Employee complained he felt repetitive motion injury during the daily course of electrical work at the plant. On 3/10/10 the employee was diagnosed with carpal tunnel and left neural neuropathy.

Employee took an 8 oz plastic bottle of diesel fuel treatment from flammable storage cabinet. He was turning the bottle around to read the directions when it burst splashing the contents in his face and chest. He was wearing safety glasses. He flushed out eyes with water then was transported to VRMC.

Employee was attempting to close a seal water line but the valve was stuck. While attempting to close it, he felt a pop and pain in his wrist while trying to close it. He was sent to the clinic. Diagnosis was a left wrist fracture avulsion.

Employee states he was to guide 9016 shovel back into stall 17, he was in the stall facing west when he claims to have slipped on the right strip of rubber belting used to protect the floor. He did not report the incident until 12/31/09, he sought medical treatment on 12/31/09, we are unable to accommodate his restrictions.

In the process of grouting new base for a replacement pump. He was mixing grout in small 5 gal portable mixer. Finished mixing up a batch, shut off mixer tried to remove pail from mixer but pail was stuck. While trying to free pail mixer started up: his hand spun round and round causing a sprain and possible fracture. Fracture ruled out at a later date. The sprain required a splint

Employee was removing a bolt with an impact wrench. When the bolt spun without loosening, he reached into the engine compartment to find out why the bolt wasn't coming loose. Sharp edge, out of sight in compartment then sliced through his glove cutting his right index finger.

Employee was loosening case mount bolts from the frame to the case on 3C tails pump working in a bent over position he felt a strain in lower back. He was using a 3/4 inch ratchet with a 2 1/4 inch socket to loosen the bolts. Did not seek medical treatment (physicial therapy prescribed) until 11/16/09. Did not notify United Taconite of medical treatment until 11/17/09.

Employee was in the process of removing right rear wheel on 9029 Loader using impact gun. Gun popped off the bolt, employee reacted to catch it, hitting the back side of left elbow on the rim of the wheel. States elbow hurts cannot fully straighten arm. Sought medical treatment.

Employee was pulling a water hose across the grade floor of the fines crusher, "F side" just on the south side of 9F conveyor, under the #4 rod screen area, when the collar of the nozzle suddenly got stuck in a section of the floor. Causing discomfort to the employee's back. Employee has been seen by a Chiropractor.

Employee was servicing 9513 pickup. He had vehicle in raised position. States he noticed the blockheater plug had come undone. He reached up to plug the cord back in said dirt fell off the outlet & got between his glasses & face entering his right eye. First responders flushed out eye but later sought treatment at the clinc. Cleaned out eye with Burr and wrote out prescrption

Using air impact wrench to tighten bolts on case of 3b tailings pump. Impact wrench slipped out of employee's hand, due to "never sieze" on his gloves. Smashed his right hand pinky finger between handle of impact wrench and pump pedestal. Employee reported on 9/8/09 that he sought treatment at clinic. Received diagnosis on 9/9/09 statinig he had fractured the pinky finger.

Mechanic was pressure washing cooler carrying wheels in steam room. The wheel had been cleaned, standing upright. The employee turned around to clean the next wheel. The wheel behind him began to roll towards him. His partner yelled a warning but the employee couldn't hear due to noise. The wheel tipped over, fell, pinned employee's right leg/ankle to the pallet and floor.

Uniy 9595 Pit F250 service pickup truck was run over by a water truck. The incident happened by the lower pit stand pipe, the one used for filling the 190 ton water trucks. The employee operating the pickup truck jumped from the pickup truck to avoid being run over. The result of this is a sore knee requiring medical attention.

Employee opened crusher up a notch because it was set too tight, after which he moved the swing rams that open and tighten the crusher setting back to its normal position. While moving one of the swing rams he felt a pain in his low back. He did not treat nor was an incident report entered into the system until 7/15/09 when he called to state he wanted to seek treatment.

While opening the Traxol drum a small amount of the contents "puffed" from the opening and hit him in the face close to his left eye. He wasn't sure if it got in his eye or not but he flushed out his eye with water. He did not feel any irritation or burning and had no pain, but it was a possible that some got in his eye, so we sent him into the clinic to get checked out.

EMPLOYEE HAD JUST FINISHED UP GRINDING ON A LOADER BUCKET. HE WAS CLEANING UP THE SURFACE FOR THE ADAPTER TO BE INSTALLED. WHEN HE FLIPPED UP HIS FACE SHIELD DUST ENTERED HIS RIGHT EYE. THE DUST HAD COLLECTED ON THE BRIM OF HIS FACE SHIELD WHILE GRINDING. WHEN THE SHIELD WAS FLIPPED UP THE DUST WENT BETWEEN HIS SAFETY GLASSES AND HIS EYE BROW AND GOT INTO HIS EYE.

Employee was removing plugs in preparation for loading blast holes. After he had removed one of the plugs, he turned on the "chip pile" to step down onto the roadway. As he stepped, his right ankle rolled. Employee reported that his bifocals contributed to poor depth perception as he stepped off the chip pile.

Unit 26 was replacing cracked turtle(cover plate) on 18 shovel, the shovel operator was assisting unit 26. Unit 26 noticed the operator's face /cheek had some blood on it. The EE was transported to the foreman's office and the 1st responder transported EE to E.R.

The elevator governor failed causing one employee to be stuck in the elevator for approx one hour. Otis Elevator was called out to the site and fixed the problem. MSHA was contacted at 8:00am on 11-03-08.

EE standing on roll feeders & partner on floor at reducer. EE prying roller east w/ 4? pry bar, stepped back slipped slightly. EE reached North w/ RT hand to stabilize self & placed hand on reciprocating carriage rail while the carriage was traveling in a reverse direction. The reciprocating carriage wheel traveled over EE RT hand severing 3rd, 4th,& 5th fingers.

Hose was buried in mud, the employee tried to pull out the hose in the mud, felt pain in his left shoulder. Sought medical treatment 7/20/08. Physician has taken him off work at this time until MRI can be done.

Employee stepped out of the doorway at the roof entry point and onto a wooden walkway constructed of spaced 2x4s. Two of the boards were broken covered with OSB board. She stepped on the board, which apparently slipped and she twisted her ankle.

Employee was walking by the south east door - grade floor level in the concentrator. When passing by the door the wind blew causing the particle to enter his right eye. Received medical indicating treatment beyond first aid on 6/6/08. Safety was to investigate further to ensure this was work related. Did not hear back from Safety until 6/12/08 am.

Every once in a while, when moving right thumb, employee states it feels like an electrical shock going the length of thumb. Employee did not give any reason for what or why this might be work related. He in fact, sought treatment but didn't indicate to clinc that he felt this was work related. So, we weren't aware that this was diagnosed as work related until 6/12/08.

Pulling out screen on 2B4 primary screen to clean it. Using puller made to pull out screens, the screen tipped to one side causing him to fall back against 2A1 south finisher. The finisher had some rusty jagged metal which the employee fell against causing a laceration requiring 9 stitches. The employee is off work as of 4/17/08. Will file amended upon his return to work.

Employee repairing ball charging hose on 2A ball mill grabbed a 12 lb hammer to knock wedges out while doing so felt a pop in low right back. On 4/10/08 received Rcvid physician report of workability stating restrictions from 4/2/08 through 4/9/08. Restrictions ended as of 4/9/08. Employee did not work on 4/8 or 4/9 as these were normal days off work.

Employee took vehicle to Lundgren Motors for vehicle repair. As he exited the drivers door he slipped and fell. Doctor visit of 3/13/08 indicates hair line fracture with restrictions of 5 lbs lifting. Next appointment is 3/27/08. We will need to file an amended copy at a later date once restrictions have been removed.

Exited prod. truck at end of shift walked to coarse tails pocket. 20 ft from pocket slipped on ice fell on back. Feet came out from under him and he fell to ground. Employee will be off work for a few days depending on check with physician. He has wedging of th T6 vertebra or possible compression fracture.
