Showing all 14Employee reported that a piece of metal fell on EE's right foot. Said EE was okay at the time and didn't think much of it. EE was wearing steel toed boots at the time. *Injury was not reported until 12/24/2022.
Employee claims EE got a hernia at work. EE reported it on 06/28/2022. Employee does not know when it happen, mentioned it happen on April. Did not indicate what EE was doing to cause a hernia.
Employee was walking near service truck, one of the doors to a compartment was open and employee did not see it and kneed the corner of door with knee. Small cut on the center of the knee
Employee complaining of bi-lateral knee pain from climbing onto equipment. Employee did not report incident and it is unclear what day or time it begun.
Cutting nylon cable ties with a utility knife towards EE's right arm, tie was hard for EE to cut. EE put a lot of pressure to cut the tie and when EE cut the tie, EE's left hand went towards the right arm right above wrist and punctured EE's skin. About a 3/8 diameter punctured wound.
Maintenance work was done on conveyor belt. Crew had meeting prior to bump. Employees were instructed to stay in one spot away from the unguarded area when bump was going to be active. Conveyor belt guard was left off during this process. Injured employee walked back to this area and stuck arm back in when the bump was activated.
Employee was kneeling down near a switch gear box. Another worker was getting ready to turn on switch, when EE did it made a noise and startled employee and caused employee to jerk left left arm, pain followed immediately after this in the shoulder.
Employee operating a water truck, drove to pond to fill up. Drive way at pond has some water that was from overfilling water truck. Employee got out of truck, when EE stepped onto the puddle EE didn't realize that there was a pot hole and lost EE's balance and fell down hurting EE's right ankle.
Employee was cutting a conveyor belt with a utility knife, cutting towards EE's right hand. Made the cut and cut the top of EE's right hand, tip of blade went into EE's hand, small laceration.
Employee was laying down on conveyor belt trying to remove a strap that was placed on the side, ratchet on strap was not releasing tension, EE reach over with EE's right arm to cut 1/2 width strap, with a forward and back motion to make the cut employee felt a sharp pain in EE's right shoulder.
Employee was grinding, using a face shield, employee handed bar to a employee on elevated platform, employee on platform grabs the metal bar and pulls it up when ee lost grip of the metal bar, the bar then fell back towards the employee on the ground hitting head, causing a small laceration requiring 4 staples.
Employee was climbing a ladder when EE slipped and fell on the right leg / right side. Employee climbed on the ladder to about 5ft to get to screen platform. Area was watered down right before the employee started climbing and EE's work boots were very muddy. Employee states had three point contact. Employee fractured left Femoral neck bone.
Two man operation, Changing 5/8 screen on # 2 screen feed, employees were placing screen in place on top of screen deck, while pulling screen in place the screen slipped and pinched right pinky finger, between screen and head pulley guard.
