Employee slipped while standing on a steel bracket mounted to a stacking conveyor. In the course of the fall, the employee's left leg became stuck between the steel bracket and the stacker itself, which fixed that leg in place as employee continued to fall. The combination of the fall and the stuck leg resulted in a fracture to the tibia and a displacement of the patella.

Swinging sledge hammer against pipe. Holding down the edge of the pipe with foot when the bar slipped and struck little finger of right hand, fracturing it.

BARD Materials · Fall from ladders

Individual was standing on a ladder welding a sheet of steel onto a rock bin. He stated that the ladder kicked out from under him causing him to fall and fracture two bones in his foot.

BARD Materials · Struck by falling object

While in the process of disconnecting drive shaft from motor mill when the shaft slipped and came down on the individual's left thumb causing a laceration. Miner was taken to Guttenberg Hospital where he received four stitches.
