Struck by falling object

Employee was in the Fabrication shop getting metal supplies for pipe rack when an 8 foot square tubing slipped off rack and struck right foot. EE was taken to medical facility day of injury. EE received second opinion on 11/30/2025 from medical professional where X-ray resulted in fractured #5 Metatarsal. EE is placed on light-duty.

Struck against a moving object

EE was operating the Kubota SCL-1000 Stand-on Compact Loader. The EE was backing up and made contact with a stairway handrail striking left leg. EE was taken to a emergency facility for injury evaluation. EE was placed on two day restriction resulting in leg contusion.

EE was using a utility knife to cut a media screen. While cutting the screen, the utility knife slipped cutting the top of EE's left thumb. The EE was taken to the emergency room and received stiches.

EE was performing maintenance and repairs on center well of thickener/clarifier, while clearing mud off, the rake shifted and crushed right #3 middle finger resulting in cut/laceration without foreign body. EE was taken to medical facility and received stitches.

EE was assisting moving a portable heater (salamander) when lost balance and crushed pinky finger on RT hand between heater and cross member on rotex deck. EE was taken to ER resulting in finger fracture and nail avulsion.

EE was performing maintenance by installing gasket strip on rotex screen. EE was trimming gasket with utility knife and cut left forearm. EE was taken to ER where EE received 3 stitches.

Over-exertion in wielding or throwing objects

Employee went to re-fuel the pump when the fuel cell sprayed diesel on EE's vest and jacket causing employee to experience a headache, dizziness, and blurry vision. Employee was picked up and went home and later began to throw up and proceeded to visit the hospital where a doctor diagnosed EE as exposure to chemical fumes.

Contact with heat

The EE was performing housekeeping when EE felt ill. EE reported heat stress symptoms to EE's immediate supervisor. The EE went inside the office and recuperated for the remainder of the shift. EE refused to go to the medical facility.

When cycling the lever for the waste silo, the lever was pressurized and it pushed back and struck the employee on the left hand.

Struck against stationary object

The EE was performing housekeeping, when EE struck a crossbeam with head/hardhat and suffered a concussion.

Fall down stairs

The EE was egressing the stairs for the UFR screen, when EE missed the last step and fell. EE struck EE's left elbow on the catwalk grating. The fall resulted in EE receiving a laceration wound, which required 6 stitches to close.

The EE was attempting to open the gate for the Show Barn, when a cow charged the EE and struck EE on the lower/mid section of the right rib cage.

Struck by flying object

The EE was attempting to grease the Haul Truck (HT), when EE heard the grease gun pop and squirt grease into right eye. The EE was wearing prescription glasses at the time of the incident.

Contact with heat

WHILE PERFORMING EE'S NORMAL DUTIES THE EE SHOWED SYMPTOMS OF HEAT STRESS AND WAS TAKEN TO THE URGENT CARE FOR MEDICAL TREATMENT.

Struck against stationary object

The EE was supervising EE's crew when EE accidently bumped EE's right elbow against an I-beam. The EE continued to work until EE noticed the elbow began to swell. EE then reported the accident and was taken to medical clinic for treatment. The result was a chipped bone on EE's elbow.

The EE was lowering the sump mixer to the ground and was holding the impeller. The EE caught left fingers between the ground and the impeller blade, creating a laceration to the index and middle finger. The EE was wearing gloves at the time of the incident.

Over-exertion in lifting objects

Employee was helping change out screens in a rotex screener and then helped with housekeeping around the dryer (shoveling sand). The next morning the employee stated that EE was having soreness in EE's back and wanted to see a doctor.

On February 3rd, 2022, at approximately 7:20AM an employee while working on a piece of equipment in the mobile shop had some sand particles get into EE's eye. EE used the eye wash station to try and clean it out. EE still had eye irritation and stated EE could still feel something in EE's eye. EE was taken to Floresville urgent care for first aid treatment.

Miner was inspecting a pump impellor and they slipped causing a laceration to finger.

Employee was cutting with a grinding wheel and the wheel got caught in the tie string on EE's jacket bringing the grinding wheel to EE's face causing a laceration.

Fall from ladders

Employee climbed a 6' ladder to clean the add water sensor. As EE was descending the ladder, EE lost footing causing EE to fall and strike their side and back on the concrete pad. The employee declined medical attention at this time.

The employee was called to the BC-140 tripper to assist in removing a blockage, while there EE turned to get a shovel, as EE was turning EE's boot caught on the grating causing EE to fall and strike the handrail with EE's left thumb

The employee was opening a 2 inch valve on the water truck, after the valve had been opened employee was called by the Supervisor, when they turned to talk their left arm struck the exhaust of the water truck.
