Fall from scaffolds, walkways, platforms

Employee had a 2"" high pressure water line blow out. Pressure of the water pushed him over handrail, causing him to fall 34 feet. Employee landed on an electrical tray, then rolled off, falling to the ground. **EE was hospitalized multiple times due to his injuries and he died on July 26, 2012**

While servicing a tire on the left front of a WA600, the technician placed a hydraulic ram between the axle suspension assembly of the WA600 and the inside flange of the wheel. When the bead of the tire came loose from the force of the ram pushing on the flange, the ram slipped and hit the technician in the face.

Struck by falling object

EE was operating a haul truck, hauling clay to the dump. EE dismounted the cab of the haul truck and walked toward the excavator's work area unannounced. At this point a chunk of clay came off the face and struck the EE in question on the outer right thigh. EE was seen by company clinic and was issued work restrictions.

Over-exertion in pulling or pushing objects

EE was changing a screen on the top west tertiary screen. EE was using a pry bar and put left foot for pressure on the pry bar to loosen the screen, while the right foot was used for balance and support behind EE. EE felt a pop in the left ankle and it began to swell up.

Struck by falling object

IE was helping the operator of a skid loader change the bucket. The operator blocked the bucket but the release lever jammed and they could not release the bucket. Another EE had the operator raise the bucket so EE could hit the lever with a wooden block. The lever released the bucket, then falling onto both feet of IE causing a contusion.

Employee injured wrist while using a shovel to clean up spillage at the secondary crusher plant

Struck against stationary object

3 employees and 1 contractor were working on the freshwater pump. They uncoupled the pump from the pipe to turn the valve around when the barge started to sink on one side. The employees/contractor climbed off the barge and swam to shore. The injured employee cut L middle finger while climbing off, requiring 2 stitches.

Struck by falling object

Employee working under a conveyor when the electrician's pulling cable and knocked a rock off the cable tray and struck employee's hard hat. Rock punctured the hard hat and cut the back of employee's head. Employee received 4 staples in head.

The screen bearing locked up employee was grabbing the v-belts to unlock the bearing when it came unlocked rolling their fingers in between the sheave and v-belts then employee pulled their finger out

Employee was dismounting the loader at the end of his shift when stepping off last step to the ground his foot was placed on a loose rock. He twisted his ankle.

Employee was working on a pump. The employee was pulling on a discharge hose when he lost his footing and tripped. Employee stated he was fine and returned to work.

Fall from machine

Employee was dismounting his loader and his foot missed the bottom step. He fell approximately 30 feet to the ground landing on his feet and jarring his back. The attending physician diagnosed the injury as a simple sprain and prescribed pain medication and rest. Employee was told he could return to his normal duties on his next scheduled work day.
