1ton dodge truck was up on jack stands with front wheels off ground,employee was under front of truck bleeding the power steering system,he asked another employee to get in truck and start the engine. The truck was started and the employees foot slipped off the clutch and the truck fell off the jack stands falling to the floor crushing the employee under truck.

Hauling longwall components, was adjusting tow chains, load shifted causing fracture to the right ankle.
At approximately 3:00 a fire was reported off mine property, early morning on the 2/27/2026 the fire was on a remote section of the mine property, there was no lost of production, no damage to any buildings, no injury's. The fire was out on 3/1/2026.

In bucket of LS 195, shoveling rock dust into bantam duster. Tripped over pallet that duster is setting on, hitting right side of face on duster causing laceration on the right ear required stiches. Small hematoma behind right ear.
Small roof fall was found in non-active area of the mine. full roof support was installed according to plan, with additional 12 foot cable bolts and monster mats. Ventilation was not interrupted. Fall was approximately rib to rib, 7-8 foot at the anchor zone, 9 foot wide.
While washing ATV on wash racks, sun was shining in eyes. Miss judge the opening between wheel tracks. Fell between landing on left side of chest.
EE was removing pipe in the longwall tailgate. Pipe came off wheel of man basket striking right jaw and neck area. Had CT scan with negative results. First aid treatment only.
A fire occurred on the surface of the mine in the compressor building. 911 was called and the local fire department responded and worked with the mine to extinguish the fire. No injuries occurred as a result of the incident.
Driving grader into mine, while pulling pull cord on out-by airlock door to open. the machine rolled foreword, pinching finger between cab and pull cord causing fractures and lacerations to right ring finger.
Contract EE was in class room having experience miner training. EE suddenly collapsed unconscious having a seizure. Transported to hospital for evaluation. Is released to return to work 05/22/2025.
employee was using a wrench to hold nut to tighten bolts with cordless impact when middle finger got caught between flange and washer and pinched it on left hand. There was only first aid to treat it.
While splicing cable, helper moved cable causing knife to slip and causing laceration to right top of hand, between thumb and index finger. Required four stitches with follow up with Ortho specialist.
Changing bits on shearer, rotating drum with hand. Sprags were extended shields set, pan pulled back as far as possible. A small rock fell from face striking small finger between bit and rock on left hand causing fractured bone and laceration to the middle phalanx of small finger. The medical team at the hospital removed the top part of the finger. 3 days later closed injury.
Removing cylinder on pineal hitch on tow tractor, while removing hydraulic hoses, hitch dropped pinching left hand between cylinder and frame. Causing laceration in left palm. Requiring 7 stitches.

Removing staple out of shield return hose. When staple came out line still had some pressure on it, causing hose to whip striking right side of face. Laceration to right eyebrow (5 stitches) and a hematoma under right eye.
While moving high pressure emulsion pipe, attempted to adjust pipe collar, while lifting pipe. Pipe slipped and pinched left middle finger between pipe and pallet, causing laceration and small fracture to distal bone. Received four stitches in finger.
Removed forks off LHD-5, and installed bucket on equipment. While hooking up hydraulic lines to bucket, the forks rolled back pinching palm and fore arm between tire and forks causing fracture to right wrist.
EE was trying to move vent tub that was stuck in the mud, handle broke causing body to twist, foot was stuck in the mud, causing knee to twist in an awkward position. Felt a popping sensation in the left knee, finished shift. Next day any movement of knee was very painful, safety took to seek Medical care. 9-30-2024 restricted from any work by specialist.
A non-injury roof fall was found in the 1 east tailgate x-cut 12, 2-1 entry.
EE has had surgery on shoulders previously, during their mining carrier the shoulder has been causing more pain, On 9-14-2024, EE was having pain in their shoulder and felt they could not return to work, currently EE is seeking medical assistance. There was no incident
A roof fall was discovered in the 1 east tailgate at x-cut 14 2-3 entry. the fall was confined within the x-cut and did not obstruct ventilation, travel ways or escapeways, the width was approximately 18 feet and 25 feet across towards 3 entry.
While pulling pin on fifth wheel hitch, cut the top of thumb on left hand. On 9-24-24, safety looked at injury, the wound was infected. Transported to medical for treatment for the infection.
Throwing feeder nip over discharge pipe, tried to catch nip. Hit right hand chipping bone on right middle metacarpal knuckle.
Bent over to pull miner cable, felt pain in lower back. Tried to walk off, no relief.

CONTRACT EMPLOYEE WAS STEPPING OFF ROOF BOLTER, STEPPED INTO HOLE TWISTING RIGHT ANKLE CAUSING AVULSION FRACTURE TO THE RIGHT TALAS BONE.
EE was working in shield, stepping over hose, when placed foot down on back of shield, knee cap dislocated.
Contract EE was lifting mine cable over head, felt slight pinch in lower back. Went to hospital next day lower back strain.
EE was recovering steel pipe from mine roof, pipe slipped and fell on left foot. Causing facture to small toe with a laceration between small toe and next, requiring four stiches.
EE was getting into trash trailer to arrange trash, felt a pain in right knee. Filed an incident report refused medical treatment, 4:30 4/28/2024 EE decided to get knee checked. Hospital gave full release to return to work. EE continued to work full duty until 12/27/2024. EE had follow up visit with ortho, surgery was suggested to stop the degeneration in the knee.
EMPLOYEE WAS GUIDING THE SKID STEER OPERATOR, LOADING SPILL TRAYS ONTO A TRAILER. EE WAS STANDING ON THE BACK OF THE TRAILER WHEN THE SPILL TRAYS SLID OFF THE FORKS, CONTACTING PREVIOUSLY LOADED TRAYS; THEN THOSE TRAYS SLID AND HIT THE TOP OF THEIR LEFT FOOT.
Stocking bolts on bolters boom. Went to step over walkway, lost balance and slipped to walkway, hitting right foot on walkway. Causing pain in right foot. Finished shift went to doctor next day. Fractured bone on the 5th, center metatarsal right foot.

EE opening doors on trash trailer, finger was pinched. Refused medical treatment continued working. On 9-19-2024 EE went to occupational health was diagnosed with a mallet finger. Became a lost time accident at this time.
Raking under belt, pulled back and elbow came in contact with with bolt. Causing small chip in right elbow.
Working on shearer, removing ranging arm pin. I had my hand in the eyelet pushing on pin while EE was pulling on pin, the pin bounced back in pinching middle finger on right hand. No broke bones, doctor release pressure on finger nail.
backed in LOXC rear of scoop first, pulled around corner and noticed cable on ground(bolter reversed direction, removed hand from bucket lever as operator slid on seat grabbed cab of scoop to adjust body position, scoop fully jack knifed articulated crushing hand between cab and fender.
Cutting bands off bacon skins, band sprung back hitting inside of wrist. Small cut/puncture on right wrist. Required one stitch.
Bent over to pick up Mine Bits, leg popped and caused EE to fall.
Temporary staffed labor (C2605) was pumping diesel fuel when they used a knife to cut away an obstruction. The knife slipped cutting their left palm.
Was watching and helping with blasting, standing on toes of shields. EE could hear rock falling, went to get out of way when EE was hit by a piece of rock on the hard hat. Knocking of hard hat of head. Started looking for hard hat, found and signaled for help. Then continues down face to get more help.
Rock in face conveyor, was in the process of drilling and shooting the rock. When moving to load the 5th hole, noticed movement in the rock. Told everyone to clear the pan, rock caught the right foot, amputating all toes on right foot.

Cutting the tailgate out, was headed toward the headgate, getting ready to turn around to cut snake out when a slab of rock came over the spill tray hitting the left leg.
Had pain in my ankle for several months. On 1/20/2023 went home after work and was having trouble walking, went to the doctor on 1-24-2023. The doctor said that I had tendonitis in my left ankle. Able to return to work when tolerated. This was reported on 1-24-2023

Installing wheel on trailer. Pushed wheel over with left hand to align the wheel, the walking axle fell down striking left arm. Breaking bone in left forearm.
Moving metal storage rack with electric hoist to replace with new rack. When slacking off on hoist the back side of rack set down on a piece of wood causing the rack to tilt and the plates of steel fell over landing on left lower leg. Fracturing bone in lower leg.
At approximately 5:16 pm 8/2/2022 is was reported that small fire approximately 4 miles east of mine office was burning on the mine property. DNRC was on site fighting the fire. This is a very remote area, with no mine activity in the area. Called MSHA report line at 5:26 PM. There are no structures or equipment in the area.
Filling soap dispenser at 3-2 transfer when water manifold blew off hitting me in the face causing 2 cuts to the face. Stitches were required on both lacerations.
Jumped out of shield that EE was working on, glasses fell down on nose. Face popped and chunk of coal hit EE in the left eye. Flushed eye out several times for two days then went to the doctor on 7/14/2022. Accident was not reported until 7/14/2022.
The spool holder for the leaky feeder was on the tongue of the shop car. there was Material in front of the shop car so EE was carrying the spool holder tripped on crib and the holder hit right ankle. Injury was checked and cleaned felt good went back to work. On 7/15/2022 ankle was hurting, Safety checked and took EE to doctor.
Employee was assisting Strata Contractors on strata chambers. Employee was squatted down pulling to secure blow up tent inside of chamber when d-ring that strap was through failed causing employee to fall backwards. Causing injury to right lower leg.
EE was lifting belt structure, closest Section to tail piece assembly, Belt was off structure. Structure height was approx. 4' high, When lifting felt pop on out side of right knee. Area was sticky mud on floor. continued working. on June 21,2022 knee hurting went to doctor. Not released to come back to work

Was having trouble breathing, returned to WDA, when returned to WDA not breathing, no pulse.
A clevis was used to extend chain from monorail to roof. The clevis was not properly secured to the chain causing it to come apart when the work station was advanced out by. The work station dropped approximately 3 feet to the mine floor with personnel in the work station.
Pulled roof bolter into face a couple of rows back to check the suction and pull slider back into the tubes when a rib sluffed off and hit me.
While installing a horizontal pin on the pan line, prying up on the snakehead with a prybar to line up the pin, prybar came loose hitting the right eye on safety glass. Pushing the lens against the right eye.
Roof fall in the south portal #3 slope. Approximately 370' from base. Roof fell to a maximum of height of 8 ft (1 foot over the anchor zone). This did not cause any change of ventilation or danger to employees. This is a very remote area of the mine.
Walking thru compressor room Tripped on a grounding wire that was sticking out of the concrete about 1/2 inch.
Shoveling Belt plow accumulations went to pick up a shovel full of accumulations Left foot/leg slid in the accumulations twisting knee, heard and felt a pop in the inner left knee.
Making a wrench for bolting. Just rolled up my sleeves of my sweatshirt. I was pounding the stinger onto the socket head. A small piece of metal chipped off and went into my arm.

Packing structure into belt line and installing. Was installing top frame on rail felt a catch in lower back. The area was silty from washing the feeder could have contributed to the incident. this incident is a reportable as of 01/6/21. Lost time accident 1-26-21.
Employee was cutting on the TG side of shearer heading towards HG when a piece of face coal rolled off over the spill tray contacting the employee's left lower leg fracturing it.
While installing timber, ladder shifted causing employee to fall to floor. Hitting left shoulder on floor.
employee was walking down the LW face towards the HG, going through spotted shields a rock fell from between shields hitting the left thumb causing laceration and breaking the tip of it.
While putting TEK Seal bags on pump, employee went to throw an old pallet off of trailer and felt three pops in right arm resulting in partial torn bicep.
At approx. 4:30pm on September 2, 2020 noticed smoke in the air and at that point the Bobcat Wild Fire was approx. 6 miles away from the mine property. At 6:10pm the Bobcat Wild Fire reached the mine property and we promptly evacuated the underground employees. Never lost power to the Main Mine Fan. At 6:13pm MSHA was notified.
While stepping off the rear of the roof bolter, employee stepped on some loose coal causing EE's knee to twist.
Roof fall was found when investigating a water line leak in the No. 3 entry. There were no witnesses to the fall. Roof fell to the bottom of the Rehder seam approx. 10 ft. where a localized wet area developed.
EE was tending bolter cable holding it tight to the rib. EE grabbed rib mesh and when bolter pulled cable caused EE to pull out on mesh causing a small piece of coal to slide down mesh striking back of hand causing a slight fracture.

Measuring new parts for the shear and some coal rolled off of the face striking right leg causing a fracture.
Employee was holding support for chainsaw operator. Chain broke and a piece struck the employee on EE's heel causing a laceration resulting in sutures.
Employee was preparing to get mesh for the roof bolter, unchained mesh, mesh shifted catching finger and twisting/straining finger. Did not become a reportable accident until August 13, 2019.
EE was cutting a rubber hose with a knife, when almost cut thru hose, the hose rolled causing knife to cut the palm of the right hand, requiring sutures.
EE was washing the ROM tunnel with hot water to get frozen debris off of floor. When setting hose down the hose ruptured and scalded the right arm, resulting in a minor second degree burn.
Prying on ice jam with pry bar, bar slipped pinching finger between pry bar and side rail of chute.
Removing a bolter wrench chuck by striking it on a metal surface, missed catching finger between the drill steel and metal surface, requiring stitches.

Changing a bad hose, when hose was pulled pressure hit right hand breaking index finger and slightly injecting fluid into back of hand at finger.
After loading belt structure into the back of a flatbed pick-up, employee had to adjust the structure to fit. Employee pulled on the structure and when it came loose it pinched a little finger between the structure frame and the pick-up bed resulting in an amputation of the distal phalange of the little finger.

While removing monorail components, employee began to remove a stiff arm between two trolleys. When bolt came out, the stiff arm came down smashing the left thumb requiring stitches.
Isolated roof fall in remote South Portal entry No. 1. In this area there was a thicker lens of mudstone and a steep slope on the surface. Cover was 300' and the area was bolted with 5' bolts. Entry width of 20ft. No personnel injured, no interruption to ventilation. Roof fall was re-bolted and supplemental support was added in the affected area.

A localized wet area developed where the Rehder to Mammoth interburden was less than 10 ft thick. Entry width of the belt entry was 20ft. The fall concentrated in the belt line for approx. 100ft and fell to the bottom of the Rehder seam 8-9 ft above the top of the Mammoth seam.
Trainee on Roof Bolter, took eyes off of hand to run lever. Smashed finger requiring 15 sutures
While placing Acetylene bottle on shoulder, placed hand on bottom of bottle and pinched left middle finger between bottle and the mine roof requiring 4 sutures.
Employee slipped on ice and hit knee, on January 4, 2014 and was fine at that time. On April 24, 2014 employee went to doctor because of discomfort in knee, had knee scoped removing some debris. At this time the accident became reportable.

An employee was assisting with installation of supplemental roof support using a cobra drill when a piece of roof rock struck him on the left pinky finger.
The mine operator was notified on 7/29/2013 by an employee that a skin irritation on his leg became infected and required medical treatment. The skin irritation was reported on 7/16/2013 from handling concrete by the employee and first aid treatment was administered at that time.
An employee (Miner Operator) stepped behind the tail of the mining machine to check sites, the shuttle car operator pulled up behind mining machine and pinched operator between the shuttle car and the tail boom of the machine.
An employee was changing a coupler on a belt gear box when the metal plate he was standing on broke out. The employee fell approx 18' to 20' to the platform below. The employee received multiple fractures, head, back, scapular.
An employee was releasing a com-a-long with stored energy, the com-a-long released jerking his arm/shoulder. X-rays were taken and nothing broken. Upon the employee's follow-up visit with the doctor on 05/13/2013 a prescription for medication was given for inflammation and pain.

While hoisting employee to middle of derrick (approx. 15 ft) employees foot caught underneath a girt before operator stopped.
Employee was loading a fire barrel skid into the bucket of a skidsteer. The barrel skid slid into the bucket and smashed the ""pinky"" finger on left hand between skid and bucket of skidsteer. The mine floor was wet and muddy adding to slickness of metal on metal conditions.

Employee was assisting in making a belt splice when he moved too close to other employee using a hammer to pound in belt nails. When employee drew back to swing, the hammer hit other employee in the safety glasses cutting above eye.
The mine experienced an elevated amount of carbon monoxide in the gob area of the long wall approximately 9,000 feet behind the active long wall face. After bag samples were taken and analyzed MSHA was notified.
On 11/04/2011 an unintentional roof fall was determined in the 3-right section between X-cut #5 and #6 in the #3 entry (belt line). The fall was approx. 20' long X 20' wide X 10' high.
An employee was lifting heavy longwall parts when he felt pain in his abdomen. He thought it was just a muscle pull, and did not report until over a week later when it was not getting better. He was taken to the hospital on Oct.7th and was diagnosed with a hernia. He had an appointment with a surgeon on the 11th and surgery is scheduled for Oct. 20th. His is on restricted duty.
An employee were assisting in placing roof mesh on the ATRS of the roof bolter machine when cap rock from the roof fell striking the employee on the right forearm fracturing arm.

Employee does not exactly know how or when the injury occurred. He believes that it occurred while walking in the mud pulling vent tubes. Employee worked the following two shifts of the week 09/01 & 09/02 for a total of 27 hours.
At approx. 12:40pm an unintentional roof fall was determined in the 2-right section #2 entry from X-cut #103 to X-cut #104. The roof fall we believe is a continuation from the previous roof falls in that area. The roof fall is approx. 120' long, 18' wide, and 8' to 10' high.
An unintentional roof fall occured in the longwall section enrty #2 X-cut #98 intersection, approx. 30'outby, 30'inby, 50' from entry 2 to 1 and 30' to seal. This is possibly a continuation from the roof fall on 7-18-2011.

Hooking up structure trailer to Getman using a handy man jack. Trailer was very heavy and hard to jack. Hooked up Getman and was trying to lower the trailer onto Getman, when employee slipped causing the jack handle, which was under pressure, to strike him in the shin.
An unintentional roof fall occurred on the longwall recovery face. An area on the face side, in front of the shields fell at the anchor point of the 7' bolts. The area was approximately 7' high x 3' wide x 15' long. No one was trapped, or injured. The escape way was not blocked.
An employee was working on the face conveyor chain of the long wall when the stored energy of the chain released striking the employee at the ankles, amputating the right foot above the ankle and compound fracturing the left ankle.
Employee was working on the face conveyor tail sprocket, when the large cover for the sprocket rolled striking employee on the back and shoulder. The employee walked off the face and was taken to the hospital to get checked out. X-ray were taken with negative findings at this time. Further testing for other possible damage is being conducted with no results at present time.

Employee was removing bit inserts from the shear drum with a porta power and a pry bar when the pry bar came loose striking employee in the mouth breaking tooth and lacerating upper lip.

Employee was using a come along to move equipment and the chain broke and hit him in the back.

Employee was cleaning under a conveyor belt in the wash plant with a water hose. His hand slipped off the belt frame onto a top roller pulling his hand and arm through the moving roller, bruising the muscle and lacerating arm.
The area was muddy, with plank in the bottom. The miner was carrying crib block, slipped on plank twisting left knee.
Employee was stepping down off a piece of equipment and slipped off the step and fell to the ground landing on his foot dislocating his ankle. The force of the dislocation tore the skin from the top of his leg around to the back requiring stitches to close.
Employee was lifting 8 inch empty water line when he felt a pop in his rectum. He finished his shift and went home. During the course of the night the pain increased and had to go to the doctor where they removed exposed hemorrhoids.
Employee was in process of filling water tank. The tank pressure blew the cap towards his head and struck his lip causing a cut lip. Employee was treated for cut lip at Roundup Hospital and released for work. 3 sutures were made to his lip.

employee was trimming a cable with a razor knife, the knife slipped and cut right pointer finger.
Employee was walking down the long wall face on the toes of the shields when a rock fell between the shields striking the employee on the thigh. He finished his shift and the following shift. The employee noticed his thigh starting to swell and went to the hospital where the DR. found the bruised thigh needed the pressure relieved. The wound needed to be stitched to close.
While operating shear on long wall a rock was pushed out of the pan line by shear and struck employee on wrist. Employee was taken to emergency room and had x-ray taken of wrist. er doctor could not see any fracture but refered employee to a specialist. After seeing specialist 10 days later a small fracture was found. (Later determined to be a sprain).
Climbing ladder to work on discharge. Foot slipped through second rung on ladder, twisting back. Did not fall to ground.
Employee was securing load in truck with a bungee cord, when bungee cord came undone striking employee in face.
An unplanned roof fall was discovered by the weekly examiner in the #2 return entry, #3 x-cut it was 15'long by 12'wide by 7'high.
Employee was pulling in shields and put his hand in the pan line as the shear was moving, the bret-b caught his hand and cut finger.
Employee was tightening bolts on stage loader when a metal cover was bumped and swung down and smashed fingers. Right pointer finger and right middle finger.
EE was moving a spool of conspec cable, he slipped in mud and fell on right knee.
EE was installing a 6.5 X 6.5 square steel tubing core over shaft of a belt winder. EE and coworker placed the winder shaft onto cribbing and placed the core over end of shaft and proceeded to push core onto shaft. As core slid onto shaft ee pinched his left ring finger between core and shaft. Fractured bone in finger tip and laceration, 8 stitches to close wound.
An employee was helping on a belt move and was walking past the feeder breaker and his arm hit a sharp piece of metal lacerating his forearm.
Walking through mud, stepped on a chunk of coal, causing knee to push backward.

Large fall behind longwall shields caused air concussion that pushed employee into coal rib of #3 entry (headgate). Injury to employee includes cut on top of head, broken ribs, broken scapula. Employee was transported to hospital for treatment of injuries.
Employee was struck by another employee with a hammer while working to splice conveyor belt. Smashed middle finger of left hand. Dr. treatement for a fractured finger.
Muddy floor conditions were partially covering a board with a nail in it. The board was in close proximity to a visual disconnect ee was working on. When he stepped on the board, the imbedded nail penetrated the sole of his boot & punctured his foot. On 12-9-09 he experienced redness & swelling in the puncture wound. He sought medical attention & continued to work.
While hooking a chain to the scoop bucket, Employee slipped on haulageway road causing him to fall and twist his left knee. Employee is being treated for torn cartlidge.
Employee states he was using a chainfall and pc. of angle to lift a chute. The come-a-long got stuck on the floor & when it came loose it smashed employee's right hand between the floor and the chute. Medical treatment was received and employee returned to work the next day.
Employee was pounding wedges for a roof support, swung hammer and hit hand.
A round roof support(can) was on the forks of the loader. The loader turned and the can rolled off of the forks and against the side of employees foot.
Employee was loading belt structure when a spreader bar was relocated along with structure that was being loaded. The spreader bar struck the top of right hand. Employee was wearing gloves. A cut resulted and was reported to foreman later in shift. At 3:00 pm employee was taken to hospital for treatment. 3 stitches were done.
Employee states he was assembling an x-brace when one side slipped and fell on the back of his head as he was trying to bolt up a piece below, causing a laceration that required six (6) staples in his scalp to repair.
EE was tightening a porcelin insulator on a power pole and it shattered and a piece fell on the right inside of his forearm and cut it open. First aid with a guaze bandage and direct pressure was applied on site and member was immediately evacuated for further medical treatment. Injury required 12 stitches.
EE states dust build-up on his welding hood entered his left eye when he flipped his shield, causing a foreign object to enter eye and require medical attention to remove. No lost time is associated with this injury.
Employee states he was guiding forklift as it was picking up steel and a piece of steel fell off of forks and landed on employee's left foot causing a laceration and crushing injury. Employee sought medical treatment and is on restricted duty at this time.
EE states he was compacting dirt with a compactor and stepped on a loose rock, twisting his right ankle and spraining his muscle.
EE was assisting in setting a storage container on a tractor trailer. A strong wind blew the container around, pinning the EE's right arm between the outrigger on the crane and the container. The EE was off work until 5/20/09, when he returned to restricted duty.
Employee states he broke a tooth when a choker hit him in the mouth while unhooking a shackle from a piece of steel. He sought detal treatment on 5/15/09 for a crown and dental impressions, hence the reason report was not sent at time of injury.
The employee was running a lever and got his hand in a pinch point.
Employee states he crawled under conveyor frame work to reset a string line, when a co-worker hit the leg of support jack with a hammer to align the frame. The east end of the assembly fell approx 2 ft, striking employee in lower back, left side. Employee missed 4/14-4/18. Returned to restricted duty 4/20/09.
The employee was walking in the section when he took a wrong step on his leg and wrenched his knee. He was taken to the hospital and released with a miss diagnosis of nothing wrong. The employee was released and came back to work later he recieved an MRI when his knee was not getting better and found significant damage to his knee.
The injured person was holding a roof bolter cable for his partner that was skinning the cable back when the knife slipped and cut the right pointer finger at the first knuckle.
EE suffered muscle strain to the center of his back when he was assisting co-worker with crane pads on outriggers. EE missed 3/17/09 and worked restricted duty 3/18/09-3/20/09. Back to regular duty 3/23/09 with no restrictions.
Crew was going in the hole with the drill string. Tongs bit then slipped and flew into a crew member, throwing him to the floor, where he landed on other equipment. He hit and cut his left side of his head (5 stitches) and hit left side (3 broken ribs).
Employee states he was unloading a trailer when a piece of the wire popped up from the load knocking his hard hat and safety glasses forward & poking him in the right eye causing an abrasion on the cornea. Employee worked restricted duty 3/3/09 & 3/4/09 returning to regular duty 4/5/09.
EE states that he suffered a contusion to his right foot after a skidsteer operator set the forks from the machine on the EE's foot while adjusting the load.
EE states that he reached over to pull on some #8 rebar and felt a sharp pain in his lower back. EE suffered low back muscle strain and is on restricted duty for approx 2 weeks, per treating physician. This is no lost time, EE returned to work 1/27/09 on restricted duty.

EE was changing bottom roller on the clean coal stackout belt. He was using a bar to lift the belting and install the new roller and he felt his muscle strain. This was not reported for 4 work days. On 9/15/06, the EE was treated at hospital for muscle strain and released to light duty.
EE was working to lift a 36 centrifuge, using a chain to lift and move the load on a trolley. The load got stuck on the trolley; while pulling the chain to move the load, the chain slipped. His hand was caught in the chain. EE was taken to the hospital and treated for injury to right hand.
EMPLOYEE WAS WALKING OUTSIDE THE D&R BUILDING. WHEN OUTSIDE THE DOOR, HE SLIPPED AND FELL. HE INJURED HIS LEFT HAND INDEX FINGER. HE WAS TREATED AT THE HOSPITAL FOR A FRACTURE AND RELEASED FOR WORK.
Employee was working to repair conveyor chain on the continuous miner. He was holding a hammer on the link and was struck on his finger with a hammer by another employee who was helping with the repair work. He was treated for cut finger with stitches and released for work.
Employee was putting magnetite into the heavy media washer. While lifting a bag of magnetite, the top of the bag ripped open releasing powder into the air. Powder got into his eye. He was taken to the hospital for treatment and was released for work the next day. Employee was wearing safety glasses.

EE was walking to enter foreman's change room prior to the start of shift, when he slipped on ice. He grabbed a support ple to keep from falling after working his shift until 2 hrs before end of shift he went home. He was treated for lower back strain at hospital.
Employee was off loading a shuttle car axle from the scoop bucket. He was standing on level ground and using a pry bar to roll the axle. When the bar slipped the employee had pain in his lower back. He was taken to the hospital and treated for lower back pain.
Employee was cleaning the 1 east beltway at xcut 20, loading lump coal into the belt. When a lump broke in his hands he hit himself in the right eye. Employee received medical treatment including 5 stitches and was released for return to work.
There was no evidence of unsafe conditions. Discovery of the fall was spet 15. about 2 pm. The fall was discovered as a result of checking on discharge water pipe. The fall had broken this pipe. Review of fan chart recorder data would indicate this fall occurred around 6 am. There was an increase in mine air resistance from 1.2 to 1.4 h2o gauge.
Mine foreman was performing examination of conveyor belt. He stepped off of a belt crosscover and slipped in some mud and fell. He sprained his right ankle. Medical exams showed no broken bones.
EE WAS ROOF BOLTING TO INSTALL SCREEN FOR A NEW SECTION SET UP. EE INADVERTANTLY USED FAST INSTEAD OF SLOW FOR THE BOOM LIFT AND A S RESULT PINCHED THUMB BETWEEN THE DRILL GUIDE AND CHUCK. A PORTION OF THE THUMB, BEHIND THE NAIL WAS CUT OFF. EE WAS TAKEN TO HOSPITAL AND TREATED FOR INJURY
Employee was assisting foreman install mine sights when a slab of roof fell from between bolts. The roof rock struck the employee on the hard hat. He was taken to hospital for medical evaluation and was treated for neck injury. Employee was standing on scoop to help put in sights.

EE WAS OPERATING THE ROOFBOLTER. WHEN MOVING THE MACHINE CAP ROCK FELL AND STRUCK HIS FORE ARMS. EE WAS TAKEN TO CLINIC AND TREATED FOR BRUISES.
EMPLOYEE WAS WORKING ON THE ROOF BOLTING MACHINE WHEN HIS LEFT ARM BEGAN TO HURT. THERE WAS NO FALL, STRIKING, PINCH OR OTHER IMPACT. EMPLOYEE WAS EXAMINED BY A DOCTOR AND TREATED FOR ACUTE BURSITIS.
EMPLOYEE SUFFERED HEAT EXAUSTION & DEHYDRATION. WHILE WORKING WITH FELLOW EMPLOYEE IN THE BELT PRESS THIS MAN WAS DIZZY WITH UPSET STOMACH. EE WAS TRANSPORTED BY AMBULANCE FOR MEDICAL TREATMENT AND EVALUATION. EE SPENT THE NIGHT AT HOSPITAL & WAS RELEASED FOR WORK.
EE was operating the scoop #2, turning the scoop around a corner in the main haulage. He had placed his left hand outside the cab to grip the scoop frame. His hand was pinched between the front and rear scoop frame. Both the index finger and thumb were pinched.
WORK WAS BEING PERFORMED ON THE CLEAN COAL COLLECT BELT AT THE SURFACE PREP PLANT. WHILE WORKING TO REMOVE THE BELT, EE LOST HIS GRIP ON THE BELT AND FELL DOWN STRIKING HIS HEAD ON THE STAIRWELL. THE TOP OF HIS HEAD WAS CUT AND HE WENT TO CLINIC FOR TREATMENT. 10 STITCHES WERE MADE.

WHILE ROOF BOLTING THE EE WAS STANDING ON THE FLOOR AND WAS STRUCK ONT HE RIGHT KNEE BY A PLATFORM. THE PLATFORM WAS RAISED DUE TO LOW ROOF AND WAS NOT LATCHED. THE KNEE INJURY WAS MINER SCRAP TCH BUT LATER BECAME INFECTED. ON 4-9-03. THE EEWAS TREATED FOR.
EE WAS OPERATING THE SCOOP, CLEANING THE BELT ENTRY. HE DID NOT NOTICE PLASTIC PIPE IN THE GOB PILE. WHEN THE SCOOP RAN INTO THE PIPE WAS STRUCK AND THEN THE PIPE SNAPPED INTO THE CAB OF THE S COOP, ALSO STRIKING EE LEFT WRIST. HE WAS TREATED FOR A HAIRLINE FRACTURE AND RELEASED FOR LIGHT DUTY.

EE WAS STRUCK BY A METAL RACK FOR GAS TANKS WHEN THE PICKUP TRUCK STOPPED. THE TANKS SHIFTED AND FELL OUT OF THE TRUCK. EE WAS RIDING IN THE BED AND WAS THROWN OUT WITH THE RACK AND WAS STRUCK IN THE LEFT ARM AND SHOULDER. HE WORKED HIS NORMAL JOB UNTIL NOV. 25 AND IS NOW UNDER DOCTOR'S CARE FOR TREATMENT OF INJURY.
