Employee had put golf cart on charge in golf cart charging station. As he started to walk away he hit a fire suppression nozzle suspended from mine foor which resulted in a laceration to his left ear which required 13 stitches.

Employee was installing roof bolt and as he started to raise boom to insert bolt into roof the other boom was swung hard by other operator. The roof bolt came out of the chuck and struck employee on left wrist causing pain.

Employee was installing roof bolt. Pushing roof bolt up into hole he had his left hand on the bolt. He accidentally started the rotation causing his hand to twist around the bolt resulting in a sprain to ligaments in hand and wrist.

Employee was rockdusting belt entry and rockdust hose caught on something. He jerked on hose to free it and felt pain in lower back. He did not start missing work until september 13.

Employee was changing shuttle car tire and it fell over. He and another employee were lifting the tire back upright when he felt pain in his lower right abdomen and later in his groin.

Employee was pulling shuttle car cable out of reel. He later felt pain in groin at the site of an old hernia where he had surgical repair three months ago, at a previous employer.

EE was picking up pull chain on shuttle car and twisted right knee.

EE was walking through curtain & struck his head on a roof bolt in the mine roof, jamming his neck & causing pain in his neck.

Employee was in bath house and bumped against hook that holds clothes baskets aloft. Hook disengaged and empty metal clothes basket fell striking him in left eye resulting in laceration to eye lid.

Ee was climbing up on dozer and his foot slipped, causing his left knee to strike cleat on dozer track. Impact was at lower edge of left knee, resulting in severe pain.

EE had just dumped his car at feeder and was turning around in operator's compartment, when another car cable anchored next to him, jumped to the side hitting him on the right side of the head, driving his head into edge of his car canopy. Resulted in lacerations, requiring stitches.

A roof fall has occurred int he first southeast maws, in no.1 entry inby spad 10 & 50. The dimensions of the roof fall area 50' long, 18' wide and 19' high.

Fall from machine

EE was helping splice belt and usually a piece of diesel equipment was out of service. EE got a golf cart to use for transportation. While traveling down inclined travelway, cable broke on braking system. EE jumped off golf cart and bruised his left hip.

Employee was on his knees and in process of installing a roof bolt. When he started to get up his right knee popped and locked into one position and could not bend.

Employee was unloading pump motor from personnel carrier and had positioned himself between a shuttle car and the personnel carrier. Another shuttle car came down the entry and did not see the personnel carrier sticking out of the crosscut about 4 feet, he hit it which in turn caught the employee's leg between the equipment.

Employee was helping splice miner cable and was kneeling on the mine floor. Whe he got up his left knee popped and started hurting. He could not straighten it.

Employee was drilling hole in mine roof to install roof bolt. Lowered roof bolting machine boom temporarily catching left knee under it, resulting in a strain to knee area.

Employee was driving 14-man personnel carrier (mantrip bus) and ran over something in roadway causing him to bounce up into mine roof and hurt his neck.

Employee was helping load pinner pot on the personnel carrier to take underground. When he and another employee lifted the pinner pot he felt pain in lower back.

Employee walked through wing curtain and lost his balance falling on self-rescuer and bruising his hip.

Employee was helping pull a shuttle car drag across mine floor to install it on a shuttle car when he felt slight pain in his groin. Employee did not start missing work until July 15, 2006.

Employee was walking a belt entry, examining the belt, and felt pain in his right hip. The pain got worse as the shift progressed. There was no incident to identify as the cause of the injury.

Pulling on cable and chains on previous shift when miner was hung, his back started bothering him. Then tonight while lifting bundle of glue from pinner, his back started hurting him bad.

Mechanic was checking cable on shuttle car that was not picking up cable. He took cover off and moved car up because line curtian was in the way. Reel unexpectedly took up tripping him causing him to fall backwards on his rescuer.

EE was loading bolting machine with glue when he felt pinch in his lower left back. He thought he would be ok went home came back to work the next day worked approx. 1/2 shift when he began hurting bad enough he could not stand it anymore he was brought out and taken to ER by company offical.

Employee had finished installing roof bolts in a place and had sat down and leaned back against roof bolting machine and took his hard hat off to wipe sweat off when a piece of top coal popped off upper rib hitting him on upper lip resulting in a cut that required two stitches.

EMPLOYEE WAS WALKING DOWN CATWALK AT RAW COAL LOAD OUT BELT AND SLIPPED AND FELL HURTING HIS LEFT WRIST.

Employee was repairing brattice that had been damaged by a piece of equipment. Several blocks had been knocked out but some were still in place at top of brattice. As employee started replacing blocks a block above him fell striking his left thumb.

EMPLOYEE WAS HELPING RECLAIM HIGH VOLTAGE CABLE AND LOADING IT ONTO A TRAILER WHICH WAS BEING PULLED BY A 4-MAN DIESEL PERSONNEL CARRIER. EMPLOYEE WAS NOT AWARE OF HIS POSITION IN RELATION TO THE EQUIPMENT AND GOT HIS HEAD CAUGHT BETWEEN FENDER AND MINE ROOF.

Employee was in the process of drilling a hole and reached back for a section of pinner steel when a piece of rock fell from the mine roof and struck his right middle finger causing immediate swelling and resulting in a fracture of the last segment of bone in finger.

EE was starting to walk beneath a belt underpass guard and hit his left shoulder on the guard causing him to loose his balance, and fall. The area was muddy and he had his coat and lunch bucket in his hands. When he fell he broke the lower end of his fibula on his right leg.

Employee was throwing loops of miner cable at slack pile as he taping cable and felt a burning sensation and sharp pain in his lower back.

WHILE HANGING MINER CABLE, EMPLOYEE PULLED GROIN MUSCLE NEAR RIGHT LEG.

Employee was attempting to cross over a belt and stepped on a square timber lying on the mine floor. The timber rolled pitching the employee forward against the belt and framing. The employee struck his left knee against tubular belt framing.

Employee was working on left miner in #7 entry. He was trying to change a hose that went from conversion value to pump motor. When doing so he pulled and tugged, hurting his lower back using a pair of channel locks to break the fitting.

Employee was reporting for work on third shift and he got out of his truck. He dropped his lunch bucket on right foot. He told his supervisor what happened and that his foot was hurting. He went underground and worked, but his foot became more painful and he was brought out of the mine and sent to RMC by ambulance.

Employee was stepping down off of Gob truck and missed bottom step with his foot causing right knee to hit step. Employee did not start missing work until July 17, 2005.

Employee was leaving the dumping point, in a shuttle car, and was looking back at his boom as he lowered it to make sure it cleared some cables hanging across the haul road. His car veered to the right and as he passed a parked golf cart, it struck his right arm which he had outside the operator's compartment.

The ee was operating the cont miner and backed miner up after cutting right side of face and he split the cats on the miner to move it toward left side of face. The right rear corner of the miner mashed the ee against the rib.

EE WAS REPAIRING BOLTER CABLE THAT HAD BEEN CUT BY A CAR. AT POWER CENTER HE PUT HIS LOCK & TAG ON BOLTER CATHEAD AND SPLICED THE CABLE. WHEN FINISHED, HE TOOK HIS LOCK & TAG OFF & PUT THE CATHEAD BACK ON THE POWER CENTER. TRIED 3 OR 4 TIMES TO RESET THE BREAKER - WOULD NOT RESET, SO HE REACHED DOWN TO TAKE THE CATHEAD OFF WHEN THE BREAKER BLEW UP ON HIM.

A roof fall was found on 9-27-04 around 8:45 am by safety director in the 1st southeast #5 entry between spad 12 and 60 and spad 13 and 30 in the pillar. Fall was 24' long 13 1/2' wide and 5' high. 5' fully grouted roof bolts were used in this area.

A ROOF FALL WAS DISCOVERED IN THE MAIN SOUTH RETURN AT THE PORTAL JUST INBY THE FAN HOUSE ON 7-12-04 BY MINE FOREMAN. THE FALL WAS 30' LONG, 20' WIDE AND 12' HIGH.

Injured party lifted the miner cable on #1 unit to hang, without waiting for help and felt pain in his lower back, Injured party continued to work and did not go to the doctor or miss any work until 7-2-04.

EE WAS PASSENGER IN MANTRIP WHICH LOST POWER COMING UP THE MAIN SOUTH SLOPE. DRIVER STARTED PUMPING THE FOOT BRAKE BUT IT FAILED TO HOLD THE RIDE AND THEY STARTED ROLLING BACKWARD. DRIVER PUT IT IN PARK, WHICH DID NOT HELP. DRIVER THEN SET OFF THE PARKING BRAKE BUT THEY HAD HIT THE COAL RIB ABOUT THIS TIME, INJURING EE'S RIBS.

INUNDATION OCCURRED AT THE 1ST SOUTH PANEL SEALS WHICH ARE OFF THE MAIN EAST INTAKE. FOREMAN FOUND DURING DAILY INSPECTION OF THE MINE. HE WALKED OVER TO CHECK THE #4 SEAL WHEN HE SAW WATER COMING OUT OF THE 4"" RISER. SOON AFTER HE FOUND WATER COMING OUT OF THE 4"" U-TUBE IN THE #1 SEAL. MINE EVACUATED. MSHA 103-K ORDER ISSUED; LIFTED 6/8/04.

INJURED PARTY SQUATTED DOWN UNDERGROUND TO WORK ON A MINE PHONE & THERE WAS A PIECE OF ROCK LYING ON TOP OF THE PHONE LINE. WHEN HE MOVED THE PHONE LINE, THE ROCK FELL & HIT THE TOP OF HIS LEFT KNEE.

INJURED PARTY HAD JUST PUT UP HIS 1ST BOLT IN THE ENTRY. WHEN HE STARTED TO SWING HIS BOOM, HE PULLED THE LEVER THE WRONG WAY, STRIKING HIS RIGHT KNEE.

HE WAS SITTING IN THE SCOOP BUCKET WHICH THEY USED TO CHANGE THE MOTOR WITH WHEN A PIECE OF DRAW ROCK FELL OUT BETWEEN BOLTS, MEASURING 17' LONG, 3' WIDE & 1/4"" THICK ON ONE END & 7"" ON THE OTHER END. THE ROCK STRUCK HIS SHOULDER & PUSHED HIM OVER HURTING HIS BACK.

INJURED PARTY WAS HELPING ROOF BOLTER OPERATOR MOVE FROM ONE PLACE TO ANOTHER WHEN HE RAN IN FRONT OF THE BOLTER AND MOVED A CABLE. HE THOUGHT HE HAD GOT OUT OF THE WAY. ROOF BOLTER OPERATOR S TRUCK THE RIB WITH THE ROOF BOLTER STRIKING THE INJURED PARTY ON HIS RIGHT KNEE.
