Showing all 26EE was shoveling belts on the surface. EE's shovel hit a chunk of ice. At that time EE heard a snap and EE's shoulder gave out.
EE was walking in mud about 15" deep and lost EE's balance. EE's left foot was stuck in the mud and twisted EE's knee while trying to catch self from falling. EE continued to work and on 3-12-25 it was getting worse and we took EE to get it checked.
The continuous miner was parked next to a rib and EE was checking something on the machine while the belts were down. The rib made a little noise and came down. EE didn't have time to move and the rib pushed EE's knee into the miner bruising and cutting the knee.
Employee was shoveling belts on the surface in the cold weather. 2 fingers on their right hand started hurting. it was possibly a slight condition of frost bite.
EE was walking low to avoid the pipes in the belt line. EE hit a fire valve and it pushed EE's hat into EE's nose. EE went to the doctor on EE's own with out informing GMM. after getting the loss run report today I realized that EE had fractured EE's nose.
EE was driving in the 4L intake, drove off of a lip and the inner liner of the cab broke loose and landed on EE's head jamming neck and back
While roof bolting the operator was installing the second steel to continue drilling the hole and they pinched a finger between the steels.
Loading stock on the bolter. Slipped on a piece of brattice. Slipped backwards and felt a pinch in lower back.
EE was behind the line curtain, putting chunks of coal on the brattice. A rib rolled off and struck EE on the left side, fracturing some ribs and spraining EE's right ankle.
While trying to position a plate for a door, the plate slid off the ledge and came down on left foot and bruised EE's foot.

EE was walking past 4 west drive on the off walk side, EE stepped in a hole and twisted EE's ankle
Roof bolting, putting pusher and starter steels together, didn't get connected. put pressure on drill steels, let pressure off and steels came apart and one of the drill steels sprung back and hit EE's hand, cutting it open.
around 12:00 pm right shoulder started to hurt for no apparent reason.
EE was lifting a top cluster off of the rails and the bottom slid off one of the rails and came down and smashed EE's pinky toe on EE's right foot.
The roof bolter drill steel was stuck in the roof. EE was pulling on the steel with EE's hands and when the steel came apart the momentum of the steel caused it to come back and hit self in the mouth. EE's mouth was sore and EE went to the dentist on 11/1 and EE went to dentist again on 11/16. EE quit and we just found out they removed 2 teeth.
On 10/14/2022 EE was pulling cable off of the reel of the roof bolter and felt a sharp pain and a burning in their groin at the end of the shift. EE had a lump in their groin. EE continued to work until 10/22/2022 then EE decided needed to go see a doctor.
Employee was dragging HV cable backwards and tripped over HV nip, HV cable fell on their knee. Employee was originally released with no restrictions. they went to the doctor again and was put on restricted duty on 9/22/22.
tightening bolts on the continuous miner with an allen wrench. The wrench slipped striking and cutting finger.
slipped in a mudhole, tried to twist and jump out, EE's foot was stuck and EE twisted to jump out and EE's knee popped. EE worked at EE's normal duty's and went to doctor again on 7/18. Doctor ordered an MRI. EE is off work until results come back.
EE was taking down a Kennedy stopping, when the panel slid down it cut through EE's glove and cut EE's pinky finger and had to get 5 stitches.
Employee was pre-shifting 1N belt and caught hard hat on the mesh and twisted left knee. Employee heard and felt it pop.
EE got out of the scoop and couldn't put any weight on knee, EE doesn't know what EE did to hurt the knee. It didn't pop or twist, just hurt. EE said EE did have problems with the knee in the past. EE worked 2 more days then went to the doctor the next day. There was some fluid build up in the knee.
Putting water line together, rib fell and struck EE in head and right leg
EE was working on a pump in the intake roadway, bent over to pick up a slick line. EE felt a pain in lower back.
