EE was lifting slabs of sandstone to place them on a pallet, when he sustained pain to his lower back. Physician diagnosis: Low back sprain/strain.

EE was standing a slab of sandstone up on it's edge, when it broke in two. The bottom half of the slab struck his lower left leg and left foot, causing contusions to leg and foot.

EE was loading slabs of sandstone onto a pallet by hand when he sustained pain to his lower back and left groin areas. Physician diagnosis was strain - sprain lumbar and left groin. He will be off work for an undetermined length of time.

EE was lifting pieces of stone onto a pallet when he sustained a sharp apin in his lower back. Dr diagnosis, acute back strain, EE will off work until authorized to RTW by Dr.

EE was splitting sandstone using a 4-lb. hammer and stoke chisel when a metal fragment broke off the chisel head and pasted under his safety glass lens striking his left eye. Metal fragment was removed by a physician and prescription medicaiton was given. He was release to full duty.

EE was splitting sandstone using a stone chisel and 4lb hammer, when a metal fragment broke off the chisel head and impaled in his right thigh. EE received medical treatment for the removal of the metal fragment. EE received tetanus booster.

EE was injured on 11/27/07, when a piece of stone broke away from a large slab of stone he was placing on a pallet and struck his elbow. EE declined medical attention at that time. On 12/12/07, EE requested medical treatment for the injury, which occurred on 11/27/07.

EE was splitting a slab of sandstone that had a fracture crack in it, which he did not notice. While splitting the stone, it broke in half, the stone fell and struck him on his right ankle. He sustained a lacertion and contusion. EE will be off work until released by physician.
