The mill had 2 scall man doors removed for maintenance. The repairman entered through the upper door via a ladder. He accidently put one foot into the lower door hole, lost his balance and fell onto his left side inside the mill. He was the first person to enter the mill and had not yet put a temporary light in place or covered the lower door hole. He has a torn rotator cuff.

The production utility man was disconnecting a 4" vacuum hose from a manifold and when it came loose it hit him on the lower lip and caused about a 0.25" split. He was taken to the hospital where a doctor glued the cut closed. He was immediately released to go back to work.

A repairman was hammering a pin punch with a 20 Oz hammer to drive out a pin. The hammer glanced off the punch and traveled down and hit his thumb. The thumb sustained an open fracture on the tip of the thumb and a laceration of the nail bed. He returned to modified work with the use of his right hand only.

Mechanic was hammering a wedge to hold a crusher liner in place. Hammer glanced off of the wedge and traveled down and hit his other hand. He was supporting his weight with the hand that was struck. The position was slightly awkward and left his supporting hand in the line of fire. Working surface was somewhat unstable.

The worker went to a mill bin to get the cement to flow. He attached high pressure to the internal air pads and tapped on the bottom to loosen the material. While checking for flow at the discharge, the seal came out from under the holding seal band and blew cement on him knocking off his safety glasses and getting into his eyes.

A contract laborer was assisting cleaning up a lime spill with a shovel and a big bag with several others. After he went home he noticed a burns on the upper inside of his legs. He went to the doctor the next day and the doctor treated the burns and told him to come back in 2 days.

A contract laborer was assisting a lime spill cleanup with a vacuum hose. He noticed a burn on his neck later that evening and went to a doctor on monday. It appears that some lime dust got on his neck and reacted with body moisture.
