Contact with hot objects or substances

While troubleshooting a water pump that wasn't pumping, the suction hose was broke open to check for blockage in the hose. Once the connector was broke loose hot water shot from the opening spraying an employee and contractor with the scalding water.

The refuge had began a slow oxidation process which produced moderate heat and smoldering.

The Mack #1 fuel truck was backed up to the South side of the prep plant when the driver finished fueling the portable fuel tank. As the driver was reeling up the fuel hose, there was a pin hole leaking in front of a running heater and ignited. Therefore burning the tail end of the fuel truck and siding of the prep plant.

Fall onto or against objects

Employee was walking up a pile of rocks to where the piece of equipment was parked. When EE's right foot slipped from under EE causing EE to hit right knee and shin on a rock.

Employee was attempting to climb a ladder to turn a valve in the wash plant. Employee slipped from the step and fell to the clear concrete flooring from around 8' above. Employee fell over against a pipe and injured right side of ribs.

Employee was climbing down the Komatsu dozer after checking fluids. EE had EE's right hand on the hand rail, left foot on the tracks. EE stepped with right foot onto the push arm and heel of boot slipped causing EE to fall onto top of right shoulder. The dozer was parked on the freshly removed #3 coal bed, flooring was smooth, flat and level.

Employee was fueling equipment at the parking ditch. As EE was dragging the fuel hose EE slipped in the mud. Employee fell to the ground, EE was complaining of EE's back and shoulder being sore.

Employee was climbing up onto Komatsu 475 bulldozer. When EE caught the toe of the boot on the track causing EE to fall forward. EE landed on right wrist resulting in a small closed fracture.

Fall from machine

Employee was climbing the ladder of Cat 785 Haul Truck to do repair work in the cab. As employee reached for the next hand hold, while employee was stepping up employee missed the hand hold causing employee to loose employees balance and fall.

Employee was stopped on haul road in a Cat 777 haul truck waiting on an approaching Cat 785 haul truck. When the approaching 785 truck was blinded by the sun while leaving the coal yard and struck the parked Cat 777.

Employee driving 777-4 haul truck backed into 777-2 haul truck. Employee in 777-2 haul truck received minor whiplash from the accident. Contributing factors to the accident were low light conditions, raining, and not paying attention to surroundings.

Employee was cleaning threads out on a D11 track frame. Employee was using a bolt and an impact gun to clean the threads. When employee was done, employee sprayed thread cleaner in the hole. This caused material to shoot out into the employee's eye.

Contactor employee was attempting to get a frozen roller to spin. After hitting it with a hammer and not being successful. EE grabbed it with left hand and tried turning it. The roller then unfroze pulling hand into the roller and belt line.

Contractor employee was using a utility knife when EE slipped and cut left leg right above knee cap.

Contractor employee was checking the antifreeze in bulldozer that was starting to run hot. As EE removed the cap it sprayed EE with hot antifreeze on right hand and arm.

Employee was operating Caterpillar 16G grader. When ee reached down to adjust the seat ee stuck fingers into the blower fan of the heat/ac system. The guard had fallen off. Injuries recieved were on left hand, index finger and middle finger which required stiches.

Employee was stepping down off of a Cat 988 Loader when EE slipped on the step and fell to the ground. Employee didn't receive any injuries from this incident.

Employee was climbing down from a Cat 777 haul truck. When EE slipped from the top step of the ladder. Employee was holding on with EE's left arm when EE fell and hit the bumper of the truck, and then falling to the ground. Employee recieved injury to left arm, resulting in a torn bicep tendon.

Employee was shoveling stemming into the drill holes when he felt a strain in his left arm and shoulder.

Employee was stepping down off of a Cat D11 Dozer when he slipped and fell off backwards to the ground. He landed on his left elbow resulting in a hairline fracture of his arm.

Employee was working on the AC of a Cat 785 haul truck. He was sitting in the cab of the truck, as he reached up to feel the cool air coming out of the vents. He got his middle finger into the blower fan.

Employee was operating water truck watering the mine haul roads. The truck got hot and overheated. When he took the radiator cap off to check the antifreeze level, it sprayed antifreeze on his right forearm.

Employee was climbing up the cable ladder of a Cat 785 haul truck. As he stepped up the ladder swung inwards causing a strain to the back of employee's left knee.

Employee was walking down sidewalk behind shop. When he reached the end of sidewalk he stepped down approx. 5-6 inches to the ground. As employee stepped down he rolled his ankle resulting in a sprained right ankle.

Mechanic was installing jack stands under a dozer using screw drivers to pry up on upper section. The upper section started to slide down, he reacted by putting his left hand on the upper section. The upper section grabbed his glove as it slide down and pulled his middle finger into the area between the upper and lower section. Resulting in a laceration and a fracture.

EE was operating a 992G loader,in the process of removing 3 rock. He was backing away from the wall when a rock fell from the wall. The rock broke through the front windshield of the loader. It struck him in the knees and continued down his left leg, landing on his left foot.
