Employee was operating a Front End Loader for several hours. At the end of EE's shift EE felt sore. On 4/30/25 we were notified that the Employee sought medical treatment. EE was Dx with a Left shoulder strain and was given restrictions. Employee was moved to Light Duty. Reporting within 10 days.

Showing all 4Employee was descending from the Superior Guardian Tandem 7203 Screen Plant (Unit #273) using the ladder when EE felt a large pain in EE's left knee and it locked up. EE continued working until around 10 a.m. when EE decided EE needed to have it checked out. Per the doctor EE sprained EE's left knee from planting wrong while descending from the Screen Plant.
On January 19th, 2022 at 8:29 am rocks came off the east highwall striking the employee from approximately 38-42 feet up the highwall. The employee sustained multiple injuries to their head, arm, and leg.
Employee entered dozer cab to verify need for fueling, while looking at gauge door blew shut by a gust of wind trapping left fourth finger causing broken nail and crushing injury to left fourth finger.
