Injured party was working to repair a leaking hydraulic fitting. When disassembling the fitting, the O-ring failed spraying EE with hot, pressurized fluid.

Miner was clearing brush with a chainsaw on the outskirts of the quarry. The chainsaw kicked back and cut into the miner's boot past the steel toe causing a laceration on the miner's foot. Surgery was performed three days later resulting in the amputation of one toe on the right foot.
