2 techs installing rollers with a forklift. Tech1-aligning the roller while communicating to move the forklift. Using forks on the roller and the alignment tool, 1 of the forks slipped out of the roller crushing tech 1's right hand between the fork and the alignment tool. The Tech2 quickly moved the fork and assisted the first tech.

EE was taking a nut off a bolt on the left guard on Terex 11 with EE's left arm extended when something popped in EE's left shoulder. EE could not raise EE's left arm more than a few inches without pain. EE said the shoulder joint was tender to the touch.
Employee was crossing top of #0 tank and encountered a puddle of water and attempted to "long step" it when EE slipped and strained EE's lower back
