The Rock Truck bed was in raised position when emp went let the bed down it started down normal then free fell about halfway down. Emp said EE hit EE's head on the cab but was fine, EE then went to the ER and was released to go back to work 12/24/25. EE then went to PMC Primary Care on 12/29/25 where they excused EE from work until EE sees a neurologist and they release EE.

The employee was going down the hill to dump the truck so that EE could do repairs on it. As employee was going down the hill EE lost control and was thrown from the truck.

While doing safety check on equipment, employee was putting the pin in the equipment when employee lost hold causing it to fall and smash employee's right thumb

Climbing out of bottom deck after changing screen. While holding on lowering self out, went to stand up, foot slipped went into screen, falling forward hitting chest area.

The employee was climbing on the Excavator when they slipped from the early morning dew and fell off the excavator landing on their left shoulder hurting their rotator cuff.

While on mantrip, face contacted roof, resulting in cut over eye and blackening of right eye.

Cut finger while bolting requires stitches in finger employee said he would be back to work monday 4/6/09.

Employee was found laying on the ground unconcious. Ambulance was called and employee was taken to hospital.
