Employee lost control of 242D skid steer. Employee's negligence resulted in multiple collisions resulting in their LH index finger tip being amputated. Employee reported injury to another the lead person & was asked what happened they stated, ""I put my hand where it doesn't belong.""

Maintenance employee utilized a pocket knife to cut open a package. The employee was cutting toward himself when the knife slipped cutting his left thumb. The employee was taken to the local emergency room. The cut that measured approximately 3/4"" long required 8 sutures to close.
