Showing all 21Employee was in the Fabrication shop getting metal supplies for pipe rack when an 8 foot square tubing slipped off rack and struck right foot. EE was taken to medical facility day of injury. EE received second opinion on 11/30/2025 from medical professional where X-ray resulted in fractured #5 Metatarsal. EE is placed on light-duty.
EE was operating the Kubota SCL-1000 Stand-on Compact Loader. The EE was backing up and made contact with a stairway handrail striking left leg. EE was taken to a emergency facility for injury evaluation. EE was placed on two day restriction resulting in leg contusion.
EE was cutting water line hose at wet plant with a utility knife. EE was holding hose up while cutting towards themselves. EE hand slipped at cut their arm. EE was taken to medical facility where they received 4 stitches on left forearm. EE was discharged and returned to work with no restrictions.
On August 5, 2025, at approximately 10:25 AM, an employee was adjusting the forks on an all-terrain forklift when their finger became pinched between the left fork and the frame. The incident resulted in a fracture to the employee's right middle finger. The employee subsequently returned to work on full duty.
EE was cleaning up at the wet plant facility and felt something bite EE under EE's shirt. EE went to a medical facility where EE received treatment for a spider bite on EE's left upper arm.
EE was using a utility knife to cut a media screen. While cutting the screen, the utility knife slipped cutting the top of EE's left thumb. The EE was taken to the emergency room and received stiches.
EE was performing maintenance and repairs on center well of thickener/clarifier, while clearing mud off, the rake shifted and crushed right #3 middle finger resulting in cut/laceration without foreign body. EE was taken to medical facility and received stitches.
EE was assisting moving a portable heater (salamander) when lost balance and crushed pinky finger on RT hand between heater and cross member on rotex deck. EE was taken to ER resulting in finger fracture and nail avulsion.

EE was changing v-belts on sheave at the elevator head pulley. EE's finger was caught between sheave and belt when installing belts. EE received medical attention resulting in laceration and bone fracture on finger tip.
EE was exiting out of a Front End Loader when the door sustained a wind gust shutting the door on the EE left hand between door and frame. EE was sent to hospital that resulted in fractured thumb (XR Finger(1st Digit)2+Left) on left hand due to crush injury. Return to work at full duty.
EE was installing primary media screens when foot was caught between media support deck bars and twisted ankle when losing balance. EE sustained acute right foot pain known as tendinitis. EE received medical treatment resulting in the use of ankle stabilizer boot.
EE was repairing a screen deck motor, chain hoist was used to line bolts for motor and slipped sticking hard hat and EE head causing laceration resulting in 4 staples.

Employee was closing a conex door when a gust of wind blew, causing the door to strike EE's ankle and knock EE down. This resulted in fractures to the top of EE's left foot.
Employee went to re-fuel the pump when the fuel cell sprayed diesel on EE's vest and jacket causing employee to experience a headache, dizziness, and blurry vision. Employee was picked up and went home and later began to throw up and proceeded to visit the hospital where a doctor diagnosed EE as exposure to chemical fumes.
On October 21, 2022 at approximately 6:15 pm EE broke their pinky finger. EE was trying to center the clamps on the Jersey barrier. EE got pinky in a pinch point that closed on EE's pinky finger. EE was then taken to the Hospital by EE's son where the doctor confirmed it was broken. EE was wearing PPE.
EE was opening and closing the 18 wheeler trailer hatches on the Chaulk Mountain Truck Yard and indicated EE pulled a muscle when EE slipped or stepped onto the top of the trailer. The weather was clear. There were no witnesses to this incident. There were no visible signs of injury. The only logical conclusion was a mis-step by EE caused the incident.
On 4/1/222 @ 12:45 pm an EE and IE were tightening bolts on the bottom of the bag house on Dryer 2 when the back up wrench slipped out of other EE's hand striking IE in IE's right eye
Employee was climbing down a ladder from screen deck. When EE reached the bottom of the ladder, EE stepped on soft dirt and twisted ankle. Employee was taken to the hospital due to discomfort in EE's left ankle. X-rays were taken and results show fracture of lateral malleolus of left ankle. Employee will not have any loss of work days.
