Employee was attempting to lay over a slab of stone in the saw bay with a pry bar. The stone slab weight shifted back as he was pulling. To compensate, the employee pulled harder and felt a pain in his lower back.

Dismounting from 644H front end loader. As he stepped off bottom rung of ladder on loader, he stepped onto uneven ground & twisted right ankle. Ground was soft due to recent snow melt. Had 3 point contact but all of his weight was on his R ankle at the end of dismount. EE had returned to work 3-24-14, but complications from injury had made surgery necessary.

EE was attempting to remove a piece of stone from a turntable located on the hydrasplit. Stone was elevated about 8-10"" on the turntable. Stone slid off turntable smashing the EE's right index finger between stone and metal rollers on hydra split. The stone weighed approx.75-80 lbs.
Employee was stacking pieces of stone approximately 48""x6""x3"" onto a pallet from a conveyor belt. He had already stacked 20 pieces and stated that as he put the 21st piece on the pallet, he felt a ""pull"" in his lower back as he raised up.
