Showing all 19Employee was working in a quench pit and got boot stuck in 2" of mud. When the employee tried to free the foot the employee twisted / strained knee.
Employee was exposed to arc flash from air arc process, while performing confined space attendant duties. Employees were removing structure holding a CIL screen. Employee had eye pain that evening and returned to work the next morning. Reported to leadman that their eyes hurt from the previous days work. Was taken to doctor and prescribed antibiotic eye drops for infection.
Employee was accessing the cooling pond to retrieve level measurement. The employee stepped on the cooling pond liner with right foot and EE's leg slid out from under EE's body. The slip caused EE to do the "splits" resulting in an injury to their hip and thigh. Employee was assessed at NNRH and was put on restricted duty for a hip/thigh injury.
A Jerritt Canyon employee was task training a Redpath Employee on the operation of a water truck. The trainee was driving the water truck towards West Generator from the water stand. The trainee lost control of the loaded water truck on a downhill ramp resulting in a rollover accident.
A Jerritt Canyon Employee was task training the Redpath employee on the Water Truck. The trainee drove to the water stand and got a load of water with the trainer riding in the passenger seat. As the trainee drove the water truck to it's destination the miner lost control of the loaded water truck and it rolled.
While changing out the slides & guides on Classifier Bucket Elevator, the East Guide was removed without incident. Fellow workers trying to remove the west guide got it out of alignment to remove easily. Injured worker had taken bolts out of the guides on both sides. After removing guide on East side, worker went to west side to assist the other when incident occurred.
Employee was changing out a chlorine tank at the gas handling circuit and after the tank was removed, they went to go get a tool to see if there were any leaks, and employee was exposed to a cloud of chlorine gas.
Employee was crushing samples in sample prep at the lab. The employee's left finger was smashed when the mobile crusher lid closed. The employee was placing a sample bag down when the lid inadvertently smashed employee's finger. The employee sustained a fractured finger as a result of the injury.
Employee was working on a ladder when another employee brought forklift forks down onto the employee pinning EE to the motor. When freed the employee then stepped off the ladder landing on one leg with the other hung up in the ladder twisting EE's back. 7000-1 created 02/09/2022 due to Workers Compensation authorizing physical therapy 02/07/2022 and setting it up 02/09/2022.
Winter Weather, While walking the victim slipped on patch of ice causing injury to right shoulder.
When pushing a reject cart on the pad the cart struck a hole in the ground and stopped suddenly, muscle soreness was noticeable after returning to heavy physical activity.
The employee was going to dump mop water outside of the Security building when employee slipped on a patch of ice from a water drip from the awning and landed on left side injuring left elbow and wrist.
While the employee was walking to the scrubber containment unit by the FL room the employee slipped on mud on the concrete and fell back onto the right wrist twisting back during the fall.
The employee was attempting to clean the front headlight on the right hand side of the loader 42. The employee locked down the equipment and articulated it for easier access. The employee could not reach the light from the lower railing and decided to lean over the railing on top. As the employee was retracting they felt a pop on the right hand side of the ribs.
A fire occurred at the Enfield Bell Mine ID. A surface water truck was dispatched to help put the fire out. The MSHA hotline # was called at 1:42am to report the fire. The fire was reported to be out at 2:06am. The cause of the fire is currently under investigation. No damage to structures or injury to personnel occurred.
The employee was removing glass from a window in the lab when the window broke causing a laceration to the left wrist.
While cleaning CIL containment the employee attempted to lift a wash down hose that was buried in the mud and strained EE's lower back. A full duty release was received at the initial Dr. visit. On July 1, 2021 EE attended a follow up appt receiving a light duty release hence making this injury a medical reportable injury.
The employee was breaking core with a standard rock pick when an employee next to EE was moving a box of core. As this employee moved the box of core EE inadvertently made contact with EE's arm mid-swing causing EE to miss the core and hit EE's thumb.
