Showing 50 of 224An unintentional roof fall has occurred in the #9 Entry at the 6XC of the S1C. The fall was approximately 30 feet in length and 18 feet wide by 14 feet high.
The encoder shaft that has electronic transmitters to send back to the PLC to let the hoist no its location inside the shaft was damaged and would not allow the transmitters to relay information back to the PLC for the hoist to operate.
A mine mechanic was operating a diesel mantrip along the M5 Travelway when it appears the vehicle struck the coal rib and rolled on its side. The mechanic was partially ejected from the ride and the canopy fell on ee chest.

The utility company supplying the power checked their lines and they discovered a line that had been hitting together.

Injured broke the tip of 4th finger on right hand, after hitting it with a miner bit. While trying to clean out the clip area for holding the miner bits in the insert, the injured hit finger with a bit causing a small fracture on the tip of finger.
