Showing all 9Employee was using a manual post pounder to install a grounding rod. Pounder came off rod and went in between pounder and employee's hand. Causing laceration to employee's right hand. employee received 6 stitches.
Helper was placing an inner tube on the tube rack. When they slid the inner tube into place, they pinched their right pinky (5th finger) between the inner tube and tube rack.
Employee was mucking out the round and the bucket caught on a divot. This caused the machine to stop abruptly. EE's hard hat fell off and EE hit the side of EE's head on a handle inside the cab. We transported EE to medical facility and EE was prescribed Naproxen for pain from a mild concussion. EE was released to full duty.
The site lost feed power from the electric provider. This rendered the secondary escape hoist inoperable. We evacuated the mine until power was restored.
Employee was walking from parking lot to the dry before shift started. Employee hit EE's head on a window mounted A/C unit. We took employee to Workmed where EE received 9 staples.
when no one was in the area the back fell in at the intersection, miners came to bolt and saw the back on the ground.
Employee was cutting timber and saw kicked out cutting EE's free hand. Employee was taken to hospital and received 5 stitches in left hand middle finger. EE returned to full duty.
Employee was hanging pipe and EE's hand slipped off and EE cut right elbow on a pipe hanger. We cleaned it and put super glue on it. The employee was fine for a few days. 10 days later the employee had an infection. EE was taken to work med and they prescribed Cephalexin. The employee returned to work with no restrictions.
