Employee was walking in an area of low head room, between 2 screens, and walked into a lighting fixture striking EE's head. The employee continued to work until 1-25-2016, when EE saw a doctor and was diagnosed with a compressed nerve.

Employee was walking through plant and struck head on pipe. Employee continued to work missing no time. Employee started physical therapy treatments on 07/17/2014. He received his third treatment on 07/21/2014. This accident is now reportable.

While walking across the fourth floor level, employee hit a pipe support with his head, causing a cervical sprain and a headache.**Accident was not reported until May 27, 2009. Employee was seen at hospital ER on May 28, 2009. He missed May 29th & June 1st and returned June 2nd at full capacity.**

Employee was walking across the ground floor of the Preparation Plant underneath an elevated conveyor belt. While going under the belt, he struck his head on a piece of angle iron support structure. (Misc Mail 8/7/08 #8-On June 30, he was being treated by a chiropractor. At the time, he continues to receive treatment.)

WHILE WALKING AWAY FROM THE TAIL OF WEIGHFEEDER CONVEYOR, EMPLOYEE STRUCK HIS HEAD ON A PIPE. THIS CAUSED A SORE, STIFF NECK. TO DATE EE HAS RECEIVED (3) CHIROPRACTIC ADJUSTMENTS. HE IS CURRENTLY WORKING WITH NO RESTRICTIONS.

WHILE WALKING ON THE 3RD FLOOR OF THE PREP. PLANT, EE STRUCK HIS HEAD ON A LOW STEEL PIPE. THIS RESULTED IN A CERVICAL AND MUSCLE STRAIN. EE WAS TREATED BY A DOCTOR ON 5/25/05 AND HAS COMPLETED 3 PHYSICAL THERAPY SESSIONS TO DATE. HE HAS 3 MORE PHYSICAL THERAPY SESSIONS SCHEDULED. HE CONTINUES TO WORK WITH NO RESTRICTIONS OR LOST TIME.
