Repairman was inside an electrical control room to program an electrical component for a piece of equipment. Repairman was on knees, tracing a wire down with right hand. Repairman went to stand up, grabbed the bottom of the enclosure to support self, and left wrist touched a 480 volt AC lead.

Showing all 8Employee was cleaning near a tail pulley using a shovel. Employee was not wearing a hard hat at the time. Employee was bent over and when they stood up, they struck their head on a conveyor cross brace.
Miner was clearing crusher blockage, rock fell lacerating neck.
Laborer slipped when descending from conducting drop box maintenance on a 1 1/2 in stacker. Suffered a bad ankle sprain from landing on the ground. No one witnessed the incident. Laborer reported missing foot hold to supervisor, but told doctor at clinic employee slipped from a ladder.
Miner was fueling his Dozer from the Lube truck. When reaching for the nozzle at the completion of fueling, miner felt a pain in his chest. Determined that miner had strained a chest muscle.
Employee says he injured himself on 7/21/15 while attempting to lift a gear box. He said he continued to work "thinking nothing of it." He did not notify us until a week later. Doctor diagnosed him with a strain and performed no treatment, but asked that he return in a week. Last doctor's report on 7/29/15 showed he may have a possible strain.
Gravel screen fell on right thumb causing crushing laceration and fracture.
Employee hooked a chain to a jaw crusher cheek plate to be lifted by an operator using an excavator. The lifting chain's eye slipped, hitting employee in the mouth. Split right side of employee's lower lip, damaging four teeth.
