Bull Mountain Coal Mining Inc · Over-exertion in lifting objects

EE was changing bottom roller on the clean coal stackout belt. He was using a bar to lift the belting and install the new roller and he felt his muscle strain. This was not reported for 4 work days. On 9/15/06, the EE was treated at hospital for muscle strain and released to light duty.

EE was working to lift a 36 centrifuge, using a chain to lift and move the load on a trolley. The load got stuck on the trolley; while pulling the chain to move the load, the chain slipped. His hand was caught in the chain. EE was taken to the hospital and treated for injury to right hand.

EMPLOYEE WAS WALKING OUTSIDE THE D&R BUILDING. WHEN OUTSIDE THE DOOR, HE SLIPPED AND FELL. HE INJURED HIS LEFT HAND INDEX FINGER. HE WAS TREATED AT THE HOSPITAL FOR A FRACTURE AND RELEASED FOR WORK.

Employee was working to repair conveyor chain on the continuous miner. He was holding a hammer on the link and was struck on his finger with a hammer by another employee who was helping with the repair work. He was treated for cut finger with stitches and released for work.

Employee was putting magnetite into the heavy media washer. While lifting a bag of magnetite, the top of the bag ripped open releasing powder into the air. Powder got into his eye. He was taken to the hospital for treatment and was released for work the next day. Employee was wearing safety glasses.

EE was walking to enter foreman's change room prior to the start of shift, when he slipped on ice. He grabbed a support ple to keep from falling after working his shift until 2 hrs before end of shift he went home. He was treated for lower back strain at hospital.

Employee was off loading a shuttle car axle from the scoop bucket. He was standing on level ground and using a pry bar to roll the axle. When the bar slipped the employee had pain in his lower back. He was taken to the hospital and treated for lower back pain.

Employee was cleaning the 1 east beltway at xcut 20, loading lump coal into the belt. When a lump broke in his hands he hit himself in the right eye. Employee received medical treatment including 5 stitches and was released for return to work.

There was no evidence of unsafe conditions. Discovery of the fall was spet 15. about 2 pm. The fall was discovered as a result of checking on discharge water pipe. The fall had broken this pipe. Review of fan chart recorder data would indicate this fall occurred around 6 am. There was an increase in mine air resistance from 1.2 to 1.4 h2o gauge.

Mine foreman was performing examination of conveyor belt. He stepped off of a belt crosscover and slipped in some mud and fell. He sprained his right ankle. Medical exams showed no broken bones.

EE WAS ROOF BOLTING TO INSTALL SCREEN FOR A NEW SECTION SET UP. EE INADVERTANTLY USED FAST INSTEAD OF SLOW FOR THE BOOM LIFT AND A S RESULT PINCHED THUMB BETWEEN THE DRILL GUIDE AND CHUCK. A PORTION OF THE THUMB, BEHIND THE NAIL WAS CUT OFF. EE WAS TAKEN TO HOSPITAL AND TREATED FOR INJURY

Employee was assisting foreman install mine sights when a slab of roof fell from between bolts. The roof rock struck the employee on the hard hat. He was taken to hospital for medical evaluation and was treated for neck injury. Employee was standing on scoop to help put in sights.

EE was operating the scoop #2, turning the scoop around a corner in the main haulage. He had placed his left hand outside the cab to grip the scoop frame. His hand was pinched between the front and rear scoop frame. Both the index finger and thumb were pinched.

WORK WAS BEING PERFORMED ON THE CLEAN COAL COLLECT BELT AT THE SURFACE PREP PLANT. WHILE WORKING TO REMOVE THE BELT, EE LOST HIS GRIP ON THE BELT AND FELL DOWN STRIKING HIS HEAD ON THE STAIRWELL. THE TOP OF HIS HEAD WAS CUT AND HE WENT TO CLINIC FOR TREATMENT. 10 STITCHES WERE MADE.

WHILE ROOF BOLTING THE EE WAS STANDING ON THE FLOOR AND WAS STRUCK ONT HE RIGHT KNEE BY A PLATFORM. THE PLATFORM WAS RAISED DUE TO LOW ROOF AND WAS NOT LATCHED. THE KNEE INJURY WAS MINER SCRAP TCH BUT LATER BECAME INFECTED. ON 4-9-03. THE EEWAS TREATED FOR.

EE WAS OPERATING THE SCOOP, CLEANING THE BELT ENTRY. HE DID NOT NOTICE PLASTIC PIPE IN THE GOB PILE. WHEN THE SCOOP RAN INTO THE PIPE WAS STRUCK AND THEN THE PIPE SNAPPED INTO THE CAB OF THE S COOP, ALSO STRIKING EE LEFT WRIST. HE WAS TREATED FOR A HAIRLINE FRACTURE AND RELEASED FOR LIGHT DUTY.

EE WAS STRUCK BY A METAL RACK FOR GAS TANKS WHEN THE PICKUP TRUCK STOPPED. THE TANKS SHIFTED AND FELL OUT OF THE TRUCK. EE WAS RIDING IN THE BED AND WAS THROWN OUT WITH THE RACK AND WAS STRUCK IN THE LEFT ARM AND SHOULDER. HE WORKED HIS NORMAL JOB UNTIL NOV. 25 AND IS NOW UNDER DOCTOR'S CARE FOR TREATMENT OF INJURY.
