Employee was busting rock, attempted to strike a rock 4in thick, 2ft. wide, when EE did, the rock flew toward EE hitting EE's right hand. EE was taken to ER for treatment where EE received x-rays and 6 stitches. EE was released to return to full duty the same day.

Non injury roof fall was discovered 2/26/18 at 6am on the return side #4 entry spad# 1 break inby 1814. Cribbed area off, did not impede travel &/or hamper ventilation. Approximately 8' high, 20' wide x 20' long.

Employee was attempting to bust a block of coal and accidentally contacted the roof with the hammer and dislodged a rock which fell and struck employee's hand causing a laceration.

The 3rd shift electrician was crawling back to buggy while scooting a hydraulic reel motor, when a rock fell from the top and struck EE on the right hand between the thumb and index finger. The next day it swelled up and after a couple of days it wouldn't go back down. EE went and had x-rays and it was determined to be broken.

Rock Fall occurred at spad 1005 neutral beside #7 belt. Rock was 20'x 22' from 0 to 42". Area was cribbed off and did not impede travel or ventilation.

EE was backing out of the #3 heading with a load of coal. He crossed under the roof bolter cable with the back of the scoop and then he turned his head to be certain the front would go under the cable then he ran over a lump of coal and it pushed him against top.

The operator of the #3 roof bolter was attempting to put another cable bolt up (after having hung one) he started a hole adjacent to the previous hole when a piece of draw rock became dislodged from the top and struck him on his left wrist.

The accident occurred when the victim was attempting to back out of the battery charging station on his permissible buggy and while looking over his shoulder he backed into a cable bole plate and it cut above his eye.

The victim was attempting to pick a rock of the drag chain when the off-side rock picker attempted to bust the same rock with a sledge hammer. When the sledge hammer came in contact with the rock the victims left hand was still there and his pinky finger was smashed.

The #3 roof bolter operator was attempting to connect two pieces of drill steel together when he hit the rotation lever & the connector on the steel broke and swung around and struck the operator on his left wrist.

Rock picking and shoveling belt. Complained of abdominal pain and upset stomach. Walked into mine office to see MET, sat down in chair, and fell over. Alleged Miocardial Infarction (Heart Attack). No sign of a work related accident. This report is informative only due to it occuring on mine property.

After 9 days of rain & 5.75" of rain on Saturday May 9, 2009 and upon further exploration of the mine. Water had entered the mine filling a low area and some of the face area. The water traveled by the face by penetrating some stoppings and traveled the low area of the mine. The mine was not working due to area flooding.
