An employee broke both heels when EE jumped to the ground from the deck of a 785B rock hauler. EE jumped because a steering jack hose ruptured onto the turbo causing flames to come up to the outside of the cab area. The EE also received burns to the back of the neck and right arm.

EE climbed up a 3 step ladder to hand another technician a tool. EE was standing on the 2nd step of the ladder and holding onto the handholds on a 7858B Truck. The ladder twisted out from under EE and they slipped off the ladder and twisted an ankle when they hit the ground.

The employee slipped as EE was walking up an incline. When EE slipped, EE fell on EE's right ankle causing a broken tibia.

Employee was walking down the steps of the filter trailer and fell when he got to the bottom step and turned his ankle, resulting in an ankle sprain and small chip to a bone in the ankle.

Employee went to get on the push arm of a D10R dozer and his foot slipped causing him to fall and hit the push arm with his left rib cage causing cracks to two ribs. He returned to full duty the next shift. His regular job duty is an excavator.

As employee stepped off of the Lube truck his foot slipped and he fell into the ladder on the used oil tank causing a laceration to his left ear that took sutures to close the cut. He was in the process of emptying the used oil tank on the truck into the ground tank. He reported to full work on his next scheduled shift.

Employee was unloading supplies when cutting blades for a grader started shifting. He stepped to get in the clear and tripped over a hydraulic unit, falling to the ground catching his fall with his left hand causing a hair line fracture. He is continuing working his regular job.

Employee is a mechanic and he was working under a truck. As he was getting out from under the truck he twisted his knee. When he stood up, the pain of the knee & standing up made him pass out & he fell on his face.

Employee was walking from a dozer to a hauler and tripped over a small berm created by the dozer pushing, he fell to the ground. He caught his fall with his right arm and stoved it up. He continued to work and started missing work on 9-7-2010 due to complications.

EMPLOYEE WAS PULLING ON THE CABLE THAT COMES FROM THE POWER CENTER TO THE HIGHWALL MINER. AS HE WAS PULLING HE SLIPPED AND TWISTED HIS BACK. THE GROUND WAS WET AND MUDDY.
