employee was helping replace a cone liner using a sledge hammer to loosen three inch nuts from liner while striking the nut broke loos resulting in the nut falling on employees foot breaking little toe on right foot requiring 9 stitches
Employee was removing worn cutting edges from front end loader using cutting torch to remove old bolts when cutting edge fell landing on left foot breaking 5 toes
employee stepped off trailer rolling left ankle resulting in hairline fracture returned to duty after missing 2 days of duty
Employee was replacing screens, screen slipped out of hand
Employee was using a pry bar to install a wiper on a sand belt when the pry bar slipped striking a second employee in the right forearm causing fracture.

Employee was checking fluid levels on a track stacker after it shut off. Radiator cap was loose and hot fluid hit employee in face and wrist.
Employee changing fuel filter on loader. Had build up of mud on boots, slipped from loader step.
Employees were changing a sprocket on crusher feeder. Sprocket slipped, lacerating the pinky on left hand requiring stitches.
Walking around the plant site, stepped on uneven ground/rocks, twisting. X-rays showed fractured ankle.
Were replacing a conveyor belt on sander, employee was using screwdriver to travel or rotate belt when hand slipped off striking pulley resulting in stitches to little finger and ring finger on left hand.
Employee stepped out of cat haul truck tweaking ankle which has since turned into a knee injury.
While attempting to hitch trailer to pickup, the weld on the trailer broke. The trailer fell onto miner.
Employee was in the process of cutting a new wiper to replace an old one. The utility knife being used slipped off the wiper & punctured the employee's upper left leg. The employee was taken to the medical center & was given two sutures to close the wound. The employee did not miss any work nor was given any work restrictions.
EE was trying to stop the belt & another ee ran and turned belt off by releasing a lever & turning key off. Another ee gave ee a knife to cut the belt to remove ee right arm that was in tangled in tail pulley. Notified superintendent by mobile phone who called 911. EE assisted ee to pickup wrapping arm in coat. Met EMS on county road I in Donley County.
