Employee was dismounting the loader at the end of his shift when stepping off last step to the ground his foot was placed on a loose rock. He twisted his ankle.

Employee was working on a pump. The employee was pulling on a discharge hose when he lost his footing and tripped. Employee stated he was fine and returned to work.

Employee had a 2" high pressure water line blow out. Pressure of the water pushed him over handrail, causing him to fall 34 feet. Employee landed on an electrical tray, then rolled off, falling to the ground. **EE was hospitalized multiple times due to his injuries and he died on July 26, 2012**

Employee was dismounting his loader and his foot missed the bottom step. He fell approximately 30 feet to the ground landing on his feet and jarring his back. The attending physician diagnosed the injury as a simple sprain and prescribed pain medication and rest. Employee was told he could return to his normal duties on his next scheduled work day.
