Employee was welding an object over head. Hot slag from welder dropped in right ear.
Injured person is not a direct employee of Omega Surface Mining, LLC. EE is a truck driver employed by a contractor to haul coal from Omega's jobsite. EE came to the jobsite and got out of EE's truck to see if EE's truck bed was clear; EE slipped and fell on ice injuring EE's left hip.
Employee was removing the hydraulic tank from a 992K Front End Loader; the pressure in the tank caused the lid to shoot upwards striking the employee in the forehead.
Employee was cleaning an area around #1 dodge coupling with starting fluid. Mechanic was cutting lid off with a torch. Fumes ignited and blew back on employees right hand and right side of face.
Employee was removing a pin from the right angle of the Highwall Miner with a pry bar. The pry bar kicked back from the tension and pinning EE's left hand on the tail chain and striking EE on the head.
Right index finger was smashed while placing the dozer cutting edge on the tracks.
Employee was placing a strap under a 36 inch corrugated drain pipe down in a ditch
Cross member of lifting jack fell and landed on left foot.
Employee was welding on the bucket of a loader when felt a strain in lower back when ee went to re-position self.
Stepping off left side of rock drill and when employee twisted body ee felt a pop/tear in right hip.
Employee was using a slate bar to pry a pin out of the right angle on the highwall miner. The bar kicked back bowing up employee's arms and throwing employee backwards. Fractured left elbow.
Employee was attempting to pressure test an after-cooler with 30 psi. The test instrument slipped out of the in-take of the after-cooler. Employee then instinctively attempted to grab the testing apparatus causing a cut to EE's left thumb which required 5 sutures to repair.
While in the process of tramming the Highwall Miner to a new pit, the track popped and a piece of metal flew off striking employee on the left wrist.
Caught finger in the side door of the generator. injury to left middle finger.
Employee was standing nearby watching another employee put in a bit ring (insert) when a piece of metal sheared off striking employee in the left leg
Employee slipped and fell to the ground when attempting to mount an excavator.
Employee was swinging a sledge hammer when it slipped out of his hand causing him to twist his back
Ascending ladder with a 5 gallon bucket of oil and momentarily lost footing causing a strain when he regained his footing

Working under hood of Komatsu HM400 truck and sprained/strained his back
Employee was working on the highwall miner hose chain using a metal bar and strap to lift the chain. The strap slid down causing the pressure to release on the bar. The bar came loose and hit the employee on the forehead above the right eye.
Rish Employee was standing approximately 20' away from two other individuals whom were associated with another organization who were attempting to free the rollers on an excavator. One of the employee's associated with the other organization utilized a hammer to contact the roller and that is when a sliver of metal entered into our employees' upper torso area.
Blowing slag with an air nibbler when a piece of metal enter left eye
Driver was opening lunch box 2 hours into the shift when front wheel of truck struck beam damaging rim and tire. He signed form stating carelessness was the cause of accident and was sent home for three shifts. This was on 6/9 he made no mention of being injured. On 6/11 he called and stated he needed medical information for filing workers compensation claim.
Breaking bolt loose on rocker box of loader when wrenched slipped and felt a strain.
Employee was using a slate bar to remove pins from the right angle conveyor as the pin was dislodged, the shifting weight of the conveyor caused the slate bar to pivot upwards, causing the slate bar to strike him on the right cheek.
Employee was getting off equipment to replace a grease tube on hammer. While he was exiting he slipped on track falling. He had a hold of hand rail and held on to try to stop his fall. His body weight was on one arm and he twisted around hurting rt. shoulder.
Dozer operator was widening the road in preparation for moving the Highwall miner when the cutting edge of the dozer blade contacted a low pressure polyethylene coal bed methane 8" gas line. This contact caused the cutting edge to puncture the line releasing methane. the dozer operator was the only EE in the area. He immediately contacted the foreman who accounted for all EEs.
A rock was stuck in #1 tailpiece (approx 200#). Employee was attempting to remove rock and injured shoulder attempting to pull it out.
EE worked on 8/17/11. Did not report any injury. EE called on 8/18/11. He stated he had something in his eye and was going to get checked out. Upon discussing incident with ee he stated he did not know when Dr where he got something in his eye. EE wears prescription glasses at all times. The company protested claim to workmans comp but was denied on 9/30/11
EE was operating a roof bolt machine, installing roof bolts. EE had drilled a hole and was removing steel from drill head. While removing the starter steel, a piece of rock (24" X 8" X 3") fell between the machine canopy and the ATRS, striking EE on the right hand. EE was wearing gloves. Hand not broken, no stitches, but hand was lacerated.
