Caught in, under or between running or meshing objects

IE drilled a hole, and as they rotated the bolt toward the flushing head, it popped out of the finger. IE walked toward the boom to put the bolt back into the finger, and as they grabbed the bolt, their slicker pants got entangled with the rotating drill steel.

Absorption of radiations, caustics, toxic and noxious substances

while the employee was checking the plant a chemical line leak was not detected before the a small amount of chemical got into the left eye

Struck against stationary object

An employee was walking out of the parts connex when EE stepped onto loose rocks, causing EE to lose footing, and EE twisted right knee. EE was taken to urgent care on 08/09/2024 and was given a full work release with a referral to a specialist. EE saw the specialist, and the specialist scheduled EE for surgery on 12/04/2024.

Struck by rolling or sliding object

Powder crew was in a heading loading a round when a slab of material slipped off the face striking the IP.

Struck by powered moving object

Two employees were in a basket installing a blast mat. The IP went behind the Skytrak to prep another blast mat. The Skytrak operator was unaware that the IP was working behind the operator. The miner in the basket told the operator to back up, and when the operator did, struck the IP.

Struck by falling object

A bolter operator and helper were bolting up a heading. The swellex head was leaking and the helper went out with a wrench to tighten a fitting. While EE was there, material fell out of the back pushed through the wire and fell on the employee.

Fall onto or against objects

Employee tripped and struck face on the bed of the truck causing a laceration to lip.

Struck against stationary object

Driller and the assistant were pulling rods out of the bore hole. The driller unscrewed a drill rod and lowered the now free rod down to the rod rack. The driller removed the lifting bale. The helper grabbed the rod and slid it back to match the position of the other rods. While sliding the rod, EE's right ring finger came into contact with a stationary rod.

On the morning of the May 25, 2021 an employee noticed insect bites on their arm. The employee went to the clinic where medications were prescribed for the bites. The employee returned to work the following day, however May 27, 2021 the employee was not able to work due to reactions to the medications provided for the bites.

Miner suffered an event of unconsciousness while driving onto the mine site. Witnesses described seizure like activity lasting between 15 and 60 seconds in duration.

Accident type, without injuries

During the muck cycle, left rib failed - operator was not in the drift when rib failed.

Struck by falling object

Operator was bolting up 4540 heading. He went to repair a leaking hose when the right rib failed resulting in a buried bolter. He was able to escape over the top of the material.

Struck by rolling or sliding object

While spraying shotcrete the dry pot stopped. The miner stepped away from the face as the dry pot was being worked on. The miner briefly pulled his attention away to check the progress of the dry pot when a rock rolled off the sloped face on to the muck pile and down to the flat. The rock continued rolling in to the miner's right leg resulting in a contusion.

While being loaded, the truck driver was shaken side to side causing pain in the left of his back.

While trying to align striker bar to drill steel, the boom on drill became in a bind, when it came loose from the sill it swung to the left striking his hand (left hand middle finger).

The hammer was inspected prior to use. Was not loose or damaged. Was used several times prior to incident. Hammer had a wooden handle & was 2 years old at least. After looking at the handle after incident, the handle looks to have shrank, causing the head to slip off and strike the employee in the head. Laceration to head 11 stitches. Full release.

Flash burns (electric)

An electrician was testing the power center for an 800hp, 4160v pump. His meter was undersized and became damaged by high voltage and carried a current. The probe arced to the structure and an arc flash resulted causing burns to the electrician. No electricity was contacted by the injured.

Injured was positioning a wood frame door and stepped back slightly for balance. At that point a forklift's right rear tire rolled onto/against his foot.
