Operator was bolting up 4540 heading. He went to repair a leaking hose when the right rib failed resulting in a buried bolter. He was able to escape over the top of the material.

While spraying shotcrete the dry pot stopped. The miner stepped away from the face as the dry pot was being worked on. The miner briefly pulled his attention away to check the progress of the dry pot when a rock rolled off the sloped face on to the muck pile and down to the flat. The rock continued rolling in to the miner's right leg resulting in a contusion.

While trying to align striker bar to drill steel, the boom on drill became in a bind, when it came loose from the sill it swung to the left striking his hand (left hand middle finger).

Operator was exiting the Dozer on the Leach Pad, he stepped down on a rock, losing his balance and twisted his ankle.

The Operator was operating a loader. After shutting down the Loader, he was climbing down the ladder to the ground. He had his lunch box and jacket in this hand. While climbing down he fall from the ladder hitting his head on the ground. He was unconscious for approximately 2 minutes and had a cut on the back of his head.

Loader Operator worked all shift loading hopper at Crusher. Looking over his left shoulder to see behind him during the shift. At the end of the shift his neck started clamping up. Later that night his neck felt worst and his leg was beginning to become numb.

Accident occurred with the employee tripped over his own feet, falling backwards on a concrete floor located in the Process Plant. Employee reached back with his left hand to catch himself and strain his left wrist.

Twisted left knee while pulling fuel hose from the Lube Truck. ER doctor x-rayed knee, placed a brace on it and gave the employee crutches and returned the employee to light duty. After employee was examined by an orthopedic, he was placed on loss time status.

Operator was clearing a clay-plugged transfer chute with a pry bar. The operators left hand slipped off the pry bar and got pinched between the bar and the chute causing a laceration of the left middle finger. The laceration was cleaned, stitches were required and prescription for pain killers was given to the employee. The employee was given three days off.

Technician was loosening the flange bolts next to a valve assembly to change a gasket. The line was charged with heated Sodium Hydroxide. The technician drained the line via a bleed valve upstream, but noted that there was little discharge. As he loosened the flange, the liquid in the pipe sprayed into his face. He suffered minor chemical burns.

The hammer was inspected prior to use. Was not loose or damaged. Was used several times prior to incident. Hammer had a wooden handle & was 2 years old at least. After looking at the handle after incident, the handle looks to have shrank, causing the head to slip off and strike the employee in the head. Laceration to head 11 stitches. Full release.

Employee backed his truck up to a light plant to move it.(LP21) To hitch up the trailer, he physically moved the trailer tongue to the truck. He felt pain in his back and legs. The physician said he has Sciatica and swelling at the junction of S5-L1 in his back. He prescribed steroids, muscle relaxant, and pain medication, and was released back to work on light duty.

An electrician was testing the power center for an 800hp, 4160v pump. His meter was undersized and became damaged by high voltage and carried a current. The probe arced to the structure and an arc flash resulted causing burns to the electrician. No electricity was contacted by the injured.

Miner was reinstalling guards on CV10 after repairing the pulling and an other miner pushed on the guard smashing his hand between the guards. (crusher was not running at the time).

Employee was injured when he removed a cap from a pressurized (100 psi) air line without relieving pressure and was struck on the right forearm by rust and debris. Employee received minor first aid, was transported to medical facility. The arm was cleaned and wrapped, given tetanus shot and prescribed an antibiotic.
