EE stepped off the excavator tracks on to muddy and uneven ground when EE felt a sharp pain in left ankle.
A sliver of metal from driving a bucket pin by sledgehammer hit EE in the nose.

EE was assisting in changing out a jaw wedge on our jaw crusher when one of the adjustment springs unloaded pinching two of EE's left hand fingers. The spring had not been checked to ensure no kinetic energy was left in the spring. EE's supervisor took EE to Workplace Health where EE's fingers were xrayed (no broken) and cleaned and stitched up (5).
EE was assisting in changing screen decks when EE lifted a 50 lb grizzly cylinder to place in a loader bucket. EE felt some pain but over the weekend it got worse, EE was sent to Workplace Health on Monday then directly to emergency hernia surgery by the ER.
Employee reported pain in left elbow after operating 330 with hydraulic hammer for 2 days.
EE lost EE's balance while EE was standing on a 3 foot high conveyor tail roll cover. EE fell to the ground bruising EE's right knee and shoulder. EE was seen by our oc health provider within an hour for bruising and a contusion.
Employee was changing out a side roll on the conveyor when he dropped the 10 lb. roll on his left knee resulting in a bruise
EE stepped back during the process of changing the screen decks on the JCI cone crusher and scraped/bruised his right calf. He told his foreman at the time but it did not appear to be of concern. On 5/15, he told his foreman that the leg was swollen and very sore. He was sent to OC Health on 6-16
Driver failed to check ground conditions prior to exiting the dump truck. He stepped on a rock with his left foot turning his ankle/sprain.
Employee was in the process of stepping down from the deck of the Read screen on to the access platform of the Cat loader when he felt pain in his right shoulder
Employee was climbing back into his loader and hit his right forearm on the door latch post. This resulted in a laceration requiring several stitches to close the cut at our HCP.
