Employee slipped while standing on a steel bracket mounted to a stacking conveyor. In the course of the fall, the employee's left leg became stuck between the steel bracket and the stacker itself, which fixed that leg in place as employee continued to fall. The combination of the fall and the stuck leg resulted in a fracture to the tibia and a displacement of the patella.

Individual was standing on a ladder welding a sheet of steel onto a rock bin. He stated that the ladder kicked out from under him causing him to fall and fracture two bones in his foot.

Slipped while stepping up to tighten packing on booster pump for sand plant (approx 2 ft.) causing a torn muscle in employee's right calf

Employee slipped coming down off of steps of an endloader and twisted his knee.
