM TO TURN HIS RIGHT ANKLE.
INJURED PARTY WAS WALKING THE 1ST SOUTH BELT WHEN HE STEPPED ON A SLICK SPOT AND FELL. HE LANDEDON HIS BACK INJURING HIS LOWER BACK.
