The employee was going down the hill to dump the truck so that EE could do repairs on it. As employee was going down the hill EE lost control and was thrown from the truck.

While doing safety check on equipment, employee was putting the pin in the equipment when employee lost hold causing it to fall and smash employee's right thumb

Climbing out of bottom deck after changing screen. While holding on lowering self out, went to stand up, foot slipped went into screen, falling forward hitting chest area.

The employee was climbing on the Excavator when they slipped from the early morning dew and fell off the excavator landing on their left shoulder hurting their rotator cuff.

Employee was operating a Cat D8T dozer pushing coal in the clean coal pile when a cavity above the #3 feeder gave away. This caused the dozer to be covered in coal, trapping the operator inside the cab of dozer. There was no injuries to operator and rescue was successful after 2 hours.

Employee was prying a pin out on a 14H Grader and right shoulder started hurting. EE started missing work on 2/7/18

Employee was working on screen door when employee's hand slipped causing finger to get caught between the door and the coal plate

Rope broke on raw coal magnet while employee was pulling it over to clean it off causing EE to fall and twist ankle.

Employee was trying to throw the flop gate at the direct ship hopper, when the chain ratchet free spooled letting arm of the flop gate pinch his left hand between the flop gate arm and beam causes cuts and a fracture to his index finger of the left hand.

Burning paint off of cutting edges, hit spot that must of had rust under paint. It popped off putting something into eye.

Stepped off stairs onto concrete pad that had a little patch of ice on it. Slipped causing him to fall and twist his knee

Coal sample operator slipped while exiting the auger control room and strained his lower back.

Operator backed a dozer into an uncovered voided area sliding the dozer backwards. Neither operator nor equipment became engulfed by coal.

Mechanic moving pins for a loader into storage, picked up a pin (approx 45 lbs) and strained his back while moving. Sought medical attention from family physician.

Operator was dismounting from Komatsu Excavator when his foot slipped and he fell resulting in a bruised left buttock

Operator was dismounting and mounting a Volvo articulating truck throughout his scheduled shift. Later in the evening, EE noticed pain in his knee and notified supervisor via text message that evening and sought medical tratment the following day.

Employee was repairing a belt press belt and cut his finger while attempting to cut a zip tie.

Steps were wet. Fell & hit elbow.

Subject was attempting to use a hoist device to manually open the doors on a mud bin. One of the hooks connected to bin failed, thus straightening out. The hook came free, striking the back left shoulder of the subject, causing a knot to form on the back of this shoulder. Subject went to the ER and was released and able to return to work.

Employee climbed a ladder in the sampling room to check on a plugged crusher. While working from the ladder employee allegedly twisted her knee and back.

EE was dressing out conveyor section on belt truss when the structure rolled over, due to unstable ground, causing sprains to EE's back and left knee.

The EE was installing rollers on a belt truss structure, when the strucure rolled over, resulting in a laceration to the back of his head.

Air hose on air compressor came apart, hitting employee in the right hand causing contusion. This incident not reported until 7/18/07.

EE was setting a ladder in place and suffered a pulled muscle when the ladder slipped and he reached to catch it. EE did not seek medical treatment until 6/23/07.

Employee's foot slipped while he was climbing on head chute to cut crane loose. Employee twisted his back when he lost his footing causing muscle strain to lower back.

EE was installing bent plate around screens in a kneeling position. Whe he stood up he experienced a catch in his right knee, causing pain & the inability to straighten it. EE missed 5/7 & 5/8 and 6/14 - 7/5/07 for knee surgery. This office notified of surgery and lost days 10/23/07. (addition to narrative per MM 10/26/07 #10)

Walking to ck belt on plant belt turned and headed back to picking table slipped fall Other employee went to talk to employee ask it needed ambulance. EE stated felt he could drive operation.

Employee twisted his knee when he was unloading steel from truck and kicked a piece of material he thought was packing material but it was nailed to the ground.

Employee was walking beside #1 belt in pit area of the plant when a 6""-8"" rock fell off belt and hit him in the neck.

Employee was cleaning coal from shute when a piece of slate fell out of shute and hit at his knee causing him to twist to get away hitting his back on hopper.

Employee was walking up a stair when he slipped and hit his right knee.

EE WAS STANDING ON A LADDER CLEANING OUT BELLY PAN WHEN HIS FEET SLIPPED OUT FROM UNDER HIM CAUSING HIM TO FALL ON HIS BEHIND.

EE WAS CUTTING METAL THE NIGHT OF THE 25TH. WHEN HE GOT OUT OF BED ON THE 26TH HIS LEFT EYE WAS SWOLLEN. HE WENT TO THE EYE DOCTOR WHO FOUND A PIECE OF METAL IN HIS LEFT EYE.

EMPLOYEE WAS MOVING A DRIVE MOTOR FROM A BELT ON THE 23RD OR 24TH WHEN HE BUMPED HIS LEFT KNEE INTO THE BELT STRUCTURE. KNEE STARTED SWELLING ON THE 26TH. X-RAYS SHOWED THE KNEE WAS NOT BROKE.

EMPLOYEE WAS CHIPPING SLAG AWAY FROM TRACK ON WASH BOX WITH A HAMMER WHILE OTHER EMPLOYEE WAS HOLDING THE SPRING LOADED TRACK WITH A SLATE BAR. THE HAMMER HIS THE SLATE BAR RELEASING TRACK ON EMPLOYEES LEFT THUMB SMASHING ABOUT 1 INCH ON THE END OF IT.

EE WAS MOVING A SMALL BUILDING ACROSS THE FLOOR OF THE PLANT. EE WAS GUIDING THE BUILDING WHEN IT PUSHED INTO HIS RIGHT SHOULDER.

EE WAS DROPPING A RAIL CAR DOWN THE TRACK, THE RAIL CAR BUMPED A RAIL CAR IN FRONT OF IT CAUSING THE EE'S LEFT LEG TO STRIKE A STEP ON THE RAIL CAR.
