Employee lost control of 242D skid steer. Employee's negligence resulted in multiple collisions resulting in their LH index finger tip being amputated. Employee reported injury to another the lead person & was asked what happened they stated, "I put my hand where it doesn't belong."

Snow and ice accumulation on roadway surface, created a very slippery walkway. The injured employee slipped and sprained EE's left ankle.

The plant operator was carrying an extension ladder by self, when ee turned rounding a de-watering screen ee tripped and fell against the corner of the screen.

EE and another ee were unbolting a HDPE slurry pipe feeding de-watering screen SC-03A. The HDPE pipe has a slip flange on the end for ease of alignment. When the bolts were loosened the heavy HDPE pipe slipped and turned impacting ee in the chest knocking ee down and breaking collar bone.

Cat off-road dump truck was stuck at tailings storage area. Dozer operator was trying to free the truck from stuck position by using a strap and shackle to pull the vehicle. Shackle failed causing it to hit the dozer operator in the arm. This caused a fracture in EE's right arm. Operator was transported to the Rice Lake hospital with a non-life threatening injury.

Employee was shoveling sand spillage from tail pulley area of D1-C3 Dryer discharge conveyor. Employee turned to the left to dump a full shovel of material and felt a twinge in the lower back and could not straighten upright.

Maintenance employee utilized a pocket knife to cut open a package. The employee was cutting toward himself when the knife slipped cutting his left thumb. The employee was taken to the local emergency room. The cut that measured approximately 3/4" long required 8 sutures to close.
