Maintenance Technician stepped to the side onto an impact wrench that employee had laid on the ground while working on changing screens. Employee's ankle rolled when employee stepped on the tool.

The employee swept up a small pile of trash. He left the area and took a break. When he returned to the area, he stepped on part of the trash pile. His right foot slipped on a plastic jug cap (in the pile). Due to the twisting motion, it caused an ankle sprain.

EE WAS WALKING ALONG SIDE OF A CONVEYOR. HE WAS PICKING UP 10# BAGS OF CAT BOX ABSORBENT. HE STEPPED ON A SMALL PIECE OF WOOD (BROKEN PALLET BOARD.) HIS LEFT FOOT TWISTED CAUSING A CONTUSION.
