EE was dismounting the clean coal crusher, EE stated he was coming down backwards and put his right leg down first to support him when his knee buckled and he felt a very sharp pain.

Employee was picking up belt structure in #3 entry on 3-A belt when rib fell hitting employee's left arm.
While taking back tire off of grease truck the tire was about to fall and the tire smashed the EE's hand.
Injured claims he was stepping out of the truck and his right pant leg got caught on fire extinguisher. As he started to fall he attempted to catch himself on the truck door, missed and landed on the ground with his left foot between two rocks and his head hit another rock. Injured stopped to clean off headlights.
Employee stated that as the manbus he was riding went through the truck doors on way outside, the doors shut, causing the door to strike the manbus, bouncing his foot out of the ride. The doors then struck his foot as the bus struck the doors the second time.
Employee stated that as he tossed a bag of trash off of the supply car, he felt pain in his right shoulder.
EE said that he started to bolt and couldn't lift the steel with right hand. EE worked all his shift and when he got outside he told his supervisor he hurt his right shoulder and did not know how it happened.
The employee was standing on a dozer. He was reaching for a grab handle as he slipped and fell against the tilt jack. The employee then fell to the ground.
Employee was moving flypads in #3 entry. Was to move the fly-pads straight up entry but went in the cross-cut where the shuttle car was running. The shuttle car ran over the employee's left foot.
EE stated he was walking and hit his head on a roof bolt plate. EE hurt his neck and back.
EE got off of shuttle car tripped and fell on his right knee at the feeder.
Employee stated that he was stepping off of the ladder on the fork lift. As he stepped down he slipped causing him to twist his knee. The area was muddy and wet.
Employee stated that as they were pulling a rock off the miner with a chain, the rock came loose from the chain, struck his hand and pinched his ring finger against the side of the continuous miner.
At the end of the shift, employee was riding in a 4 passenger rail mounted mantrip exiting the mine. The operator of the mantrip failed to stop as the trip approached a set of air lock doors resulting in the mantrip crashing into the doors.
While slowly driving a powder truck down a ramp, the engine died as the truck slid sideways on snow and ice. While sliding, the truck slid into an unfrozen area, caught traction and turned onto its side.
EE was moving a rubber tire ride out of his way when he struck his elbow against the power center.

The employee was preparing to spray the roof and ribs to fireproof for a charging station. He checked the machine for blockage and plaster sprayed into his eyes.
Back wheels hit a hole in road (sink hole) and truck laid over on its side.

While trying to knock a pin out of the push arm on a dozer, a piece of the hammer broke off and embedded in the employee's leg.
The equipment is in good condition. The ribs are soft and unstable in area. Employee was driving by the weakened rib, rib rolled out into the deck of the buggy. He was struck on the left wrist by a piece of coal 6" x 8" x 3-4" thick.
Employee was sitting down into shuttle car and when he pulled his jacket up the utility knfe, which was hanging on his mining belt and not known to be open, cut the end of his thumb.

Employee was working in rebar and scraped his hand on some rebar. He refused to go to the hospital at the time but went 3 days later when it was infected.
EE came in at start of shift complaining of shoulder pain, I asked him if he knew what happened and EE said "No", but it had been bothering him for a couple of days. He said he was up all night hurting, I told him he had better get it checked out.
