EE was installing a sprocket segment on the track pad and you need to put it at a specific torque. EE was applying pressure to EE's ratchet when the internal gear slipped in the ratchet causing EE's momentum to go forward and hitting EE's face against the track pad.

Showing all 45EE was operating an excavator in Pit 2 at SPS Atascosa. Pit 2 is located between two tailings ponds. 9:18 AM one of the tailings ponds breached causing tailings mixture to flow into the work site. Tailings mixture tipped over the excavator EE was operating cab side down. The front window broke, tailings mixture engulfed the cab & 75% of the vehicle. Employee could not escape.
A mine employee was ascending the line 100 Rotex stairs, when employee's right foot got caught between the steps which caused employee to fall and strike mouth against a step. Injury resulted in four sutures and caused a chipped central incisor tooth.
An Employee was opening the door for the mineral separator (Rotex), the door handle sprung unexpectedly catching the tip of EE's right pinky finger between the handle and door structure causing a laceration that required 6 sutures and an open fracture to the tip of the finger.
During the disassembly while test fitting a bearing to the shaft, the bearing got into a bind and then the bearing slipped out of the workers hands, catching a worker's hand under it and smashed the middle finger on the right hand. the wound was cleaned, bandaged and the worker transported for medical evaluation.
Dry Plant employee was conducting a walkaround when sand coming from the 3886 BC got into EE's right eye. The eye was flushed with water and saline, but the employee continued to feel irritation and requested further medical evaluation at the end of EE's shift. The employee was treated and released to work but was given a prescription for treatment.

The injured employee was changing a bag in the nuisance baghouse and when employee lifted the cage it pinched left ring finger causing a laceration to palm side of the finger. The laceration required 4 sutures. Employee was diagnosed with a laceration of the left ring finger without damage to the nail.
The injured employee's left hand was caught between the V- belt and sheave wheel of a fan while moving the belt to inspect the bearings on the fan motor. The employee was diagnosed with a fracture to the tip of EE's left middle finger.
Injured employee was helping to remove the top screen from a Rotex Mineral Separator. The screen was pulled out too far allowing the weight of the screen to transfer towards the employee. EE was diagnosed with a strained lower back and right shoulder. On 03/02/24 the injured employee was given work restrictions.
An employee was walking on the wet plant concrete pad adjacent to the west side of the process tank when they slipped and fell on to their left shoulder. On 03/04/24 the employee was diagnosed with a contusion and muscle strain of the left shoulder and given work restrictions.
Installing 4" conduit into MCC on site. Limited space for movement in electrical trailer prevented the necessary help to maneuver a pre-fab conduit piece into the gear. Employee lost control of the conduit piece, and it pinched EE's hand against the pre-cut hole it was being positioned into
While descending a fixed access ladder the employee slipped on one of the rungs causing employee to fall below striking tailbone area causing injury. On 11/20/2023 the injured employee was diagnosed with a fracture of the second/ L2 lumbar vertebra.
An employee was helping to re-install a tail pulley guard on a belt conveyor when it slipped catching EE's left ring finger in the expanded metal of the guard causing a fracture of the distal phalanx of the left ring finger.
Employee suffered an impact injury to upper body and legs while being caught between the jeep king pin connection area and fifth wheel hitch of the tractor.
Injured employee bent/knelt down to insert a bolt into a Rotex beam when they felt pain in thier left knee. Initial diagnosis was/is a sprained left knee, the employee was placed under work restrictions.

Injured employee opened the hatch for a sand box. Once back on platform, the cage caught the box lid. While freeing the cage, the truck moved pulling the gangway down the horizontal track leaving an opening in the handrail. The injured employee fell 12½ feet to the asphalt below suffering a head laceration that required 15 sutures, fractured sternum, ribs, and back.
The injured employee was using a Compact Utility Loader to clean up under a feed hopper conveyor belt when EE caught left index finger between the controls handle of the loader and belt structure causing a laceration that required 6 sutures.
The injured employee was standing on a belt conveyor guard changing a gearbox when EE slipped causing EE to fall and strike left elbow on a disconnect box that was in the area. Upon medical evaluation, EE was diagnosed with a fracture to the left elbow.
Employee was working with a 3 man crew dismantling a Caterpillar 390F Excavator. During dismantling, EE was impacted by the counterweight. The other 2 crew members did not witness the accident.
The injured employee was standing in the maintenance shop when a grain of sand blew into left eye around employee's safety glasses. The employee flushed the eye several times. On December 17th the eye was still irritated, so employee went to the emergency room for further medical treatment. No object was found embedded, but was prescribed eye drops for treatment.
On 5/23/22 a maintenance employee was greasing trunnions on a sand dryer when EE felt something in EE's right eye. On 5/25/22 the eye continued to feel irritated, so EE sought medical treatment and a small piece of metal was removed from EE's eye.
An employee was walking in the chemical room to turn off a water hose being used to fill a tank when the employee slipped and fell on the concrete floor injuring back. The employee was diagnosed with nondisplaced left transverse process fractures at L3 and L4 vertebrae.
An employee was trimming a dewatering screen panel with a razor knife when EE cut the tip of right index finger causing a laceration that required 5 sutures to repair.
An employee was removing a shackle from a centrifugal diesel pump lifting lug plate when EE caught the tip of the right 5th (pinky) finger between the shackle and the lifting lug plate. The shackle moved unexpectedly pinching the tip of EE's finger. The injury resulted in an amputation to the tip of the right 5th finger to the first knuckle and required surgery to repair.
Injured employee was pulling a water hose while cleaning up an area at the wet plant. While walking EE stepped on a sand pile causing EE to trip injuring EE's right knee and back. Employee was diagnosed with a sprained right knee and back and was given work restrictions.
An employee was working on replacing a pin for a skid steer bucket quick connect when EE caught the tip of their finger between a bar and the bucket. The employee was diagnosed with a fracture to the tip of the right 5th (pinky) finger.
An employee was injured while descending the stairs from the rotex deck at the Dry Plant. EE was maintaining three points of contact but lost EE's balance towards the bottom of the steps causing EE to tighten EE's grip on the handrail injuring EE's right shoulder. The employee was diagnosed with a dislocated shoulder.
Employee reported injury on 12/23/2019 and was treated at the clinic and released with no restrictions. Employee returned to the doctor and was given mild restrictions that kept employee working. Employee returned to the doctor stating EE was not feeling better. Evaluation made and we could not meet restrictions on the employee.
On 01/07/20 while descending the steps from the primary screener, the employee slipped and fell, striking lower back. Employee did not seek medical attention at the time. On 2/3 the employee sought medical attention for aggravation of this injury, and was placed on restricted work duty. Freezing rain that morning was a factor in this incident.
Employee was closing a truck hatch when EE stepped on a band and rolled ankle. Employee was taken to clinic and a fracture was seen on the x-ray.
Employee was spotting an employee on a ladder when EE stepped back to look up and slipped. EE struck EE's lip and back while EE fell.
A heavy equipment operator was dismounting an excavator at the end of the shift. The bottom access step was covered in mud and caused the employee to slip and land awkwardly injuring left knee. On 11/5/2019 the employee was diagnosed with a sprained MCL and placed on restricted duty as a result of the original injury.
EE was walking across even ground and went to step up onto the truck scale (approx 12-18"). As EE began to lift self onto the platform, EE felt a pop in EE's left knee. Employee was seen by medical personnel the following day and was diagnosed with an unknown internal derangement of the left knee and placed on restriction.
Employee was walking when EE slipped on a loose rock and sprained EE's ankle. The employee was x-rayed and no fracture but due to high ankle-sprain was given an Aircas (boot) to walk in.
Employee was taking off a hatch from a Bucket Elevator, when EE accidentally dropped it on EE's Right Foot. Initially it was diagnosed as a contusion type injury. Upon follow up evaluation, the Physician diagnosed it as a fracture of the 5th digit little toe. This diagnosis was made on 7/10/2019.
Employee slipped on some sand, straining back and right hand. The Employee was medically evaluated by a physician, X-Rays proved negative findings, and the Employee was prescribed muscle relaxants.
Employee was closing a gate valve, at the head box of primary screen. While closing the valve, EE struck left elbow on the flange of the top rail of the head box, causing a laceration that required two sutures. Management was notified on 5-20-19 that the employee received sutures due to this incident.
Employee was replacing a blower on line A when EE was struck on the head with an 18" aluminum pipe wrench that had been dropped from an upper level of approximately 35'- 40'
The employee was cleaning off the waste belt when EE went to turn the water off, and stepped into a small dip and twisted EE's knee and felt a pop. The employee was taken to an offsite clinic and seen and released back to work and followed up with a clinic and at this time was given work restrictions.
Contractor (MSHA ID: B9304) was in the process of changing out a valve and mufflers due to sand build-up. EE was in the process of unscrewing the valve from the air supply line when pressure in the valve propelled it striking the contractor on the left side of EE's face directly in the brow line causing an approximate 1.5" laceration.
On 4/10 EE reported pain in right wrist. On 4/12 EE diagnosed with repetitive use disorder. On or about 7/14/19 disorder determined to be work related. Surgery on 7/26/19.
Muck build up from waste belt under tail pulley. EE was hosing out from under the belt, foot became stuck in the muck, when ee pulled on leg to get out of the muck, and knee popped causing pain. EE wanted to have knee checked out. Was given immobilizer.
