While laying out the drill pattern, the Miner slipped on mud causing Miner to fall onto hand and break middle and ring finger.

While cutting a block of metal from a sheet of steel, the block of steel was levered by a support block once it became free. The block struck the Miner's helmet and then the Miner's forehead which caused a cut on the forehead.

While descending the stairs at the shower house facility after punching in, the Miner slipped causing left arm to be twisted backwards by the handrail.

While shoveling near the crusher station at the preparation plant, the Miner was struck on the left shoulder by an unknown object or debris. The Miner was seen by a doctor and was diagnosed with a bruise on the shoulder.

While operating the 777B haul truck, the operator jarred EE's back in the seat

While the Miner was removing a mine timber from the feeder area, EE strained EE's back.

After changing a deck screen, the Miner was crawling out from between the screen deck and misplaced their footing causing them to dislodge the board placed as a safeguard over an opening. The Miner fell through the opening and struck their head.

While activating the dump lever of the haul truck, the Miner received a spider bite to the lower left arm which began to swell due to an allergic reaction.

While installing a deep well pump, claimant fell into an adjacent ditch and struck leg on a rock causing a laceration which later became infected.

While responding to an electrical problem EE heard on the radio, the miner turned to rush toward the plant. EE tripped over a piece of steel pipe at the welding shop and fell to the ground where EE struck EE's head on a piece of steel and scraped EE's upper right arm.

While pressure washing the interior of the preparation plant, the miner pulled the hose to gain more slack. The hose caught an a loose piece of metal on the floor above the miner causing it to fall and cut the miner's left arm.

While backing up to the dump, the miner backed through the berm causing the Caterpillar 777B haul truck to roll over onto the embankment, coming to rest on its canopy and nose. The miner was wearing seat belt, but received facial injuries during the rollover.

While replacing a cooler on the Atlas Copco D65 drill, the Miner was underneath the drill securing the bottom bolts. Dirt fell from the machine and struck the Miner in the face resulting in a a paint chip in eye.

While striking a metal bucket shroud to remove it with a hammer, a sliver of metal came off the hammer and embedded into the welder's left leg.

While walking around the front of the loader to talk to the foreman, EE collapsed. No trip, slip or fall was attributed to the collapse. An ambulance was called and it was later learned that EE's hip was fractured and that EE has been experiencing pain for at least one month prior to the incident.

While looking down a drill hole to ascertain why the drill steel was stuck, employee's hardhat fell off when employee bent over. Subsequently when the employee stood up, employee hit head on the drill's boom.

While operating a Komatsu D275 bulldozer, the miner ran over a rock that was sticking out from the ground. The dozer teetered over the rock causing it to jar against the ground. The miner felt discomfort to lower back and is being treated for the discomfort.

While dismounting the Komatsu D375 dozer, the Miner slipped from the push arm causing them to fall and break a bone in their hand on the ground.

While operating a Komatsu HD785 haul truck during loading operations, a large rock fell from the Komatsu PC3000 backhoe that was loading the truck. The rock struck the tail end of the truck bed causing the front of the truck to bounce on the ground. The miner jammed leg causing discomfort to back and leg.

While working on the feed hopper at the plant, the Miner scraped EE's left arm against the edge of a sharp piece of metal and cut EE's lower arm. EE went to the hospital where EE received two stitches.

While mounting a new motor on a frame in the cleaning plant, employee was attempting to insert the bolts from the bottom of the frame while another employee held the motor. When the motor was lowered, the employee inadvertently had tip of thumb on top of the frame were it was pinched.

While helping the welder spread out a tarp for protection from the rain, Miner was splashed in the face causing a small metal shaving to get into the left eye. Miner did not immediately seek medical attention but later had it removed by a specialist.

While repairing the support brackets for the utility body underneath a service truck, the employee attempted to remove the bolts from the bracket. Dirt and rust fell from underneath the truck and struck the employee on the forehead and fell underneath safety glasses. Dirt or rust went into the employee's eye.

While troubleshooting the wiring for the rear lights on a Caterpillar 992 loader the employee slipped and fail when crossing from one side to the other.

After a day of welding on Monday, July 17, 2017, the employee awoke at approximately 2:00 am the following morning with a burning sensation in their eyes. The employee reported to work on Tuesday, July 18, 2018 but left to see an eye specialist once they were open. The doctor diagnosed the employee with welders flash and prescribed eye drops.

While grinding a weld, a piece of ground metal entered the eye from below and underneath employee's safety glasses. EE was taken to an eye Specialist where the foreign material was removed.

While changing the outlet fitting on a trailer mounted pump, the wrench slipped off the bolt and struck employee in the head above the left eye at about the eyebrow level. Employee was treated on site and taken to the emergency room where they received five stitches.

EE assisting in removing a cable from a 4600 drag line. The cable came loose from the drum and struck EE on the bridge of the nose. The nose area was swollen and bleeding. EE was taken to the local emergency room for evaluation and treatment.

EE was pressure washing a service truck when EE experienced severe pain in EE's right shoulder area. EE stopped what EE was doing, notified EE's supervisor and went home. The following day EE advised that the pain had not subsided. We scheduled an appointment with our panel physician for the same day.

EE was adjusting the tension on the discharge belt of the portable crusher. EE was using a large wrench. EE felt pain then numbness in EE's right shoulder. EE advised EE's supervisor and was subsequently transported to the emergency room at the local hospital.

Plant operator was replacing a roller on overhead conveyor. The roller brackets were not correct for the roller being replaced and the roller came out of the brackets striking the operator on the bridge of EE's nose. At present, we believe that the operator sustained a broken nose. EE is in the process of being examined by an ENT specialist.

Ground was frozen and snow covered. Operator noticed blockage in coarse coal chute and moved rapidly towards chute to clear it. Stepped on frozen puddle hidden by snow and right foot broke through ice causing him to fall to the ground. He strained/sprained his right knee and immediately reported the injury to his supervisor. He was sent for medical treatment.

Employee was repairing a vibratory screen. The screen elevated and employee was working 2-3 feet off of ground. Employee stepped down from screen and lost his footing. He landed sitting with his leg folded under him. His knee was over extended and he had pain in his lower leg.

A 777b haul truck ran into the back of a D10N dozer causing the dozer operator to be jarred forward subsequently he was sent for evaluation at the ER for neck and back pain

Welder was making a repair with some pre-cut pieces of steel. While lifting one of the pieces of steel into place the welder complained he felt a strain on his right side. His foreman sent him to the ER for evaluation.

Burning out steel plates in a CAT 777B haul truck. He was preparing the truck bed for relining when a shard of metal landed on his boot and melted through contacting foot. Treated at home for 2days with ointment. Today the area contacted was red ad irritated and subsequently we sent EE to the ER for evaluation. Recommendation three days with no boots or tight fitting shoes.

Employee was chipping at a frozen lump of coal wedged in the hopper of the re-screener. As he jabbed at the lump with a breaker bar a piece of coal flew off and hit him in the eye causing irritation. Sent employee to E.R. to have eye examined. Flushed, medicated and returned to work with no restrictions

Operator jarred his lower back while driving a Euclid R90c off-road haul truck. The bracketing for the seats suspension appears to have broken allowing the seat to shift forward

A rock had become wedged in the Badger Jaw crusher. Employee attempted to dislodge the rock using a pry bar when he claims he felt a pull and burning sensation in his lower back

Mechanic was working on DML track drill when he stepped on a rock while exiting via the tracks. When he stepped on the rock he lost his balance and twisted his left ankle.

Operator was changing drill steel bit when the chain wrench broke and a piece of the bar hit the back/side of the operators left knee. No initial treatment was sought. Area around the impact began to swell after a couple of days and that's when the operator sought medical attention

Laborer was getting stock screens from an elevated storage area when he stepped in an opening and fell landing on his right side.

Employee claims to have slipped off ladder coming down from a 777 truck. States he landed on his back. He did not notify his foreman of accident and states no witnesses. He did not report anything until 2/1/2012 at which time I opened a claim with our insurer and sent him to the ER for evaluation.

Mechanic was working on a truck in the garage. He had positioned a ladder against the side of the truck and was coming down off the ladder with a part he removed when he lost his balance on the second to last step and slid to the ground impacting his right knee.

Employee went to foreman on 8/23 and stated he had hurt his back on Sat. 8/20 lifting a 5g pail of oil while doing a service. The employee left on the morning of 8/23 after telling the foreman of the incident. The accident was not reported until three days after the alleged incident. Employee states there were no witnesses and not sure how it happened

While driving a Euclid haul truck over a bumpy haul road the driver jarred his back.

Employee was standing on the tracks of a CAT D9L dozer while passing the fuel hose to the operator. The employee slipped on the wet, muddy track and fell to the arm for the dozer blade. He then also further fell to the ground and hurt his shoulder trying to stop his fall.

While welding on a bucket adjacent to another welder the employee was affected by the arc of the other welder. The employee's eye was scarred.

At the beginning of the shift the employee tried to climb into his haul truck but slipped on the ladder falling, and hurting his back.

While using a torch to cut off an old wear strip, the wear strip broke loose striking the employee in the face.
