Showing all 5Ladder was leaned against a 20 ft wall. Ladder should have been leveled and tied off before worker attempted a small drill hole near the top. Ladder slipped, and worker fell. Broken both wrists/arm, finger, impact left elbow, left knee, left shoulder, and ribs. Face injury, teeth.
Employee reported this injury at least one month late, continued to work. Mentioned headaches, co. immed. scheduled Dr. apt. He fell 8 ft off ladder, in mid/late January. Descending a ladder, bottom slipped, fell to the floor. Got up, said he was fine, continued working. Four Witnesses.
Employee was lifting a Korfman chain saw with a hydraulic jack to level it for a cut. He had the jack set in 2 inches of mud, and the jack tilted to one side when it took on the weight of the saw. The machine came toward the wall and pinned him at the knee. Injured left knee, chipped small piece of bone on patella, bruised knee area.
Outside of quarry checking discharge water line. Stepped off a rock about 18 inches high into snow-covered area. Stepped on rock, twisted left ankle, resulting in strain and fracture.
