Employee was attempting to lay over a slab of stone in the saw bay with a pry bar. The stone slab weight shifted back as he was pulling. To compensate, the employee pulled harder and felt a pain in his lower back.

Showing all 2Dismounting from 644H front end loader. As he stepped off bottom rung of ladder on loader, he stepped onto uneven ground & twisted right ankle. Ground was soft due to recent snow melt. Had 3 point contact but all of his weight was on his R ankle at the end of dismount. EE had returned to work 3-24-14, but complications from injury had made surgery necessary.
