Employee was helping lift a 60 H.P. deep well pump to sit on a wooden block when he felt pain in his groin area.

A roof fall has occurred in the 1st Northwest Sub-mains at Spad 28+00. The dimensions of roof fall are 180' long, 19' wide, and 6' high.

A roof fall has occurred in the Second Southeast Mains in No. 3 entry at Spad 23+10. The dimensions of the roof fall are 50' long, 18' wide and 5' high.

A roof fall has occurred in the second Southeast Mains between No. 4 and No. 5 entries, at crosscut 32. The dimensions of the roof fall are 35' long, 18' wide and 10' high.

Employee had put golf cart on charge in golf cart charging station. As he started to walk away he hit a fire suppression nozzle suspended from mine foor which resulted in a laceration to his left ear which required 13 stitches.

Employee was installing roof bolt and as he started to raise boom to insert bolt into roof the other boom was swung hard by other operator. The roof bolt came out of the chuck and struck employee on left wrist causing pain.

Employee was installing roof bolt. Pushing roof bolt up into hole he had his left hand on the bolt. He accidentally started the rotation causing his hand to twist around the bolt resulting in a sprain to ligaments in hand and wrist.

A roof fall has occurred in the main east, No. 6 Entry, Spad 54 + 15. The dimensions of this fall are 20' long, 18' wide and 8' high.

Employee was rockdusting belt entry and rockdust hose caught on something. He jerked on hose to free it and felt pain in lower back. He did not start missing work until september 13.

Employee was changing shuttle car tire and it fell over. He and another employee were lifting the tire back upright when he felt pain in his lower right abdomen and later in his groin.

Employee was pulling shuttle car cable out of reel. He later felt pain in groin at the site of an old hernia where he had surgical repair three months ago, at a previous employer.

EE was picking up pull chain on shuttle car and twisted right knee.

EE was walking through curtain & struck his head on a roof bolt in the mine roof, jamming his neck & causing pain in his neck.

EE lifted a 5 gallon bucket that had bolts in it. (about half full). Felt pain in his lower back.

A roof fall has occurred in the second southeast mains in no. 5 entry at spad 25+20. The dimension of the fall is 40' long, 18' wide and 12' high.

Employee was in bath house and bumped against hook that holds clothes baskets aloft. Hook disengaged and empty metal clothes basket fell striking him in left eye resulting in laceration to eye lid.

Ee was climbing up on dozer and his foot slipped, causing his left knee to strike cleat on dozer track. Impact was at lower edge of left knee, resulting in severe pain.

EE was splicing cable and knife slipped, resulting in a cut on left thigh, which required five stitches.

EE had just dumped his car at feeder and was turning around in operator's compartment, when another car cable anchored next to him, jumped to the side hitting him on the right side of the head, driving his head into edge of his car canopy. Resulted in lacerations, requiring stitches.

Helping build crib with co-worker and crib block fell mashing little finger on left hand.

A roof fall has occurred int he first southeast maws, in no.1 entry inby spad 10 & 50. The dimensions of the roof fall area 50' long, 18' wide and 19' high.

EE was helping splice belt and usually a piece of diesel equipment was out of service. EE got a golf cart to use for transportation. While traveling down inclined travelway, cable broke on braking system. EE jumped off golf cart and bruised his left hip.

Employee was on his knees and in process of installing a roof bolt. When he started to get up his right knee popped and locked into one position and could not bend.

Employee was unloading pump motor from personnel carrier and had positioned himself between a shuttle car and the personnel carrier. Another shuttle car came down the entry and did not see the personnel carrier sticking out of the crosscut about 4 feet, he hit it which in turn caught the employee's leg between the equipment.

Employee was helping splice miner cable and was kneeling on the mine floor. Whe he got up his left knee popped and started hurting. He could not straighten it.

Employee was drilling hole in mine roof to install roof bolt. Lowered roof bolting machine boom temporarily catching left knee under it, resulting in a strain to knee area.

Employee was driving 14-man personnel carrier (mantrip bus) and ran over something in roadway causing him to bounce up into mine roof and hurt his neck.

Employee got off his shuttle car and sat down next to coal rib while waiting for continuous miner to move. When he got up he lost his balance and twisted right knee causing a sprain.

Employee was operating continuous mner and reached down to lift miner cable and felt pain in lower back.

A roof fall has occurred in the main east entries at spad 25 + 45 in No 1 entry. This roof fall is in the intake. The dimensions of the fall are: length 25' x width 19' and height 7'.

Employee was helping change out a set of scoop batteries and got his right thumb caught between connector on battery and arm that holds battery.

Employee was helping pick up belt and structure to put footers under belt stands to level beltline when he felt pain in lower back.

Employee was helping load pinner pot on the personnel carrier to take underground. When he and another employee lifted the pinner pot he felt pain in lower back.

Employee walked through wing curtain and lost his balance falling on self-rescuer and bruising his hip.

Employee was helping pull a shuttle car drag across mine floor to install it on a shuttle car when he felt slight pain in his groin. Employee did not start missing work until July 15, 2006.

Employee was walking a belt entry, examining the belt, and felt pain in his right hip. The pain got worse as the shift progressed. There was no incident to identify as the cause of the injury.

Pulling on cable and chains on previous shift when miner was hung, his back started bothering him. Then tonight while lifting bundle of glue from pinner, his back started hurting him bad.

Employee was working as extra man while unloading supplies. Felt pulling in lower back.

Mechanic was checking cable on shuttle car that was not picking up cable. He took cover off and moved car up because line curtian was in the way. Reel unexpectedly took up tripping him causing him to fall backwards on his rescuer.

EE was loading bolting machine with glue when he felt pinch in his lower left back. He thought he would be ok went home came back to work the next day worked approx. 1/2 shift when he began hurting bad enough he could not stand it anymore he was brought out and taken to ER by company offical.

EMPLOYEE WAS TOSSING A PACKAGE OF GLUE ACROSS THE TOP OF HIS ROOF BOLTER TO THE OTHER ROOF BOLT OPERATOR & HE FELT A PULL IN HIS ABDOMEN.

EMPLOYEE WAS CUTTING A BELT WITH A BELT KNIFE & AS THE KNIFE NEARED THE EDGE OF THE BELT IT CUT EASIER & WITH THE PRESSURE HE WAS APPLYING IT CONTINUED TO CUT HIS KNEE ON HIS LEFT LEG.

EMPLOYEE WAS DRAGGING 2 ROLLERS UP BELT ENTRY & FELT PAIN IN HIS LOWER BACK.

EE was helping mechanic splice his car cable. He had layed the cable across his knee to cut outer jacket and accidently cut his right knee.

EE was lifting a tail lift cylinder, when it slipped off his leg and caught his finger against metal.

EE was pulling with both arms on curtain, when he felt something pull in his right shoulder, causing pain.

EE hit rotation with bent steel in chuck. It came around and struck EE on his left wrist and left side of head.

A roof fall has occoured in the main east entries at spad 25 & 45 in No.5 entry. This roof fall is in the return. The dimensions of the fall are: Length 40' width 20' heigth 8'.

EMPLOYEE WAS OPERATING (DRIVING) A 4-MAN DIESEL MANTRIP. AS HE NEGOTIATED A TURN, THE POWER STEERING FAILED AND CAUSED HIM TO RAISE UP AS HE TRIED TO TURN, RESULTING IN HIS HARD HAT STRIKING THE MINE ROOF.

A roof fall has occurred int he main east entries at spad 38 & 75 in no. 3 entry. The dimensions of the fall are: length 18', width 22', height 8'.

EE was turning around in deck of shuttle car when he caught his left foot under the seat and twisted his left knee.

Employee had finished installing roof bolts in a place and had sat down and leaned back against roof bolting machine and took his hard hat off to wipe sweat off when a piece of top coal popped off upper rib hitting him on upper lip resulting in a cut that required two stitches.

EMPLOYEE WAS WALKING DOWN CATWALK AT RAW COAL LOAD OUT BELT AND SLIPPED AND FELL HURTING HIS LEFT WRIST.

Employee was repairing brattice that had been damaged by a piece of equipment. Several blocks had been knocked out but some were still in place at top of brattice. As employee started replacing blocks a block above him fell striking his left thumb.

EMPLOYEE WAS HELPING RECLAIM HIGH VOLTAGE CABLE AND LOADING IT ONTO A TRAILER WHICH WAS BEING PULLED BY A 4-MAN DIESEL PERSONNEL CARRIER. EMPLOYEE WAS NOT AWARE OF HIS POSITION IN RELATION TO THE EQUIPMENT AND GOT HIS HEAD CAUGHT BETWEEN FENDER AND MINE ROOF.

Employee was driving spad into coal rib when a piece of rock fell hitting him in crook of right arm and driving elbow into mine floor.

Employee was in the process of drilling a hole and reached back for a section of pinner steel when a piece of rock fell from the mine roof and struck his right middle finger causing immediate swelling and resulting in a fracture of the last segment of bone in finger.

Employee was closing metal airlock door and air pressure caused door to close faster than anticipated. His left hand was pinched between tip of metal door and mine roof resulting in laceration requiring stitches.

EMPLOYEE REACHED UP TO PULL LOOSE PIECE OF ROCK IN ROOF AND MORE MATERIAL FELL THAN EXPECTED. HE WAS STRUCK BY FALLING MATERIAL RESULTING IN FRACTURES IN HIS PELVIS.

A ROOF FALL HAS OCCURRED IN THE NO. 6 ENTRY OF THE EAST MAINS AT SPAD 38+05. THE DIMENSIONS OF FALL ARE 20' WIDE, 20' LONG AND 6' HIGH.

Employee was carrying 2 buckets of miner bits and hung his foot in loop of miner cable causing him to fall and hurt left knee.

Employee was loading pins and plates on top of roof bolter and a bundle of pin plates fell striking his right foot.

Employee was helping install waterline. A tee with the line that the fire valve attached to was leaking. When He tried to tighten the connection. The fire valve blew off the tee striking his right knee.

EMPLOYEE WAS HANGING A MINER CABLE AND FELT PAIN IN HIS GROIN.

EE WAS WALKING AROUND COAL AT FEEDER AND TRIPPED ON LOOSE COAL. HE TWISTED HIS RIGHT ANKLE.

EMPLOYEE WAS STACKING PIN PLATES ON HIS ROOF BOLTER WHEN A PIN PLATE FELL WITH THE CORNER OF THE PLATE HITTING HIS RIGHT FOREARM RESULTING IN A LACERATION WHICH REQUIRED 2 STITCHES

EMPLOYEE WAS REHANGING CABLE OVER TOP OF SHUTTLE CAR CANOPY WHEN A PIECE OF ROCK FELL STRIKING HIM ON THE RIGHT HAND.

Employee was helping thread belt through a tailpiece. He was pulling on belt with pair of channel locks when they slipped off of belt and struck him in the ribs.

A roof fall has occurred in no.7 entry of the 2nd south east mains start at spad 18+20 and gong inby for approx. 150 feet. the dimension of the fall are 20 ft wide, 8 ft high and 150' long.

EMPLOYEE WAS INSTALLING A ROOF BOLT WHEN A PIECE OF ROOF FELL STRIKING THE CANOPY THEN THE LEFT SIDE HIS BACK RESULTING IN BRUISES AND SCRATCHES

EMPLOYEE WAS HELPING CHANGE OUT BELT STANDS ON NO.2 BELT & WHILE CARRYING A BELT STAND HE STEPPED IN A HOLE & TWISTED HIS RT. KNEE.

EMPLOYEE WAS BENDING ROOF BOLTS IN PREPARATION FOR THIER INSTALLATION. HE WAS STANDING BESIDE THE ROOF BOLTING MACHINE WHEN A ROCK FELL FROM BETWEEN THE OUTSIDE ROW OF BOLTS & THE RIB, STRIKING HIM IN THE MIDDLE OF HIS BACK JUST BELOW HIS SHOULDERS.

EMPLOYEE HAD BEEN EXPERIENCING SOME PAIN IN HIS RIGHT ELBOW PRIOR TO THIS DATE. AFTER USING A COME-ALONG HIS ELBOW BECAME PAINFUL. HE WORKED HIS NEXT SHIFT BUT STARTED MISSING WORK ON 6/20/05.

EE was starting to walk beneath a belt underpass guard and hit his left shoulder on the guard causing him to loose his balance, and fall. The area was muddy and he had his coat and lunch bucket in his hands. When he fell he broke the lower end of his fibula on his right leg.

Employee was throwing loops of miner cable at slack pile as he taping cable and felt a burning sensation and sharp pain in his lower back.

WHILE HANGING MINER CABLE, EMPLOYEE PULLED GROIN MUSCLE NEAR RIGHT LEG.

Employee was attempting to cross over a belt and stepped on a square timber lying on the mine floor. The timber rolled pitching the employee forward against the belt and framing. The employee struck his left knee against tubular belt framing.

Person was cutting off metal bands from concrete block pallet and band struck the back of his arm.

Employee was working on left miner in #7 entry. He was trying to change a hose that went from conversion value to pump motor. When doing so he pulled and tugged, hurting his lower back using a pair of channel locks to break the fitting.

Employee was reporting for work on third shift and he got out of his truck. He dropped his lunch bucket on right foot. He told his supervisor what happened and that his foot was hurting. He went underground and worked, but his foot became more painful and he was brought out of the mine and sent to RMC by ambulance.

Employee was stepping down off of Gob truck and missed bottom step with his foot causing right knee to hit step. Employee did not start missing work until July 17, 2005.

EE was operating a scoop and using a cable to pull a conveyor chain back together in a shuttle car. The cable broke and struck EE in the face.

EE was walking in belt entry and stepped on a loose, wet rock, and lost his balance and twisted his left ankle.

EE WAS DRIVING SHUTTLE CAR TOWARD CONTINUOUS MINER AND RAN OVER A ROCK. EE WAS THROWN UPWARD AGAINST THE CANOPY, HITTING HIS HEAD AND JAMMING HIS NECK.

A roof fall has occurred at spad 10+50 in No. 1 entry of the 1st SW panel off the 2nd SE sub-mains. The dimensions of the fall are 18' long, 20' wide and 6' high. The fall is approximately 3400' outby No. 2 unit in the return.

Employee was dragging belt rollers down #3 belt and installing bottom rollers when he started feeling pain in lower back.

A roof fall has occurred in the #7 entry of the second southwest sub-mains at spad 0 and 70 in a return entry. The dimensions of the fall are 18' wide, 7' high and 70' long.

EE was helping splice #11 belt and when he trimmed the belt using a utility knife he cut his right thight which required 3 stitches.

An ignition occurred in the face of #5 entry on #2 unti mmu 002-0 no one was injured.

EE WAS CUTTING ON BLASTIC BOTTLE WITH UTILITY KNIFE AND CUT RIGHT THUMB REQUIRING 12 STITCHES

EMPLOYEE WAS UNHOOKING TRAILER FROM TRACTOR WHEN THE TRACTOR ROLLED A LITTLE & CAUGHT HIM BETWEEN THE TRACTOR & TRAILER. RT. SHOULDER WAS BRUISED BY IMPACT.

Employee was leaving the dumping point, in a shuttle car, and was looking back at his boom as he lowered it to make sure it cleared some cables hanging across the haul road. His car veered to the right and as he passed a parked golf cart, it struck his right arm which he had outside the operator's compartment.

The ee was operating the cont miner and backed miner up after cutting right side of face and he split the cats on the miner to move it toward left side of face. The right rear corner of the miner mashed the ee against the rib.

Employee started dragging an oxygen bottle and felt a sharp pain in lower back. He worked his next scheduled on Feb 27 but started missing work on Feb 28, 2005.

EMPLOYEE WAS LOADING BAGS OF ROCKDUST ON TO PERSONNEL CARRIER & PULLED MUSCLE IN HIS BACK.

Employee had rock fall from roof at last row of bolts and piece of rock struck him on right ankle.

EE WAS REPAIRING BOLTER CABLE THAT HAD BEEN CUT BY A CAR. AT POWER CENTER HE PUT HIS LOCK & TAG ON BOLTER CATHEAD AND SPLICED THE CABLE. WHEN FINISHED, HE TOOK HIS LOCK & TAG OFF & PUT THE CATHEAD BACK ON THE POWER CENTER. TRIED 3 OR 4 TIMES TO RESET THE BREAKER - WOULD NOT RESET, SO HE REACHED DOWN TO TAKE THE CATHEAD OFF WHEN THE BREAKER BLEW UP ON HIM.

A roof fall was found on 9-27-04 around 8:45 am by safety director in the 1st southeast #5 entry between spad 12 and 60 and spad 13 and 30 in the pillar. Fall was 24' long 13 1/2' wide and 5' high. 5' fully grouted roof bolts were used in this area.

injured party was picking up the miner cable to hang it up when he felt a pain in his lower back.

INJURED EMPLOYEE WAS CARRYING ROOF BOLTS FROM HIS SUPPLY TRAILER TO HIS ROOF BOLTER WHEN HE TRIPPED OVER LOOSE COAL AND FIRE CLAY. EE FELL AND STRUCK HIS RIGHT KNEE AGAINST THE GROUND.

INJURED PARTY SAID THAT ON 8/20/04 HE WAS DRAGGING A TIMBER DOWN BELT LINE TO SET WHEN HE FELT A PAIN IN HIS LEFT SHOULDER. HE CONTINUED TO WORK UP THE DATE OF 9/1/04 WHEN HE SAID HE NEEDED TO GO TO THE DR.

Injured party had squatted down beside the roof bolter and twisted his body to pick up some thing when he heard along pop in his left knee.

A ROOF FALL WAS DISCOVERED IN THE MAIN SOUTH RETURN AT THE PORTAL JUST INBY THE FAN HOUSE ON 7-12-04 BY MINE FOREMAN. THE FALL WAS 30' LONG, 20' WIDE AND 12' HIGH.

Injured party lifted the miner cable on #1 unit to hang, without waiting for help and felt pain in his lower back, Injured party continued to work and did not go to the doctor or miss any work until 7-2-04.

EE WAS PASSENGER IN MANTRIP WHICH LOST POWER COMING UP THE MAIN SOUTH SLOPE. DRIVER STARTED PUMPING THE FOOT BRAKE BUT IT FAILED TO HOLD THE RIDE AND THEY STARTED ROLLING BACKWARD. DRIVER PUT IT IN PARK, WHICH DID NOT HELP. DRIVER THEN SET OFF THE PARKING BRAKE BUT THEY HAD HIT THE COAL RIB ABOUT THIS TIME, INJURING EE'S RIBS.

INUNDATION OCCURRED AT THE 1ST SOUTH PANEL SEALS WHICH ARE OFF THE MAIN EAST INTAKE. FOREMAN FOUND DURING DAILY INSPECTION OF THE MINE. HE WALKED OVER TO CHECK THE #4 SEAL WHEN HE SAW WATER COMING OUT OF THE 4"" RISER. SOON AFTER HE FOUND WATER COMING OUT OF THE 4"" U-TUBE IN THE #1 SEAL. MINE EVACUATED. MSHA 103-K ORDER ISSUED; LIFTED 6/8/04.

INJURED PARTY WAS KNOCKING A SLEEVE OUT OF AN INSERT ON THE CUTTING DRUM WHEN A PIECE OF METAL POPPED OFF AND STRUCK HIM ON HIS LEFT FOREARM.

Injured party had just got out of his shuttle car when he raised up he hit a dislodged roof bolt with his neck cutting it.

INJURED PARTY SQUATTED DOWN UNDERGROUND TO WORK ON A MINE PHONE & THERE WAS A PIECE OF ROCK LYING ON TOP OF THE PHONE LINE. WHEN HE MOVED THE PHONE LINE, THE ROCK FELL & HIT THE TOP OF HIS LEFT KNEE.

INJURED PARTY HAD JUST PUT UP HIS 1ST BOLT IN THE ENTRY. WHEN HE STARTED TO SWING HIS BOOM, HE PULLED THE LEVER THE WRONG WAY, STRIKING HIS RIGHT KNEE.

INJURED PARTY WAS TAKING A BELT STAND OFF THE TRAILER WHEN HE DROPPED A BELT STAND ON TOP OF HIS RIGHT FOOT.

INJURED PARTY HAD JUST SNAPPED THE BAND ON THE UBUNDLE OF ROOF BOLTS AND HE TURNED AND GRABBED THE METAL BAND HE HAD JUST CUT WITH HIS LEFT HAND TO BEND IT BACK WHEN HE CUT THE PALM OF HIS LEF T HAND.

INJURED PARTY HAD JUST PARKED THE DIESEL TRACTOR HE WAS OPERATING & STOOD UP & STEPPED OFF THE TRACTOR WHEN HE FELT A PAIN IN HIS LOWER BACK.

INJURED PARTY WAS WALKING THE 1ST ANGLE BELT WHEN A PIECE OF ROCK STRUCK HIM ON THE LEFT SHOULDER AT THE COLLAR BONE AREA.

INJURED PARTY HAD A PIECE OF ROCK TO STRIKE HIM ON HIS SHOULDER & PULL A MUSCLE IN HIS BACK. *REASON FOR NO LOSS WORK DAYS, MINE WENT ON THANKSGIVING VACATION.

INJURED PARTY WAS LOADING BELT FRAMING INTO A TRAILER. WHEN HE PICKED UP A PIECE HE FELT A PAIN IN HIS LOWER BACK.

INJURED PARTY WAS LOADING BELT FRAMMING ON A SUPPLY TRAILER WHEN HE TURNED AND TWISTED HIS BACK.

A ROOF FALL OCCURRED IN THE #5 BELT ENTRY IN THE 2ND NORTHEAST SUBMAIN IS STARTING AT SPAD #9 & 35 TO AND INTO THE 9BY 90 INTERSECTION. 5'FULLY GROUTED ROOF BOLTS USED IN THIS AREA. MSHA INVE STIGATED. MOVED UNIT OUTBY THIS AREA 1250 TO A NEW SETUP

INJURED PARTY WAS TRAMMING THE ROOF BOLTER WHEN HE STEPPED OVER A PIECE OF COAL & HIS FOOT SLIPPED, TURNING HIS LEFT KNEE.

INJURED PARTY WAS HELPING CHANGE A STARGATE SHAFT IN TILE PREPARATION PLANT. WHEN THE HOUSING WAS TAKEN OFF THE ONE END IT CAUSED THE END HE WAS HOLDING TO RAISE UP SUDDENLY SMASHING HIS RING FINGER ON HIS RIGHT HAND.

INJURED WAS HELPING CHANGE OUT A TRAM MOTOR ON A MINER. HE WAS SITTING IN THE SCOOP BUCKET WHICH THEY USED TO CHANGE THE MOTOR WITH WHEN A PIECE OF DRAW ROCK FELL OUT BETWEEN BOLTS, MEASURING 17' LONG, 3' WIDE & 1/4"" THICK ON ONE END & 7"" ON THE OTHER END. THE ROCK STRUCK HIS SHOULDER & PUSHED HIM OVER HURTING HIS BACK.

INJURED PARTY WAS HELPING WORK ON A CONTINUOUS MINER BACK ON THE 7/25/03 WHEN HE SAID HE GOT A PIECE OF ROCK ON HIS RIGHT KNEE. THE ROCK FELL OUT BETWEEN ROOF BOLTS HE SAID. HE JUST WENT TO TH E DOCTOR THE 8/27/03 FOR THE FIRST TIME & HAS NOT MISSED ANY TIME UP TO THIS DATE OF 8/27/03.

INJURED PARTY SAID HE WAS TAKING DOWN A AIRLOCK ON THE OLD #1 UNIT WHEN HE GRABBED A HOLD OF A PIECE OF CURTAIN & JERKED IT. HE FELT A PAIN IN HIS LOWER BACK.

INJURED PARTY WAS HELPING ROOF BOLTER OPERATOR MOVE FROM ONE PLACE TO ANOTHER WHEN HE RAN IN FRONT OF THE BOLTER AND MOVED A CABLE. HE THOUGHT HE HAD GOT OUT OF THE WAY. ROOF BOLTER OPERATOR S TRUCK THE RIB WITH THE ROOF BOLTER STRIKING THE INJURED PARTY ON HIS RIGHT KNEE.

INJURED PARTY TWISTED HIS RIGHT KNEE ON 3/27/03 WHILE PUTTING IN BELT FRAMMING ON BELT LINE. HE HAD NOT MISSED ANY DAYS OF WORK UP TO 6/25 WHEN HE SAID HIS RIGHT KNEE STARTED GIVING HIM TROUBL E. HE THEN WENT TO THE DOCTOR. WE WERE UNAWARE HE HAD HURT HIS KNEE ON 3/27.

EE was working on a Mantrip in the x-cut between the #4 & #5 entry when the unit scoop turned into the x-cut on them. He did not see them working on the mantrip. He struck the injured party cutting his right leg below the right knee.

INJURED PARTY HAD JUST GOT THROUGH LOADING SUPPLIES WITH THE FRONT ENDLOADER ONTO A TRAILER WHEN HE WAS CLIMBING DOWN TO GET OFF HIS FOOT SLIPPED ON A STEP CAUSING HIM TO STRAIN HIS BACK.

A ROOF FALL OCCURED ON THE 1ST SOUTH MAIN ENTRY #1 ENTRY BETWEEN SPAD #16+10+17+50. IT WAS FOUND BY THE 2ND SHIFT MINE FOREMAN. THE ROOF FALL WAS 10' HIGH, 70' LONG, + 18' WIDE. 60"" FULLYU GRO UTED ROOF BOLTS'S WERE USED INT HE AREA. THE ROOF FALL WAS 6,997 OUTBY THE UNIT DUMPING POINT.

INJURED WAS ROOF BOLTING THE #5 ENTRY WHERE A COAL BOTTOM HAD BEEN LEFT. AS HE TURNED TO MOVE HE TWISTED HIS RIGHT KNEE.

A ROOF FALL OCCURRED ON THE 1ST SOUTHEAST BELT BETWEEN SPAD #11 & 20 &11 & 90 AT AROUND 12:30 PM 12-16-02. THE ROOF FALL WAS 18' WIDE 50' LONG & 10' HIGH. 60"" FULLY GROUTED ROOF BOLTS WERE USE D IN THIS AREA. THE ROOF FALL WAS 4,270' OUTBY THE UNIT DUMPING POINT.

INJURED PARTY WAS LIFTING 8""SOLID BLOCK STACKINGA BRATTICE WHEN HE FELT A PAIN IN HIS LOWER BACK. INJURED PARTY CAME BACK TO WORK & WORKED 5 SHIFTS & TOOK OFF AGAIN, HAVING TO DO WITH THIS INJ URY.

INJURED PARTY WAS KNOCKING BLOCK OUT OF A RET BRATTICE FOR A BELT REGULATOR WITH A SLEDGE HAMMER WHEN HE FELT A PAIN IN HIS LOWER BACK.

MINERMAN WAS OPERATING THE MINER IN THE #2 ENTRY WHICH WAS ABOUT 118' DEEP CUTTING COAL FOR THE LAST CAR WHEN HE SAW A FLAME GO DOWN THE BACK SIDE OF THE CONTINUOUS MINER. THE FLAME WENT OUT WHEN IT GOT TO THE SCRUBBER. SECTION SHUT DOWN IMMEDIATELY AND MSHA PERSONNEL WERE CALLED AND AN INVESTIGATION WAS CONDUCTED.

THE INJURED PARTY WAS DRIVING TO THE #1 UNIT ON THE MAIN EAST WSUPPLY ROAD WHEN HE RAN OVER A ROCK IN THE ROAD THROWING IT UP IN THE AIR STRIKING HIS LEFT LEG.

INJURED PARTY WAS BLOCKING UP UNDER FEEDER TO RAISE IT UP HIGHER TO GET IT ON THE TAILPIECE. THE FEEDER SLID OFF THE CRIB & CAT PAD CAUGHT HIS RIGHT HAND BETWEEN CAT PAD & TIMBER MASHING HAND.

INJURED PARTY WAS UNLOADING ROUND PROPS OFF A TRAILOR WHEN HE TWISTED TO THROW THE PROP AND FELTPAIN IN THE BACK OF HIS LEFT THIGH.

AS THE INJURED PARTY WALKED DOWN BESIDE THE #1 CAR HE NOTICED THE CAR CABLE UNDER THE CAR. HE REACHED DOWN & JERKED THE CABLE OUT FROM UNDERNEATH THE CAR WHEN THE CABLE REEL KICKED IN JERKING THE CABLE ACROSS HIS FACE INJURING HIS JAW & MOUTH.

THE INJURED PARTY WAS GETTING OFF THE D-8 DOZER WHEN HE SLIPPED ON THE DOZER ARM OF THE BLADE. INJURED PARTY STRADDLED THE BLADE ARM AND CAUGHT HIMSLEF. THE JOLT CAUSED PAIN IN HIS LOWER BACK AND RIGHT LEG.

WHILE TR4AMMING THE 480 S & S SCOOP THE INJURED PARTY EITHER RAN OVER A ROCK OR A HOLE INT HE ROAD CAUSING THE INJURED PARTY TO JAM HIS NECK INTO THE SCOOP CANOPY. THIS HAPPENED ONT HE 5-6-01. THE INJURED PARTY DID NOT GO TO THE DR TIL 5-13-02.

A ROOF FALL WAS FOUND BY THE PRESHIFTER ON THER 4-14-02 WHILE PRESHIFTING THIS MINER. THE ROOF FALL WAS IN THE 33 ENTRY IN THE SOUTHEAST MAINS AT SPAD 314+70. THE FALL WAS 50' LONG, 18' WIDE & 8' HIGH. 60"" FULLY GROUTED ROOF BOLTS WERE USED IN AREA.

INJURED PARTY WAS BOLTING IN THE #3 ENTRY. WHEN HE GOT FINISHED HE STEPPED AWAY FROM THE ROOF BOLTER AND STEPPED ON A ROCK CAUSING HIM TO TURN HIS RIGHT ANKLE.

INJURED PARTY WAS SCOOPING THE RIGHT HAND RIB WITH THE 488 SCOOP AND AS HE WAS TRYING TO SCOOP THE RIB THE VENTILATION CURTAIN WAS GETTING CAUGHT UP ON THE CANOPY. INJURED PARTY REACHED OUT O F THE CANOPY TO MOVE THE CURTAIN AND AS HE DID HE GOT HIS LEFT INDEX FINGER CAUGHT BETWEEN THE RIB AND THE SCOOP CANOPY.

INJURED PARTY HAD PICKED UP A BOX OF GLUE TO PUT IT ON THE ROOF BOLTER. WHEN HE SET IT DOWN HE FELT A PAIN IN HIS LEFT SHOULDER.

INJURED PARTY WAS HELPING EE STACK A BRATTICE ON THE RETURN WHEN EE ACCIDENTLY THREW A 8"" SOLID BLOCK STRIKING THE INJURED PARTY ON TOP OF HIS RIGHT FOOT. THIS IS HIS 4TH ACCIDENT SINCE STARTI NG TO WORK6-20-01. HAVE HAD A MEETING WITH HIM ABOUT THIS.

INJURIED PARTY HAD JUST LEFT THE ROOF BOLTER TO GO LOOK FOR SOMETHING WHEN HE WENT THROUGH A BACK UP CURTAIN THE SAME TIME THE SCOOP GAME THROUGH STRIKING HIS RIGHT FOOT.

INJURED PART WAS LINING THE SLOPE BELT AND AS HE WAS SWINGING HIS AXE HANDLE BROKE CAUSING HIS FINGER ON HIS RIGHT HAND TO HIT THIS BOTTOM BELT HANGER.

A ROOF FALL WAS FOUND BY THE 2NE SHIFT MINE FOREMAN WHILE PRESHIFTING THIS MINE. THE ROOF FALL WAS IN THE #1 ENTRY IN THE 2ND SOUTH PANEL AT SPAD #5+90. THE FALL WAS ABOUT 50' LONG, 18' WIDE & 4' HIGH. 41"" FULLY GROUTED ROOF BOLTS WAS USED IN THE AREA.

INJURED PARTY WAS WALKING THE 1ST SOUTH BELT WHEN HE STEPPED ON A SLICK SPOT AND FELL. HE LANDEDON HIS BACK INJURING HIS LOWER BACK.

WHILE PULLING SOME ROCK OFF THE TOP OF THE CONTINUOUS MINER THE INJURED PARTY DRUG A PIECE OF ROCK OFF AND IT FELL ON HIS RIGHT FOOT LANDING RIGHT BEHIND THE STEEL TOE.

INJURED PARTY WAS REMOVING A SMALL BOTTLE OF OXYGEN OFF A TRAILER WHEN HE REACHED DOWN TO PICK IT UP BY THE CAP. HE GRABBED THE OXYGEN CAP WITH BOTH HANDS TO PICK IT UP & WHEN HE JERKED REAL H ARD TO GET IT OUT OF THE TRAILER THE CAP CAME OFF UNEXPECTED 7 STRUCK HIM IN THE FACE ON THE LEFT SIDE OF FACE.

INJURED PARTY WAS HELPING ROCK DUST ON THE 1 SOUTH BELT WHEN A PIECE OF ROCK FELL OUT BETWEEN ROOF BOLTS AND STRUCK HIM ON THE HEAD AND BACK.

EE WAS MAKING A BELT SPLICE OUTSIDE GETTING BELTREADY TO GO UNDERGROUND WHEN THE BELT KNIFE HE WAS USING SLIPPED

INJURED PARTY HAD JUST PUT IN HIS 2ND DAY OF WORK RUNNING ROOF BOLTER WHEN ON HIS WAY HOM FROM WORK HE FELT A PAINT IN HIS RIGHT KNEE. HE SAID THIS PAIN GOT WORSE AS THE DAY WENT BY. HIS SHIFT WAS FROM 0700 TO 1500. HE SAID HE DID NOT RECALL HURTING HIS RIGHT KNEE AT WORK.

INJURED PARTY WAS WORKING IN THE BELT ENTRY PUTTING IN BELT WHEN HE TURNED AROUND & ACCIDENTALLY WALKED INTO A BELT HANGER CUTTING A PLACE OVER HIS RIGHT EYE.

INJ PARTY WAS LAPPING BELT ONTO A SUPPLY TRAILEROUTSIDE WHEN HIS PAIR OF CHANNEL LOCKS SLIPPED OFF THE BELT HE WAS PULLING CAUSING HIM TO FALL ONTO HIS RIGHT SHOULDER
