Struck by falling object

Employee was spotting a roof bolt when a small piece of draw rock fell and struck ee on the left hand. The result was a hairline fracture of a metacarpal bone on top of the hand.

Employee was assisting in the process of installing waterline when the accident occurred. EE had left hand inside the waterline in an attempt to align it when another employee pushed the waterline inadvertently smashing left pinky finger between the waterline and the rib, causing the tip of the finger to be removed.

Company employee (not mine employee) slipped while exiting excavator and cut right palm on a piece of metal bracing concrete barriers for pond ee was removing silt from.

Employee was cut on the left middle finger by a small piece of drawrock while attempting to install a crib along the roadway.

Over-exertion in lifting objects

employee was lifting a slide pan for an MBC when EE strained EE's back. Worked with it until doctor took EE off work on 10-18-16.

Employee was attempting to install an hydraulic pump motor on a Cat scoop when he strained his right shoulder. It was determined on 10-22-15 that medical treatment would be required.

An unplanned roof fall occurred in the crosscut between the no. 4 and no. 5 entries at break #25 on #4 belt. (Approximately 20 breaks outby the face location). The roof fall measured approximately 20' wide, 14' long and 3'-3.5' thick.

Employee was using a 10 lb. hammer to remove a pin on steering jack on scoop when handle of hammer slipped out of hand striking employee on left thumb breaking bone in thumb.

An unintentional roof fall occurred at the 21st crosscut just off of the intake air course on the no. 1 roadway. The fall measured 10' by 8' by 3.5' thick.

Struck against a moving object

Employee was moving a permissible mine Buggy from #3 intersection to #4 intersection to allow the bolt machine to move across the section. The Front tire on mine buggy was lodged in the mud causing steering wheel to spin(recoil) in reverse direction thus striking left side of hand resulting in broken bone in left hand.

Employee was cutting a piece of belt to make a skirt rubber for the stacker belt in a confined space, when the belt slipped and struck his right palm. The laceration require sutures.

Employee alleges that he strained his back pulling miner cable.

An unintentional roof fall occurred at break NO. 25 on the intake roadway of the No.1 Mains. The fall was approximately 6 ft. thick, 16 Ft. wide and 35 Ft. long

Employee was pulling rock dust on a shovel when he reached to grab the belt structure for support and grabbed the roller on the structure and the belt was running the belt roller pulled his right index finger into the roller causing a cut and a broken finger.

At crosscut # 4 on the primary escapeway of the #3 portal a rockfall in the intersection occurred no one was in the mines at the time of the fall the fall was discovered by the foreman on his preshift no one was injured water and loose laminated shale was the cause of the fall

EE was operating a roof bolter. Attempting to put a bolt through a piece of 3"" X 5"" wood and while drilling the hole ee had hand on drill steel and in the process drilling it jerked his pinky finger off on left hand.

Over-exertion in lifting objects

Employee was lifting a 50 pound speed reducer, he said it slipped in hands and he caught it again, causing pain in stomach. Employee did not report accident until next day, 11-05-08, after he worked half day he said he was going to Doctor. The employee called and said he will need hernia operation.

Struck against stationary object

Injured said he cut his finger while unloading roof bolt plates. On 10-3-07 went to Dr. 0n 10-09-07 with infection on right hand little finger. Reported accident on 10-19-07 to foreman. He worked till 10-10-07 and called in sick, reported to us on 10-19-07 it was the result from cut on 10-3-07.

Conveyor on a bridge unit was stopped up with coal, while removing the coal, EE dialocated his shoulder. He said his shoulder had been dislocated before and it was an injury that happened before. Shoulder sometimes became dislocated.

While driving scoop outside, the injuried person hit a hole in roadway taking steering wheel out of hand and hitting on top of hand.

Operator was operating a bob tail jeep in seam height of 26 inches. The operator ran over a rock in roadway that bounced him into the top hurting his ribs.

a rock fall was found during an exam of the left return entry, the fall was 4 breaks or about 250 feet underdround. It fell in the intersection and was approximately 4 feet thick, and 20 feet long, 20 feet wide. no one was injured by this incident.

He was dumping coal with scoop, when dump door got caught between pan and scoop. He picked up door and strained chest. He went to Dr. and Dr. told him to take off 1 day. He came back to work 1 day later and chest started to hurt again went back to Dr. and hospital found dislocated rib. Goes back to Dr. on 10-18-05.

Roof Bolter was drilling hole in roof when drill steel hung in top, roof bolter operator was trying to pull steel out of hole when he push wrong lever on roof bolter mashing left hand.
