Repairman was inside an electrical control room to program an electrical component for a piece of equipment. Repairman was on knees, tracing a wire down with right hand. Repairman went to stand up, grabbed the bottom of the enclosure to support self, and left wrist touched a 480 volt AC lead.

Miner was repairing equipment. Other miner went to move excavator that they were using in repair. Injured miner went to stand up. Bucket on excavator was above him. It is unclear whether miner raised head into bucket or if bucket was moved down. Miner hit nose on cross member.

Each of 4 miners on duty confirmed conditions were dry and did not contribute to the accident. Eq. was in proper order at the time of the accident. EE was climbing onto the machine to do a morning pre-op, lost his balance and fell off of the machine. EE went to the emergency room, the doctor did a CT scan which was negative. However the EE fractured both wrists.
