Lifting drive sprocket and lost grip. Hand was pinned between sprocket and adjustment bolt. Fatty portion of hand was pinched and slit open. PPE was worn to include gloves. Miner received stiches and no time was lost.

Miner was standing on a box attempting to locate a part. As EE stepped down the box caved causing EE to fall back. EE's head struck the floor. EE's initial report of injuries was pain in neck and lower back, along with a laceration to the back of EE's head. EE was transported to Atrium Health for scans and X-rays. Miner was cleared with 4 staples and released for work.

Cutting with a torch, using a grinder, welder, and placing T-Bolts inside Kiln 7 cooler. Debris got into the maintenances workers eye. The maintenance worker noticed the eye irritation around 08:00 pm after being home for a few hours. Most of the work was performed overhead.

EE stepped off a concrete pad (6") and rolled right ankle on 5/13/2025. EE has worked with discomfort in it since then. EE wanted to be seen for the lingering pain on 6/24/2025 and was released to work with weight baring restrictions making this accident MSHA reportable.

Kiln Helper was operating a loader in the coal yard, the loader lunged forward irritating a pre-existing condition. The kiln helper requested not to see a Dr. at the time of the accident. On 6/24/2025 EE went to see the Dr., EE pulled a muscle and was proscribed a muscle relaxer.

Employee was in the kiln shack grabbing a pair of gloves & after a while of wearing them their hand began itching and had swollen up. Harvey believes the glove had a insect in it that had bit them irritating their hand.

EE called another EE around 7:00 am as EE was pulling into the plant explaining IE was not feeling well and a bit lightheaded, When EE went up to check on IE who seemed out of it and confused at which point immediately had other EE call 911 between 7:15 & 7:30 am 01-17-25.

Employee washing out in the kiln tunnel under the cooler while doing so they crouched down to move under the cooler & when they stood up, they said they felt their back catch & pop sending pain through left leg immediately. Employee stated while standing the pain in their leg was mild & when sitting it would become worse.

Weather was clear about 65 degrees. EE was moving 3 steel plates measuring approx. 2'x2' 1/4" thick. Weighing approx. 10 lbs. according to EE. EE stated that EE had tossed them to the side and then felt a "burn" in EE's lower back shooting down EE's leg when EE started walking.

EE stated, EE & two other workers were working in the throat area of #3 cooler when a piece of castable fell from the kiln area above approximately 7 1/2 feet and struck EE on the lower left side of EE's back. The piece of material was approximately the size of a 16.9 ounce water bottle. Not sure where the piece of castable fell from.

EE was using a 24" pipe wrench and mashed finger in between the wrench's handle and another pipe lacerating and fracturing the tip of EE's right ring finger through EE's glove. The laceration required stitches thus making this MSHA reportable.

Weather: clear, no rain. Miner was walking on the walkway between kilns 1/2 & 3/4 and was going up 3 steps and pulled self up with the handrail & felt something pull. EE didn't think anything was wrong but as EE said the pain progressed. So, EE went to the doctor and was diagnosed as a sprained tendon or tear. Has not missed any days for EE's shift.

IE and co-worker went up to the 3/4 stacker feed belt to get material off due to it being overloaded. After they were done IE went to step off onto the ground and when IE did IE's foot caught the side of a rut and their ankle twisted due to the ground being frozen.

EE went to the baghouse on #5 due to the duct work being stopped up. As EE was coming out of the duct work to get back in the man lift, the handle caught a piece of metal which caused it to open up releasing the dust into EE's eye.

Employee was cutting a few feet off a hose to replace a section on a waterline that was leaking. While doing so employee went to cut the piece and that's when they cut their middle finger on the tip of the left hand.

Employee reported they were in the man-lift and man-lift basket got stuck on a piece of concrete. When the basket broke loose from the concrete it knocked them off their feet and they hit their head on a piece of steel causing a laceration on forehead. They report wearing their hard hat. Employee was taken to the ER for evaluation, they required stitches to the forehead.

EE went to number 6 dust seal to attempt to clear it out due to it being clogged. Within 30 seconds or so of beating the chute dust broke loose causing it to start flowing freely. Before EE had time to react, a small hole above EE's head allowed dust to get on EE.

EE walks into the baghouse to see why the auger wasn't working. EE then walks back to the motor and sees the cover where the fan is looked like it had been beaten to death causing the fan not to run due to obstruction. EE was pulling on the belts to get the shaft to turn and that's when the motor kicked on causing EE's hand to get caught in between the pulley and the belt.

EE was driving haul truck from upper pit. There were ruts in the road from the rain the night prior. On EE's way down the ramp, EE hit one of the ruts, it jostled EE around, and EE felt discomfort in EE's back. On Sunday, June 19th, EE went to ER and they prescribed steroids and muscle relaxers and wrote EE out till Wednesday, June 22nd, making it reportable.

While on the lower pier EE was holding the chain from the crane that was wrapped around the housing. While doing so the chain rolled causing EE's hand to roll with it, causing EE's hand to get caught between the chain and the housing.

Employee was leaving the electrical building heading towards number 6 ID fan. While walking across the loose gravel, EE remembers stepping on a rock and feeling a pop in the right knee. EE then tries to put weight on it and move it around but EE couldn't because of the discomfort.

EE was climbing on excavator after changing hydraulic hose to retrieve rags. While climbing down EE missed the last step with EE's foot and fell to the ground. EE landed on EE's right side, EE's hard hat came off and EE hit the back of EE's head on a rock. The wound required 4 staples.

EE changed the rear diff fluid in water truck at shop. EE checked the brakes and low pressure light in the cab to make sure they had pressure. On the way back to the pit, EE stopped several times and knew they had brakes. At some point on the pit road, EE was unable to stop and wrecked the truck at the bottom. EE was fine, but the doctor kept EE out on Monday.

The EE was tramming the drill in the pit. The door was latched open. While moving, the door shut onto the EE's left hand. At the time, the injury consisted of minor lacerations. On 9/7/21, the EE's finger was swollen. We took EE to the doctor and EE had a minor fracture thus making the accident reportable.

Went to 3&4 baghouse to start the lime pump. Employee called for assistance when employee wasn't successful in starting the lime flow. The supervisor came in to see there was no lime flow and the pump was not energized. The supervisor started the pump. The helper was standing at the pressure end of the pump, the hose failed, spraying the helper with lime.

While checking a slow overflow chute on the No. 2 rock silo, the employee placed this hand on the floor to look under a conveyor. A rock from the overflowing chute fell and struck the employee on the finger. The rock made contact with the finger just behind the fingernail at the joint.

EE was tightening the fasteners on the black plastic pipe (48"diameter) for the #1 kiln stack. EE had 7/8" wrench in left hand and a 1/2" impact with a 3/4" socket in right. When they pressed the trigger on the impact, the nut caught a burr on the bolt threads and the wrench spun out of hand and slapped EE across the knuckles.

The injured employee along with two others were locking down large threaded nuts on the coal mill roller journal assembly, using a large open end wrench and a 10lb sledge hammer. The injured EE was holding and adjusted the wrench when the hammer hit EE's middle finger.

Employee was operating a JLG man lift. EE had just emptied some kiln leaf seals from the basket into the dumpster. Traveling back to the job site, they ran over the cut out trench that was designed for water drainage, and when the JLG accelerated their feet lifted from the basket platform. When they came down, chin hit the control panel bar. Also feet are bruised.

Employee was trying to position a sprinkler head on a conveyor using zip tie straps. Seeing that EE had positioned the sprinkler head incorrectly, EE was going to use a utility knife to cut the zip tie. Sometime during this task, EE cut self.

The EE was going to the pit to check on a haul truck driver that said truck was overheating. While in route, the superintendent told the EE's NOT to take the radiator cap off. One of the EE's did, burning the injured EE's left elbow and forearm. The injured EE jumped off of the truck and fractured EE's left heel.
