Showing all 16Employee was cleaning threads out on a D11 track frame. Employee was using a bolt and an impact gun to clean the threads. When employee was done, employee sprayed thread cleaner in the hole. This caused material to shoot out into the employee's eye.
Contactor employee was attempting to get a frozen roller to spin. After hitting it with a hammer and not being successful. EE grabbed it with left hand and tried turning it. The roller then unfroze pulling hand into the roller and belt line.
Contractor employee was using a utility knife when EE slipped and cut left leg right above knee cap.
Contractor employee was checking the antifreeze in bulldozer that was starting to run hot. As EE removed the cap it sprayed EE with hot antifreeze on right hand and arm.
Employee was operating Caterpillar 16G grader. When ee reached down to adjust the seat ee stuck fingers into the blower fan of the heat/ac system. The guard had fallen off. Injuries recieved were on left hand, index finger and middle finger which required stiches.
Employee was stepping down off of a Cat 988 Loader when EE slipped on the step and fell to the ground. Employee didn't receive any injuries from this incident.
Employee was climbing down from a Cat 777 haul truck. When EE slipped from the top step of the ladder. Employee was holding on with EE's left arm when EE fell and hit the bumper of the truck, and then falling to the ground. Employee recieved injury to left arm, resulting in a torn bicep tendon.
Employee was shoveling stemming into the drill holes when he felt a strain in his left arm and shoulder.
Employee was stepping down off of a Cat D11 Dozer when he slipped and fell off backwards to the ground. He landed on his left elbow resulting in a hairline fracture of his arm.
Employee was working on the AC of a Cat 785 haul truck. He was sitting in the cab of the truck, as he reached up to feel the cool air coming out of the vents. He got his middle finger into the blower fan.
Employee was operating water truck watering the mine haul roads. The truck got hot and overheated. When he took the radiator cap off to check the antifreeze level, it sprayed antifreeze on his right forearm.
Employee was climbing up the cable ladder of a Cat 785 haul truck. As he stepped up the ladder swung inwards causing a strain to the back of employee's left knee.
Employee was walking down sidewalk behind shop. When he reached the end of sidewalk he stepped down approx. 5-6 inches to the ground. As employee stepped down he rolled his ankle resulting in a sprained right ankle.
Mechanic was installing jack stands under a dozer using screw drivers to pry up on upper section. The upper section started to slide down, he reacted by putting his left hand on the upper section. The upper section grabbed his glove as it slide down and pulled his middle finger into the area between the upper and lower section. Resulting in a laceration and a fracture.
EE was operating a 992G loader,in the process of removing 3 rock. He was backing away from the wall when a rock fell from the wall. The rock broke through the front windshield of the loader. It struck him in the knees and continued down his left leg, landing on his left foot.
